A health facility based cross-sectional study was conducted from March 16th to 30th 2020. A systematic random sampling technique was applied, in a total of 227 randomly selected women in the reproductive age group who attend Bishoftu’s public health facilities. A structured questionnaire was developed using questionnaires that were applied in different studies related to family planning previously. The data was entered in a computer by using EPI-Info 7 version and cleaned then exported to SPSS version 20. Bivariate and multivariate logistic regression was employed to determine independent predictors of long acting and reversible contraceptive utilization. WHO reports indicated that, in 2017 alone, over 295,000 maternal deaths that resulted from pregnancy and delivery related complication were reported worldwide indicating a decline of 45% from 1990. The sub-Saharan Africa region accounted for 62% of maternal death followed by South-Asian countries (24%). Provision of family planning is one of the most effective interventions that prevent unwanted and ill spaced pregnancies by reducing maternal mortality and morbidity. Given its effectiveness and associated fewer visits to health facilities, LARC are very important in tackling maternal mortality and morbidity. However, little is known regarding its prevalence in Bishoftu eastern Ethiopia.

Book excerpt: Background: Disparities in unintended pregnancy are partially due to ineffective contraceptive method choice among vulnerable populations. Improved understanding of the ecological, individual, and health system related determinants of contraceptive choice can provide guidance for how to reduce ineffective contraceptive method choice among women at high risk for unintended pregnancy. Objectives: Secondary data analysis is performed on visit data from women utilizing Title X Family Planning services at the Cincinnati Health Department's Reproductive Health & Wellness Program (RHWP) to determine the significance of predisposing factors (age, African-American race, education), enabling factors (income, health insurance status, socio-behavioral risks), need factors (having had a recent birth, number of living children) on choosing an ineffective contraceptive method. Mediation analysis is performed to determine if health system factors mediated the effect of these explanatory variables. Methods: Using data from 1,119 RHWP clients who were not seeking pregnancy, multinomial logistic regression is used to compare pill, patch, and ring users, depot medroxyprogesterone acetate (DMPA) users, and long-acting reversible contraception (LARC) users with a reference group of ineffective method users. Multinomial logistic regression is first performed with all independent variables except health system mediation. Then, it is performed with inclusion of health system mediation. Multiple linear regression analysis is performed to determine significant relationships between independent factors and health system mediation. Mediation analysis is performed for any independent variable that is significantly correlated with both contraceptive method choice and health system mediation for the purpose of determining if any witnessed mediation effect is statistically significant. Results: The model including health system mediation is found to account for more variance in the data than the model excluding health system mediation (Nagelkerke R-squared = 0.195 and 0.158, respectively). Through both models, and in all three comparisons, higher age is found to reduce the odds of choosing a more effective method with only minor mediation effects. Having more children results in higher odds of both DMPA and LARC uptake in both models and is unmediated by the health system. Being of African-American race results in lower odds of LARC use in both models - a finding that is also not mediated by the health system. Health system mediation effects are found in the LARC and DMPA comparisons and not in the pills, patch, or ring comparison. In the DMPA comparison, inclusion of health system mediation eliminates income and insurance status as significant explanatory variables. In the LARC comparison, inclusion of health system mediation eliminates the explanatory significance of having a recent birth. Conclusions: Analysis of the role of health system mediation suggests that health system level factors play a large role in explaining contraceptive choice among contraceptive methods that require increased health care utilization. The impact of health system mediation on the significance of other predisposing, enabling, and need variables implies that health care access, utilization, and quality are important factors that should be included in future models for understanding contraceptive choice.

Long-acting, reversible contraception (LARC) is highly effective at pregnancy prevention than other modern reversible methods subject to user error and reduces their unintended pregnancy and adverse maternal and child health outcomes. Yet, there may be gaps in providing a LARC supportive environment in some contexts. Characterizing current contraceptive LARC users could provide insights on how to better support LARC as a contraceptive option among women initiating contraception. We examined correlates of LARC debut among a cohort of current LARC users, as well as risk factors for unwanted pregnancy among all modern, reversible contraceptive users. Methods: we conducted a secondary cross-sectional analysis among 1,124 current modern? contraceptive users seeking family planning services in Northwest Ethiopia. Women were classified as LARC users if they used implants or intrauterine devices or non-LARC users if they used other modern, reversible methods (condoms, injectables, or oral contraceptives), at contraceptive debut. Potential correlates of LARC use at contraceptive debut among current LARC users, and wantedness of prior pregnancy (defined as last pregnancy among women with multiple pregnancies) among other modern contraceptive users, included sociodemographic characteristics, reproductive history, and family planning decision making. Odds ratios adjusted for age, marital status, education, and wealth were reported. Results: Overall, 330 women in the study were currently using LARC; among these 74 (22%) reported LARC was used at contraceptive debut and 118 of 817 (14%) women with a prior pregnancy reported their prior pregnancy was unwanted. Factors associated with using LARC at contraceptive debut were older age (≥35 years) (aOR=3.71; 95% CI: 1.49-9.17), being married before age 18 (aOR=0.46; 95% CI:0.20-0.97), and richest wealth quartile (aOR=3.02; 95% CI: 1.24-8.24).Factors associated with a prior unwanted pregnancy were younger age (15-24 years) (aOR=2.25; 95% CI: 1.39-3.63), being single (aOR=12.74; 95% CI: 7.52-21.88), not using the preferred method of choice (aOR=3.17 95% CI: 1.17-8.21), healthcare provider selecting current method (aOR=4.33; 95% CI: 1.55-11.73) and reporting future fertility desires were "undecided" (aOR=2.61; 95% CI: 1.53-4.36). Conclusion: Factors associated with LARC at contraceptive debut imply an environment supportive of contraceptive choice. Women who debuted with LARC may have been exposed to national programs that focused on expanding LARC access, and sociodemographic characteristics such as age at marriage and wealth could point to a LARC supportive environment. Factors associated with a prior unwanted pregnancy suggest an environment with varying degrees of support for contraceptive choice. Demographic characteristics, provider selected method, and pregnancy ambivalence could point to a nonsupportive environment, whereas fertility intentions and recognizing limitations of prior method point to a supportive environment. Future research is needed to capture the setting and context more accurately at contraceptive debut, and implications of this environment on unwanted pregnancies.
