Book excerpt: On August 13, 2001, a 34-year-old male volunteer fire fighter responded on the first alarm to a working fire in a three-story, 60-unit senior citizens apartment complex. After fire extinguishment and rehabilitation (rehab), the victim was performing a walk-through survey on the third floor during overhaul when he collapsed. Crew members carried him down the stairway and outside the apartment complex where paramedics assessed him and found him to be unresponsive, not breathing, and pulseless. Approximately 43 minutes later, despite cardiopulmonary resuscitation (CPR) and advanced life support (ALS) administered on the scene and at the hospital, the victim died. The death certificate and autopsy record, completed by the Associate Chief Medical Examiner, listed hypertrophic and arteriosclerotic cardiac disease as the immediate cause of death.

At 0408 hours, Truck 313 was dispatched to a working residential structure fire as the third-due truck company. Truck 313 responded at 0414 hours and arrived on-scene at 0420 hours. The crew of Truck 313 was five fire fighters including an acting officer. The chauffeur of Truck 313 stayed with the apparatus. The crew of Truck 313 was assigned as the RIC/FAST company, but the assistant chief from Fire Department 310 advised Command Truck 313 didn?t have the tools for that assignment. Command advised them to use the tools on Ladder 352 for the RIC/FAST assignment. Truck 313 proceeded to Side 3 (Side Charlie) and entered the house without leaving their personnel accountability tags with Command. Note: The fire departments involved in this incident define the sides of a structure by number [e.g., Side 1 (front of the building), Side 2, Side 3, and Side 4] instead of the phonetic alphabet (e.g., Side Alpha, Side Bravo, Side Charlie, and Side Delta). The Truck 313 outside vent man (OVM) had an issue with his SCBA, notified the Truck 313 acting officer, and returned to Truck 313 for another SCBA. The Truck 313 acting officer assigned the probationary fire fighter riding on Truck 313 to stay at the rear sliding glass door on Side Charlie. The Truck 313 acting officer and the Truck 313 Irons fire fighter proceeded into the house with Engine 304.

Department 19 was dispatched for a chimney fire in a residential structure at 1859 hours. Per predefined mutual aid agreements, Department 41 was dispatched for FAST (fire fighter assistance and search team) operations. The Department 41 Rescue Truck 4120, with the second assistant chief, the lieutenant, and an exterior fire fighter on-board, arrived first on-scene at 1910 hours. The Department 41 second assistant chief radioed Dispatch and reported a working fire. The Department 19 chief arrived next, followed by Engine 1910, with a lieutenant and one fire fighter onboard. The Department 19 chief assumed incident command and requested a second alarm. The Department 19 lieutenant pulled a preconnected 13⁄4-inch hoseline to the front door. The Department 41 second assistant chief and lieutenant grabbed two self-contained breathing apparatus from Engine 1910 and joined the Department 19 lieutenant on the hoseline. They donned their facepieces on the front porch, went on air, and proceeded inside and found fire burning up the walls behind a wood stove located near the center of the Side C wall. After knocking down the fire and pulling ceilings, the hoseline crew went outside. The Department 19 lieutenant reported to the incident commander while the Department 41 second assistant chief and lieutenant went to size up the basement. They found a basement door near the A-D corner and entered the basement about 10--15 feet. They found fire burning in the basement at the C-D corner and returned outside to get a hoseline. Soon after re-entering the basement with a charged hoseline, the Department 41 second assistant chief realized his lieutenant was missing and stepped outside to ask if anyone had seen the lieutenant. He reported the lieutenant as missing to a nearby chief. The FAST was activated and rescuers reentered the basement. The lieutenant was found on his knees, facing Side C near an open door leading to a utility room containing active fire. The lieutenant indicated he needed assistance. As the lieutenant was dragged outside, he became unresponsive. He received medical assistance and was transported to a local hospital where he was pronounced dead.

Book excerpt: On April 5, 2013, at 0800 hours, a 57-year-old male career firefighter (FF) began his 24-hour shift. As the Acting Lieutenant for his company, the FF and his crew responded to two emergency medical calls during the day and early evening. The FF spent much of the evening in the officer's bunk room but had ice cream with a crew member before retiring for the evening at approximately 2100 hours. At 0021 hours, on April 6, the FF and his crew responded to an automatic fire alarm at a local college fraternity house. While investigating the cause of the automatic alarm activation, the FF climbed five flights of stairs in full personal protective equipment (PPE), including his self-contained breathing apparatus (SCBA). When the crew returned to the station at approximately 0045 hours, the FF complained of indigestion. The FF was last seen alive at approximately 0100 hours when he retired to his bunk room. At 0656 hours the crew was dispatched to a medical alarm, but the FF did not respond. When the crew returned to the fire house at 0715 hours, the FF was found unresponsive in his bunk room. He had no pulse, no respirations, was cool to the touch, and had signs of lividity and rigor. Per emergency medical service protocol, the FF was pronounced dead on scene. The death certificate, completed by the County Coroner, listed the cause of death as "cardiac arrhythmia, due to myocardial infarction, as a consequence of severe arteriosclerotic heart disease." The autopsy revealed severe coronary atherosclerosis with evidence of acute plaque rupture with hemorrhage; these findings are consistent with an acute heart attack.

At 0446 hours, the officer of Ladder 5 transmitted a Mayday for crews trapped on the 2nd floor. A firefighter from Engine 4 (E402) was separated from the other firefighters on the 2nd floor. E402 was able to get to a bedroom window on the Side Alpha/Delta corner and was removed via a ground ladder. Four other firefighters came out another bedroom window and onto the platform of Ladder 7. A firefighter from Ladder 5 (L502) and a firefighter from Ladder 4 (L403) were separated from the other crews and from each other. Both firefighters moved toward Side Charlie of the fire building. As L502 moved toward Side Charlie, L502 heard an end-of-service time indicator (EOSTI) sounding and found L403. L502 moved toward the other firefighter and asked for L403’s name but heard no answer. L502 started looking for a window to escape because the 2nd floor was getting hot. L502 entered a bedroom and found a window on Side Charlie, broke the window, called a Mayday on the radio, and started yelling for help. L502 retrieved L403, led him to the window and put the firefighter’s hands on the windowsill. L502 then exited the window and descended a ground ladder. L403 did not follow L502 down the ladder. The time was approximately 0455 hours. Rescue efforts were started to remove L403 from the bedroom. Using a rope-haul rescue system, L403 was removed from the building at 0551 hours. L403 was transported to the local trauma center and pronounced deceased. The fire was declared under control at approximately 0651 hours.
