Book excerpt: This secondary analysis identifies and describes the impact of self-reported barriers between purchasing produce at farmers markets and consuming that produce at home. This analysis also compares demographic characteristics between participants who did report barriers to fruit and vegetable consumption and those who did not. Finally, we examine differences in self-reported fruit and vegetable shopping behaviors and consumption between participants who did or did not report barriers, and compare these to the USDA 2010 Dietary Guidelines for Americans. This is a secondary analysis of data collected during an evaluation of the 2013 Fresh Bucks Program, a fruit and vegetable incentive program available at Seattle farmers markets. The original evaluation collected data from a convenience sample of Supplemental Nutrition Assistance Program (SNAP) participants who chose to participate in the Fresh Bucks Program. The Fresh Bucks evaluation surveyed participants at three time points: 1) before they shopped, 2) after they shopped on the same day, and 3) one-to-two months later by phone. Our analysis used data collected from all three of the surveys. These three surveys asked questions related to the purchase, preparation, and consumption of fruits and vegetables. For this analysis, we divided participants into those who reported barriers to fruit and vegetable consumption and those who did not. Differences between the two groups were assessed with student-t and Fisher's exact tests. Barriers and solutions reported by participants were categorized and reported by themes. Of 70 participants, 35 did not report barriers and 35 did report at least one barrier. The most common barriers to full consumption of all produce were purchasing too much produce at once and that the produce spoiled before it could be eaten. Sixty-nine respondents indicated that they consumed more than half or all of their produce, and there was no statistically significant difference in full consumption of all produce those who reported some barriers and those who did not report barriers. Book excerpt: Disparities in the neighborhood retail food environment have been hypothesized to be associated with poor diet quality and adverse health outcomes such as obesity. Studies have found that lower income communities often have reduced access to affordable healthy foods. In 2009, the Centers for Disease Control and Prevention proposed direct farm-to-consumer (FTC) retail outlets (i.e. farmers markets, farm/roadside stands, community gardens and community supported agriculture programs) as a potential strategy to alleviate retail food environment disparities in communities with limited resources. Information on the behavioral and health implications of FTC outlet usage by lower income individuals is limited. This research project aimed to examine FTC retail outlet usage among women who participate in the WIC program in Birmingham, AL. A cross-sectional study design was used to address the following specific aims: Specific Aim 1: Assess associations between residential proximity to FTC retail outlets and usage among WIC recipients. Specific Aim 2: Identify the perceived barriers and facilitators of FTC retail outlet use among WIC recipients. Specific Aim 3: Examine associations between FTC outlet usage, fruit and vegetable consumption and obesity status among WIC recipients. Between October 2014 and January 2015, study participants were recruited from the WIC program office at The Jefferson County Department of Health Central Health Center in Birmingham, AL. Eligible women were Alabama residents, ≥ 19 years old and had received WIC program vouchers in the prior 3 months. After enrollment, participants provided study staff a current residential address and completed the Block FruitVegetable-Fiber Screener and The Survey of Farm-to-Consumer Outlet Use and Produce Shopping Behaviors in Birmingham, AL. Data were entered into a secure Microsoft Access database and analyzed with ArcGIS version 10.2 and SAS version 9.3. Approval for this study was obtained from the Institutional Review Board at UAB and the Alabama Department of Health. Results from this project will provide researchers, policy makers and key stakeholders further insight to the potential FTC retail outlets possess to be a viable strategy to address outcomes associated with healthy food access disparities

Book excerpt: For many Americans who live at or below the poverty threshold, access to healthy foods at a reasonable price is a challenge that often places a strain on already limited resources and may compel them to make food choices that are contrary to current nutritional guidance. To help alleviate this problem, the U.S. Department of Agriculture (USDA) administers a number of nutrition assistance programs designed to improve access to healthy foods for low-income individuals and households. The largest of these programs is the Supplemental Nutrition Assistance Program (SNAP), formerly called the Food Stamp Program, which today serves more than 46 million Americans with a program cost in excess of $75 billion annually. The goals of SNAP include raising the level of nutrition among low-income households and maintaining adequate levels of nutrition by increasing the food purchasing power of low-income families. In response to questions about whether there are different ways to define the adequacy of SNAP allotments consistent with the program goals of improving food security and access to a healthy diet, USDA's Food and Nutrition Service (FNS) asked the Institute of Medicine (IOM) to conduct a study to examine the feasibility of defining the adequacy of SNAP allotments, specifically: the feasibility of establishing an objective, evidence-based, science-driven definition of the adequacy of SNAP allotments consistent with the program goals of improving food security and access to a healthy diet, as well as other relevant dimensions of adequacy; and data and analyses needed to support an evidence-based assessment of the adequacy of SNAP allotments. Supplemental Nutrition Assistance Program: Examining the Evidence to Define Benefit Adequacy reviews the current evidence, including the peer-reviewed published literature and peer-reviewed government reports. Although not given equal weight with peer-reviewed publications, some non-peer-reviewed publications from nongovernmental organizations and stakeholder groups also were considered because they provided additional insight into the behavioral aspects of participation in nutrition assistance programs. In addition to its evidence review, the committee held a data gathering workshop that tapped a range of expertise relevant to its task.

To this end, it is crucial to provide cities with tools to assess the availability, accessibility and use of food outlets and green spaces that facilitate healthy eating and living for urban dwellers. Existing tools to assess food and green environments have been developed and used mainly in high-income countries. This study shows that these tools can and should be adapted to low- and middle-income country settings. This study shows that small neighbourhood food shops are important for household food security, in particular for low- to middle-income households. At the same time, the study shows that consumers are disproportionately exposed to ultra-processed foods (UPFs) in these shops. The policy implication of this finding is that small neighbourhood shops must be incentivized to stock and sell greater amounts of fresh and minimally processed foods to make it easier for low- to middle-income households to adopt healthy diets.

Book excerpt: The persistence of undernutrition and the increasing levels of overweight and obesity worldwide (with their associated societal costs) are calling for a transformation of food systems towards healthier diets. Fruits and vegetables are key components of a healthy diet; however, their consumption is considerably below the minimal levels recommended by the World Health Organization (WHO). This underconsumption is particularly pronounced in low- and middle-income countries and among low-income socio-economic groups in all countries. This paper uses the value chain approach to analyze the factors that affect the availability and affordability of fruits and vegetables. It examines major challenges across the value chain and identifies opportunities for improvement as seen through a nutrition-sensitive lens. Factors that negatively affect the availability and affordability of fruits and vegetables discussed in this paper include low production and productivity, the loss of agrobiodiversity, inadequate technology, logistics and infrastructure, weak organizational, business, and technical skills, and inefficient market linkages across the supply chain. The paper proposes a number of policy recommendations based on insights from documented cases of good practices and on lessons learned in domestic and export-oriented value chains. The paper makes a case for reviving native, underutilized, and neglected fruit and vegetable varieties to improve nutrition and increase agrobiodiversity. In addition, short value chains delivering to local markets are recommended as a resilience strategy for smallscale producers and low-income consumers in the face of climatic and economic shocks.

Book excerpt: This study examined senior citizens' fresh fruit and vegetable purchasing intentions at farmers' markets using the Theory of Planned Behavior. Results showed that attitude, subjective norm, and perceived behavioral control were correlated with intention to purchase fresh fruits and vegetables at farmers' markets. Multiple regression analysis indicated that attitude and perceived behavioral control most strongly predicted intentions to purchase fresh fruits and vegetables at farmers' markets. When comparing the means of seniors with Senior Farmers' Market Nutrition Program coupons and seniors without coupons, the only construct that was significantly different between the two groups was perceived behavioral control. Results of this study may be useful to senior programs and those in the food and nutrition field because it gives insight on factors that influence fruit- and vegetable-purchasing intentions.

Book excerpt: Global fruit and vegetable (F&V) production and consumption are significantly below the threshold needed to meet the FAO and World Health Organization (WHO) recommended F&V intake. In 2017, approximately 4 million premature deaths globally were attributable to insufficient F&V consumption. Globally, very few policy actions specifically promote F&V consumption beyond school-based interventions. Communication and behavior change interventions are insufficient on their own. International evidence suggests that fiscal policies, and policies and programmes that increase access to fresh F&Vs through mobile produce markets can be effective in increasing F&V consumption.
