Those disparities are caused not only by fundamental differences in health status across segments of the population, but also because of inequities in factors that impact health status, so-called determinants of health. Only part of an individual's health status depends on his or her behavior and choice; community-wide problems like poverty, unemployment, poor education, inadequate housing, poor public transportation, interpersonal violence, and decaying neighborhoods also contribute to health inequities, as well as the historic and ongoing interplay of structures, policies, and norms that shape lives. When these factors are not optimal in a community, it does not mean they are intractable: such inequities can be mitigated by social policies that can shape health in powerful ways. Communities in Action: Pathways to Health Equity seeks to delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity, what actions are needed by the many and varied stakeholders that are part of communities or support them, as well as the root causes and structural barriers that need to be overcome.

Book excerpt: The Department for Energy and Climate Change's (DECC) official CO2 figures - that count territorial emissions from power stations and transport, etc, within UK borders - show nearly 20% reduction between 1990-2009. But research commissioned for the Department for the Environment Food and Rural Affairs reveals that CO2 emissions were 20% higher in 2009 if consumption based emissions - from imported goods - are included. The fall in territorial emissions was not mainly the consequence of the Government's climate policy. Rather it was the result of the shift in manufacturing industries away from the UK and the switch from coal to gas-fired electricity generation that began in the early 1990s. Since 1990 carbon dioxide emissions from imports have almost doubled (from 166 million tonnes (Mt) CO2 to 331 Mt CO2 in 2009). If the UK wishes to encourage emissions reductions in countries that manufacture and export goods to the UK, the MPs say the Government should recognise the growth in the UK's consumption-based emissions. Acknowledging that UK consumption is driving up territorial emissions in other countries could increase the UK's leverage over those emissions and help to secure a binding global agreement on carbon cuts. There is sufficiently robust data available to develop new policy options and identify carbon-intensive behaviours that are overlooked by concentrating on territorial emissions alone. Ministers should explore the options for incorporating consumption-based emissions data in to the policy making process and setting emissions targets on a consumption-basis at the national level.
