The majority of CRC is diagnosed in those aged 50 or older. Empirical evidence indicates that early detection of CRC can decease mortality. An annual fecal occult blood test (FOBT) and a flexible sigmoidoscopy every 5 years are the current screening guidelines. Recent studies suggest that the sigmoidoscopy may be a sub optimal approach to screening compared to a colonoscopy. Utilization of these tests is very low, especially among those who are 50 or older and most at risk for developing the disease. The purpose of this study was to identify significant predisposing and enabling factors that predict FOBT and colonoscopy use. The theoretical framework for this study was derived from the Health Belief and the PRECEDE-PROCEED Models. Institutional review board approval was obtained before data collection. The instruments used in the study were previously tested for reliability and validity. Employees of a Midwest-based company were surveyed by mail with a response rate of about 51%. Chi square analysis was conducted to which variables were significantly associated with FOBT compliance (having had one in the last year), ever having had an FOBT, and colonoscopy use. Variables significant at p .25 were entered in to logistic regression analysis. Logistic regression was conducted modeling the odds of having an FOBT or colonoscopy. Those who had an FOBT in the last year were older, reported lower barriers, and reported that their providers had recommended the test to them. Caucasians or those who reported that their providers had recommended the test to them were more likely to have had an FOBT sometime in their lives. Predictors of colonoscopy use were increasing age, having higher benefits, knowledge, self-efficacy, and fear, having lower barriers. Results from this study support previous research with FOBT behavior, although there is no previous research with colonoscopy behavior available for comparison. Future research with a community-based sample is recommended. Results may also provide a guide to developing interventions to increase CRC screening behavior.

Book excerpt: Introduction Fecal-based (FT) colorectal cancer (CRC) screening is designed to prevent cancer or diagnose it early when it is most amenable to treatment. This study assessed the socio-demographic and clinical factors associated with participation and outcomes of a population-based provincial CRC screening program. Methods This retrospective cohort study included individuals age 50-74 who were eligible to participate in Manitoba's CRC screening program from 2007-2014. Sociodemographic factor and clinical variable subgroup analyses were performed using multivariable logistic regression. Results 118,096 screening FTs were completed, and 3081 follow-up colonoscopies were completed. On multivariable analysis, screening participation is associated with age, sex, deprivation index, and geography. Age and sex were associated with identifying advanced neoplasia and non-advanced adenomas on colonoscopy. Conclusions Our study suggests CRC screening participation and outcomes are associated with specific sociodemographic and clinical factors. Through identification of disparities and barriers to participation, access, or timely investigations, we can inform and direct future program initiatives.

Colorectal cancer is the third most common cancer in men and the second most common in women worldwide, and represents more than 10% of the global cancer burden. This publication provides evidence-based evaluations of the effectiveness of colorectal cancer screening in reducing colorectal cancer incidence and mortality. To investigate the prevalence and factors associated with women's use of colorectal cancer (CRC) screening. Analyses were guided by Andersen's Behavioral Model of predisposing, enabling and need factors. Methods. Using the Ontario sub-sample of the 2005 Canadian Community Health Survey, women aged 50-74 who had never screened for CRC (n=3,676) were compared to women who had screened (n=2,105). Chi-square tests and logistic regression analyses were used. Results. Approximately one-third (38.1%) of women had used CRC screening. Predisposing factors (older age, higher education, not working, history of cancer, use of other screening tests), enabling factors (urban location, having a doctor, 5+ doctor visits annually), and, need factors (non-smoking, normal weight, higher activity level) were associated with higher odds of screening. Conclusions. Healthier respondents with lower risk of CRC were more likely to screen, indicating inequitable access to screening. Findings will improve targeting of CRC screening campaigns.
