Book excerpt: Children living in poverty are more likely to have mental health problems, and their conditions are more likely to be severe. Of the approximately 1.3 million children who were recipients of Supplemental Security Income (SSI) disability benefits in 2013, about 50% were disabled primarily due to a mental disorder. An increase in the number of children who are recipients of SSI benefits due to mental disorders has been observed through several decades of the program beginning in 1985 and continuing through 2010. Nevertheless, less than 1% of children in the United States are recipients of SSI disability benefits for a mental disorder. At the request of the Social Security Administration, Mental Disorders and Disability Among Low-Income Children compares national trends in the number of children with mental disorders with the trends in the number of children receiving benefits from the SSI program, and describes the possible factors that may contribute to any differences between the two groups. This report provides an overview of the current status of the diagnosis and treatment of mental disorders, and the levels of impairment in the U.S. population under age 18. The report focuses on 6 mental disorders, chosen due to their prevalence and the severity of disability attributed to those disorders within the SSI disability program: attention-deficit/hyperactivity disorder, oppositional defiant disorder/conduct disorder, autism spectrum disorder, intellectual disability, learning disabilities, and mood disorders. While this report is not a comprehensive discussion of these disorders, Mental Disorders and Disability Among Low-Income Children provides the best currently available information regarding demographics, diagnosis, treatment, and expectations for the disorder time course - both the natural course and under treatment.

Book excerpt: Investigated the longitudinal influences of negative parent-child interactions on girls with and without Attention-Deficit/Hyperactivity Disorder (ADHD). The goals of this dissertation were to (1) illuminate the relationship between negative parent-child interactions (baseline) and negative parent-adolescent interactions (follow-up), (2) examine the predictive validity of both sets of these interactions on the developmental outcomes of girls with and without ADHD, and (3) to identify possible underlying mechanisms of the relationship between negative parent-child interactions and girls' symptomatology at follow-up. The large and diverse sample consisted of 228 girls, between the ages of 6 and 12, with and without ADHD. The girls were recruited to participate in one of three NIMH-funded summer research programs between 1997 and 1999 (baseline) and they were assessed approximately 4.5 years later (follow-up). The follow-up sample consisted of 175 adolescent girls; 110 girls with ADHD and 65 matched comparison girls without ADHD. Maternal and child negativity were indexed by objective observations of parent-child interactions (PCI) collected at baseline and parent-adolescent interactions (PAI) collected at follow-up. Outcomes were girls' externalizing and internalizing symptoms and substance use, measured via parent report and adolescent self-report measures. First, mothers and adolescent daughters with ADHD exhibited more negative and less positive behaviors while interacting compared to the comparison dyads. Second, maternal negativity exhibited at baseline (PCIs) was not significantly related to the behaviors mothers exhibited at follow-up. However, PCI maternal negativity was significantly related to adolescents' engaged behavior and girls' levels of externalizing symptoms at follow-up. Third, maternal PAI factors were not significantly related to girls' levels of externalizing symptoms, internalizing symptoms, or substance use. Yet adolescents who exhibited higher levels of negative behaviors while interacting with their mothers had higher levels of externalizing symptoms, internalizing symptoms, and substance use. Fourth, none of the PAI maternal or adolescent behavior variables measured mediated the relationship between PCI maternal negativity and adolescent girls' outcome variables. Fifth, diagnostic status had a marginally significant moderating effect on the relationship between PAI maternal directive/engaged behavior and adolescent girls' levels of internalizing symptoms. Findings suggest that it may be important to look beyond overt, observable behavior patterns and examine more subtle aspects of parent-adolescent relationships in an attempt to understand pathways to negative adolescent outcomes. A better understanding of the mechanisms by which negative parent-child relationships may influence adolescent outcomes can aid intervention and prevention efforts for families and adolescents children who suffer from the impairments of ADHD.
