The marked declines in fertility seen in some developing nations have been accompanied by slowing population growth, which in turn provided a window of opportunity for rapid economic growth. For many sub-Saharan African nations, this window has not yet opened because fertility rates have not declined as rapidly there as elsewhere. Fertility rates in many sub-Saharan African countries are high: the total rate for the region is estimated to be 5.1 births per woman, and rates that had begun to decline in many countries in the region have stalled. High rates of fertility in these countries are likely to contribute to continued rapid population growth: the United Nations projects that the region's population will increase by 1.2 billion by 2050, the highest growth among the regions for which there are projections. In June 2015, the Committee on Population organized a workshop to explore fertility trends and the factors that have influenced them. The workshop committee was asked to explore history and trends related to fertility, proximate determinants and other influences, the status and impact of family planning programs, and prospects for further reducing fertility rates. This study will help donors, researchers, and policy makers better understand the factors that may explain the slow pace of fertility decline in this region, and develop methods to improve family planning in sub-Saharan Africa.

Book excerpt: Every developed country has gone through a demographic transition, where both mortality and fertility decline as societies modernize and develop industries. In addition, much of Asia and Latin America have experienced both mortality and fertility transitions. Sub-Saharan Africa, however, is in the beginning stages of the fertility transition. Using data from the Demographic and Health Surveys (DHS), this paper will examine how and where fertility has changed in recent years in sub-Saharan Africa, including changes in national total fertility rates, fertility rates in rural versus urban areas, and differences between capital cities compared to other urban areas and rural areas. It will also consider changes in age-specific fertility rates in the four major regions of sub-Saharan Africa. This paper confirms the findings of Shapiro and Tambashe (2002) that Africa is going through a three-stage fertility transition process, where fertility rates first fall quickly in urban areas, then fertility falls in both rural and urban areas, but faster in urban areas, and lastly when overall fertility levels are relatively low, rural fertility rates declines more quickly than urban fertility rates.

Even within Ethiopia, the second largest country at the Crossroads of Africa and the Middle East, different narratives emerge from analysis of longitudinal, micro-level analysis as to how demographic change and responses are occurring, some more rapidly than others. The book compares Ethiopia with other Africa countries, and demonstrates the uniqueness of an African-type demographic transition: a combination of poverty-related negative factors (unemployment, disease, food insecurity) along with positive education, health and higher age-of-marriage trends that are pushing this ruggedly rural and land-locked population to accelerate the demographic transition and stay on track to meet most of the MDGs. This book takes great care with the challenges of inadequate data and weak analytical capacity to research this incipient transition, trying to unravel some of the complexities in this vulnerable Horn of Africa country: A slowly declining population growth rates with rapidly declining child mortality, very high chronic under-nutrition, already low urban fertility but still very high rural fertility; and high population-resource pressure along with rapidly growing small urban places”

Book excerpt: Fertility in Africa remains the highest in the world, the average total fertility rate for the continent is about 6.3 children per woman. So far little evidence is found of the beginning of a sustained and irreversible fertility decline in sub-Saharan Africa (SSA) of the sort experienced in other developing areas. Contraceptive use is low (except for spacing purposes and outside of marriage) in sub-Saharan Africa, but there is little evidence that this is due to short supply. Reported ideal family sizes remain quite high suggesting that demand for contraception is low. Analysis of the determinants of fertility in Africa using recently available data is likely to provide new insight into the prospects for fertility decline and the design of population policy. Future analysis should focus on four questions that may be answerable using existing data, and may prove useful in evaluating policy and targeting resources : 1) what are the sources and determinants of observed fertility decline in Africa?; 2) what effects does education have on fertility, family size, and contraceptive use?; 3) what are the likely effects of increases in availability and costs of schooling, health care and family planning services on contraceptive use and fertility? and 4) how will these increases affect measures of child survival, educational attainment and anthropometric status?

They have traditionally been viewed as repositories of information and wisdom, and are critical pillars of the community but as the HIV/AIDS pandemic destroys family systems, the elderly increasingly have to deal with the loss of their own support while absorbing the additional responsibilities of caring for their orphaned grandchildren. Aging in Sub-Saharan Africa explores ways to promote U.S. research interests and to augment the sub-Saharan governments' capacity to address the many challenges posed by population aging. Five major themes are explored in the book such as the need for a basic definition of "older person," the need for national governments to invest more in basic research and the coordination of data collection across countries, and the need for improved dialogue between local researchers and policy makers. This book makes three major recommendations: 1) the development of a research agenda 2) enhancing research opportunity and implementation and 3) the translation of research findings.

Book excerpt: Fertility transition, a phenomenon of lowering fertility rates after significant socioeconomic progress, is of great concern to sub-Saharan Africa as a means of curbing dramatic increases in population in the region. Many sub-Saharan African countries are experiencing fertility stalling, however, meaning the total fertility rate (TFR) in these countries is failing to decrease and may be increasing once again. This paper addresses the stalling exclusive to sub-Saharan Africa and attempts to find key socioeconomic, economic, and cultural explanations that may lead to solutions for reinitiating fertility decline in sub-Saharan Africa. Past literature has motivated three approaches to research in this field. First, the paper will examine fertility transition and stalling based on socioeconomic development indicators using data from the Demographic and Health Surveys (DHS). Through the use of multivariate regression analyses at the national, urban/rural, and regional levels, this section will determine socioeconomic factors that influence changes in fertility decline between consecutive DHS surveys in order to better understand how to reinitiate fertility decline in stalling areas. The second approach will address a specific issue to sub-Saharan Africa that may explain its unique fertility trends--the HIV/AIDS pandemic. Although the exact impact of HIV/AIDS on fertility and fertility decisions is yet unknown in the current literature, this section uses a multivariate regression analysis to link adult HIV prevalence to changes in fertility. The third approach will use a recent 2008 Ghana DHS survey to conduct a case study of fertility at the individual level in this country. Ghana used to be experiencing stalling fertility, but as of the 2008 DHS survey, fertility decline has resumed in this region. This section will explore women's age, education level, ethnicity, and type of residence to find possible influences on the number of children ever born per woman.
