Shortly after troops started returning from their deployments, some active-duty service members and veterans began experiencing mental health problems. Given the stressors associated with war, it is not surprising that some service members developed such mental health conditions as posttraumatic stress disorder, depression, and substance use disorder. Subsequent epidemiologic studies conducted on military and veteran populations that served in the operations in Afghanistan and Iraq provided scientific evidence that those who fought were in fact being diagnosed with mental illnesses and experiencing mental healthâ€"related outcomesâ€"in particular, suicideâ€"at a higher rate than the general population. This report provides a comprehensive assessment of the quality, capacity, and access to mental health care services for veterans who served in the Armed Forces in Operation Enduring Freedom/Operation Iraqi Freedom/Operation New Dawn. It includes an analysis of not only the quality and capacity of mental health care services within the Department of Veterans Affairs, but also barriers faced by patients in utilizing those services.

In 2015, approximately 26,500 female veterans, representing 6.5% of all females with service-connected disabilities, received compensation for major depressive disorder (U.S. Department of Veterans Affairs, 2018). However, studies indicate that veterans, inclusive of female veterans, demonstrate low VA mental health services utilization rates for mental health problems, particularly depression (Gorman et al., 2016; Harpaz-Rotem et al, 2014; Miles et al., 2017). Davis et al. (2016) examined the use of mental health care services in 874 female veterans with depression and reported that 43.5% of the women did not receive adequate depression care. Evidence suggests that predisposing attitudes, caretaking, and employment, illness perceptions, alcohol use, and depression influences utilization of healthcare facilities. Based on the Andersen Healthcare Utilization Model, this study examined relationships between predisposing (attitudes, employment, caretaking), need factors (depression, illness perceptions, alcohol use) and utilization of mental healthcare services in 110 female veterans enrolled for care in an urban VA healthcare facility. Results indicated that the majority of the employed women (59%) reported employment barriers and the majority of women (55%) who were caring for either children or adults reported caretaking barriers to utilization of healthcare services. Overall, the women reported positive attitudes toward mental healthcare services, viewed depression as somewhat threatening rather than benign, and reported low alcohol intake. Based on the past twelve months, the majority of women (60%) had 1-8 appointments and 12% had 10 - 21 appointments. However, the majority of women (66%) had at least one "no-show" appointment within the past 12 months. Multivariate analyses indicated that depression and attitudes accounted for 10.9% of the variance in utilization, F = 6.089, p = .003. Average and high levels of depression moderated the relationship between attitudes and utilization, p = .02, R2 = .22.

Book excerpt: Department of Veterans Affairs Office of Inspector General conducted a review to assess the Department's capacity to address combat stress in women veterans. It assessed women veterans use of VA health care for traumatic brain injury (TBI), posttraumatic stress disorder (PTSD), and other mental health conditions, and whether the Veterans Benefits Administration (VBA) properly adjudicated women veterans' disability claims for these conditions. It also assessed whether VBA developed and disseminated military sexual trauma (MST) training and reference materials and policies to claims processors and the feasibility of requiring MST training and testing as part of VBA's claims processor certification. The results show that, with variations in degree, female veterans generally were more likely to use VA health care. They were also more likely to continue using VA health care services, even years after separating from active military service, and to use it more frequently. VA generally diagnosed higher proportions of female veterans with mental health conditions after separation, but lower proportions were diagnosed with the specific mental health condition of PTSD and with TBI. These patterns corroborated our findings from data analyses and from review of claims files that higher proportions of female veterans generally were awarded disability for mental health conditions other than PTSD, and a higher proportion of men were generally awarded disability for PTSD and TBI. Data analyses of the study population indicated that VBA denied females more often for PTSD, and denied male veterans more often for a mental health condition other than PTSD, although the denial rates for male and female veterans for all mental health conditions were almost the same. The study did not find any evidence that claims processors applied VBA's current policies and procedures differently when evaluating male and female veterans' disability claims.

Book excerpt: There are more women enlisting in the military and, as a result, the Veterans Administration (VA) is experiencing an increase in women veteran's utilization of healthcare services. This study examined the factors that facilitate and/or impede women veterans with a substance use disorder seeking VA substance use treatment. The current study examined predisposing, enabling, and need factors related to utilization of VA substance use treatment. An intact dataset of 1004 participants were utilized in addition to a subset of 143 women veterans with a substance use disorder who sought substance use treatment. Predisposing factors significantly differentiated women veterans with and with a substance use disorder. A significant difference was not found between severity of substance use diagnosis and health insurance status. Marital status and socio-economic status were the only predictor variables that significantly predicted women veterans with a substance use disorder and utilization of VA substance use treatment. The results provide mixed support related to previous research. Future directions for research are discussed.

Book excerpt: Women Veterans are the largest growing population Veterans, yet have a significant mental health disparity, greater than both civilian women and Veteran men. This disparity continues in the mental health outpatient treatment options for women Veterans. Veteran Administration (VA) healthcare services may not be suited to their needs as women, and civilian services are not well suited to manage their needs as Veterans. Therefore, this dissertation study used constructivist Grounded Theory methods to explore the experiences of women Veterans when accessing mental health outpatient services, their decision-making process when make the choice to enter mental health service, and aspects of the experience that are important or meaningful to them. Twelve women Veterans revealed meaningful, personal stories on their experiences of trauma and their use of mental health outpatient services. While addressing each of these factors, what emerged from the data was a broader Grounded Theory Process model of how women Veterans process trauma, and the categories of Trauma, Transitions, Identity and Structure. Women Veterans who participated in the study used mental health outpatient services to assist in reestablishing identity after trauma and to propel forward in their lives. This research provides key insight into how women Veterans make healthcare related choices and process traumatic events, like military sexual trauma (MST). This has implications for research, practice, and policy to improve the provision of care for women Veterans.
