Book excerpt: This book analyses the reasons for relying on behavioural contraception methods among urban ‘elites’ in India and examines their efficacy in controlling fertility. It also traces variations in contraception choice over the reproductive cycle of women. Although researchers and policy makers generally equate reliance on behavioural contraceptive methods with low levels of education and awareness and lack of desire to control fertility, this perception has been questioned in recent years. The authors’ analysis of the first three rounds of the National Family Health Survey (NFHS) data in India reveals that behavioural contraceptive methods are popular in eastern India. Moreover, it is urban educated women who rely on behavioural methods, and are apparently able to regulate fertility quite effectively with such methods. NFHS data, however, has some limitations and this motivates the authors to explore birth control methods through primary surveys of currently married graduate women in Kolkata. The use of behavioural contraception methods is a little researched area globally and this is the first book focusing on the topic in India.

Book excerpt: More than 30 years ago, the Institute of Medicine (IOM) and the National Research Council (NRC) convened a committee to determine methodologies and research needed to evaluate childbirth settings in the United States. The committee members reported their findings and recommendations in a consensus report, Research Issues in the Assessment of Birth Settings (IOM and NRC, 1982). An Update on Research Issues in the Assessment of Birth Settings is the summary of a workshop convened in March, 2013, to review updates to the 1982 report. Health care providers, researchers, government officials, and other experts from midwifery, nursing, obstetric medicine, neonatal medicine, public health, social science, and related fields presented and discussed research findings that advance our understanding of the effects of maternal care services in different birth settings on labor, clinical and other birth procedures, and birth outcomes. These settings include conventional hospital labor and delivery wards, birth centers, and home births. This report identifies datasets and relevant research literature that may inform a future ad hoc consensus study to address these concerns.

The book offers an explanation for the continued debate about one road safety intervention - the speed camera - by situating that debate within contemporary literature about the 'risk society' (Beck, 1992) and more broadly understood experiences of risk faced on a daily basis by drivers. Rather than a focus on risk as something that can be objectively assessed, measured and managed separately from the social context in which it is encountered, it suggests that 'risk' is something that permeates this particular debate from every angle. Book excerpt: The Centers for Medicare & Medicaid Services (CMS) have been moving from volume-based, fee-for-service payment to value-based payment (VBP), which aims to improve health care quality, health outcomes, and patient care experiences, while also controlling costs. Since the passage of the Patient Protection and Affordable Care Act of 2010, CMS has implemented a variety of VBP strategies, including incentive programs and risk-based alternative payment models. Early evidence from these programs raised concerns about potential unintended consequences for health equity. Specifically, emerging evidence suggests that providers disproportionately serving patients with social risk factors for poor health outcomes (e.g., individuals with low socioeconomic position, racial and ethnic minorities, gender and sexual minorities, socially isolated persons, and individuals residing in disadvantaged neighborhoods) may be more likely to fare poorly on quality rankings and to receive financial penalties, and less likely to receive financial rewards. The drivers of these disparities are poorly understood, and differences in interpretation have led to divergent concerns about the potential effect of VBP on health equity. Some suggest that underlying differences in patient characteristics that are out of the control of providers lead to differences in health outcomes. At the same time, others are concerned that differences in outcomes between providers serving socially at-risk populations and providers serving the general population reflect disparities in the provision of health care. Systems Practices for the Care of Socially At-Risk Populations seeks to better distinguish the drivers of variations in performance among providers disproportionately serving socially at-risk populations and identifies methods to account for social risk factors in Medicare payment programs. This report identifies best practices of high-performing hospitals, health plans, and other providers that serve disproportionately higher shares of socioeconomically disadvantaged populations and compares those best practices of low-performing providers serving similar patient populations. It is the second in a series of five brief reports that aim to inform the Office of the Assistant Secretary of Planning and Evaluation (ASPE) analyses that account for social risk factors in Medicare payment programs mandated through the Improving Medicare Post-Acute Care Transformation (IMPACT) Act.
