Book excerpt: To find out the factors influencing self-care behaviors of diabetic patients treated at the Diabetes Mellitus Clinic, Changhan Hospital, Roi Et Province. It was a cross-sectional study with simple random sampling among 315 samples. Data collection was in December 2009. Descriptive statistics was employed (frequency, percentage, means, standard deviation) and Chi Square test was used to find out the relationship among social-demographic characteristics, knowledge and attitude about diabetes mellitus, and self-care behaviors of diabetic patients. The result found that demographic data of majority of samples was females (72.1%), more than 60 years old (39.7%), married (73.3%), finished Prathomsuksa or lower (88.3%), agiculturers (77.1%), with household monthly income and expenses of less than 2,000 baht (56.5% and 44.8% respectively). The duration of their diabetes illness was less than 5 years (53.3%). Sixty-two point five percent of the patients had no family members with diabetes mellitus history, and 66.7% had people around them to motivate them to control diabetes. Ninety-one point one percent had information on diabetes from public health officers. It also found that knowledge had the relationship with the level of self-care behaviors of diabetic patients. In the future, there should be cooperation of people in the community for diabetes control with a promotion of knowledge and attitudes correctly.

Book excerpt: Controlling type 2 diabetes mellitus requires the individual to perform specific self-care behaviors. The current study investigated the impact of anxiety and depression on these behaviors. In addition, the study investigated the role of sociodemographic variables, sleep and pain in the performance of specific self-care behaviors. Using data collected from primary care clinics providing integrated care to individuals with type 2 diabetics, the Chi-Square Test of Independence was conducted to investigate relationships. This was followed by a series of logistic regressions to identify variables that could predict changes in self-care. Depression and anxiety was measured by scores on the PHQ-9 and GAD-7. Levels of self-care were measured by information collected from patients completing the Summary of Diabetes Self-Care Activities. Findings from this study suggest that self-care behaviors can be influenced by numerous factors: anxiety, age, ethnicity, gender, marital status, and sleep difficulty. Both a correlation and predictive relationship between anxiety and general diet scores were found, but anxiety did not have a statistically significant impact on any other individual self-care behavior. The current study makes a meaningful contribution to the literature by examining the influence of anxiety, depression, sociodemographic factors, sleep and pain on improvements in self-care behaviors.

Book excerpt: People living with diabetes must engage in a range of self-care behaviors to manage their illness in order to prevent future complications. This study examines how specific diabetes complications experienced earlier in the disease trajectory influence the performance of diabetes self-care behaviors. This study utilized a multi-method research design including secondary analysis of two years (2000 and 2007) of quantitative data from the Centers for Disease Control and Prevention Behavioral Risk Factor Surveillance System (BRFSS) as well as 60 semi-structured interviews with older diabetics. Results indicate that the experience of feet sores was significantly related to a greater frequency of feet checking, and having diabetes-related eye problems was related to receiving a more recent eye examination and more frequent checking of blood sugar levels. Results also indicate that the relationship between diabetes embodiment and self-care is moderated by key indicators measuring the level of medical involvement in the treatment of the illness. A time comparison of diabetes self-care behaviors between the years 2000 and 2007 indicated that four of the five diabetes self-care behaviors were engaged in more often or at a higher level in 2007 compared to 2000. Secondary analysis of 60 qualitative interviews with diabetics aged 50 and older indicated that respondents think of diabetes embodiment in similar ways as established by the biomedical model. However, additional themes arose from the data including: do not know, general damage to the body, specific body part or organ damage (other than those discussed in biomedical model), effects on normal body functioning, mental health, and positive influences. Also, some respondents made explicit connections to self-care when talking about what diabetes does to the body. The results suggest that people living with diabetes think of and make connections about their diabetes in ways not fully accounted for in the biomedical model. This study contributes to the self-care literature by being the first to test in a quantitative model the role that specific diabetes complications play in shaping self-care behavior. Contributions are also made to policy and clinical settings through informing policy makers and providers about how and why patients manage their diabetes.

Book excerpt: "The purpose of this descriptive study was to explore the self-care behaviors of college students with diabetes, compare their behaviors to college students without diabetes, and explore the relationship between select demographic characteristics on self-care behaviors of college students with and without diabetes. The target population was college students between the ages of 18-24 who completed the American College Health Association-National College Health Assessment II in spring, 2009. The self-care behaviors examined through an analysis of the survey data were related to food, activity, rest, and prevention of hazards to life as identified in Orem's self-care deficit theory of nursing. An ecological framework was used to describe the demographic characteristics, or basic conditioning factors, at the intrapersonal, interpersonal, and community levels of influence. A total of 1216 students met the inclusion criteria for age, with 528 students reporting they had been diagnosed or treated by a professional for diabetes in the past 12 months. Approximately 63% of the total participants were female and nearly 78% were White, Non-Hispanic. The frequency of students with diabetes meeting the self-care requisites was extremely low, with less than 10% meeting the recommendations for daily fruit and vegetable intake, approximately 30% meeting the recommendations for weekly exercise, and less than 5% achieving adequate rest. Prevention of hazards to life was assessed by examining the frequency of meeting age and gender guidelines for responsible alcohol use and recommendations for weekly alcohol limitations, not smoking, not drinking and driving, and using a seatbelt. Thirty percent of the students with diabetes met the gender guidelines for responsible alcohol consumption and 46% limited their alcohol intake to no more than 1-2 times per week. Seventy percent of the students reported smoking in the previous 30 days. Eighty percent reported they did not drink and drive in the last 30 days, and 70% reported they always use a seatbelt. While students with diabetes reported better outcomes in the self-care requisites of food, activity, and rest, they more frequently engaged in risk behaviors related to alcohol, smoking and unsafe driving practices than students without diabetes. Multiple logistic regression analyses were used to examine the association of intrapersonal, interpersonal, and community levels of influence with meeting each self-care requisites. Predictors of self-care behaviors at the intrapersonal level included gender, race/ethnicity, age, grade average, stress, and health knowledge. Predictors found at the community level were status in Greek organizations, place of residence, and participation in organized athletic. Relationship status and marital status found at the interpersonal level were not found to predict the meeting of any of the self-care requisites. New knowledge on the self-care behaviors of college students with diabetes was developed. Study findings provide insights into the needs for education, further research, and anticipatory guidance from healthcare providers and parents as young people with diabetes transition into college. Study findings may inform interventions that will provide this vulnerable population with greater opportunities for academic success and prevention of long-term complications of this complicated chronic illness."--Abstract from author supplied metadata.
