Book excerpt: Biodynamic Research Corporation (BRC) of San Antonio, TX, completed an SBIR Phase I project to port the Air Force's Head-Spine Model (HSM) to a PC-DOS environment and provide a recommended roadmap for the future of the HSM. The impetus for this project was the Air Force's desire to have a software tool capable of modeling the internal forces and motions of the human head and spine during impulsive acceleration events, particularly aircraft ejections. Although models exist to predict the gross motion of a human under acceleration loading such as the Air Force Articulated Total Body (ATB) model, Dynaman, and MADYMO, the Head-Spine Model (HSM) is the only tool able to provide estimates of internal forces. For this reason, the HSM could be valuable to the Air Force and other scientists for simulating acceleration environments.

The HSM was created to aid the solution of problems related to spinal loads resulting from ejection seat acceleration. Unfortunately, the original researchers who developed and used the HSM left the Government and the original code has not been maintained or used in research for several years. Biodynamic Research Corporation contracted with the USAF to create a computer model of the dynamic response of the human head and spine that executes on a PC compatible computer under a Windows environment The HSM-PC is aimed at creating simulations of the biomechanical responses of the head and spine to potentially traumatic impulsive acceleration and impact events.

Book excerpt: The United Nation's Sustainable Development Goals call for the establishment of Good Health and Well-being and target a universal digital healthcare ecosystem by 2030. However, existing technology infrastructure is ineffectual in achieving the envisioned target and requires massive reconfiguration to achieve its intended outcome. This book suggests a way forward with fair and efficient digital health networks that provide resource efficiencies and inclusive access to those who are currently under-served. Specifically, a fair and efficient digital health network that provides a common platform to its key stakeholders to facilitate sharing of information with a view to promote cooperation and maximise benefits. A promising platform for this critical application is ‘cloud technology’ with its offer of computing as a utility and resource sharing. This is an area that has attracted much scholarly attention as it is well-suited to foster such a network and bring together diverse players who would otherwise remain fragmented and be unable to reap the benefits that accrue from cooperation. The fundamental premise is that the notion of value in a digital-health ecosystem is brought about by the sharing and exchange of digital information. However, notwithstanding the potential of information and communication technology to transform the healthcare industry for the better, there are several barriers to its adoption, the most significant one being misaligned incentives for some stakeholders. This book suggests among other findings, that e-health in its true sense can become fair and efficient if and only if a regulatory body concerned assumes responsibility as the custodian of its citizens’ health information so that ‘collaboration for value’ will replace ‘competition for revenue’ as the new axiom in delivering the public good of healthcare through digital networks.

As a result, the variety in methodologies and knowledge paradigms are contrasted, revealing potential gaps and opportunities for integration. Chapters have been contributed by selected authors in the relevant domains of tissue engineering, medical image acquisition and processing, visualization, modeling, computer aided diagnosis and knowledge management. The multi-scale and multi-disciplinary research aspects of articulations in humans are highlighted, with a particular emphasis on medical diagnosis and treatment of musculoskeletal diseases and related disorders. The need for multi-scale modalities and multi-disciplinary research is an emerging paradigm in the search for a better biological and medical understanding of the human musculoskeletal system. This is particularly motivated by the increasing socio-economic burden of disability and musculoskeletal diseases, especially in the increasing population of elderly people. Human movement is generated through a complex web of interactions between embedded physiological systems on different spatiotemporal scales, ranging from the molecular to the organ level. Much research is dedicated to the understanding of each of these systems, using methods and modalities tailored for each scale. Nevertheless, combining knowledge from different perspectives opens new venues of scientific thinking and stimulates innovation. Integration of this mosaic of multifaceted data across multiple scales and modalities requires further exploration of methods in simulations and visualization to obtain a comprehensive synthesis. However, this integrative approach cannot be achieved without a broad appreciation for the multiple research disciplines involved.
