Cette étude avait pour objectif de rechercher, dans une population de mères consultant dans les centres départementaux de Protection Maternelle et Infantile du Val de Marne (PMI), les facteurs liés à l'allaitement maternel (AM) à 1 mois et à 4 mois. Méthode. Cette étude prospective portait sur l'ensemble des mères de nourrissons consultant en centres de PMI et présentant les critères d'inclusion. Les mères ont été interrogées par un questionnaire administré aux 1 mois de l'enfant, qu'il soit allaité ou non. Les mères allaitant leur enfant à 1 mois été interrogées à 4 mois par un nouveau questionnaire. Des données ont été recueillies sur la mère et son entourage, la grossesse et l'accouchement, le mode d'alimentation et de garde, et le soutien à l'AM. Résultats. 179 mères ont été incluses du 15 juin au 31 décembre 2015. Les facteurs en lien avec l'AM à 1 mois en analyse multivariée étaient l'allaitement d'un enfant précédent (OR=90,1[6,7-2961]) et l'absence de difficultés de mise en place de l'AM (OR=0,2[0,03-0,8]). Les facteurs en lien avec l'AM à 4 mois étaient l'exercice à temps partiel de la mère (OR=2,87[1,1-174]) et l'information donnée sur l'AM depuis les 1 mois de l'enfant (OR=2,48[2,3-312,1]). Discussion et conclusion. Les résultats permettent d'orienter préférentiellement les actions d'information et de soutien de l'AM envers les femmes ne présentant pas de facteurs de poursuite de l'AM : les primipares, les mères n'ayant jamais allaité ou ayant eu des difficultés de mise en place de l'AM. Il est important d'informer les mères sur l'AM après le 1er mois, notamment sur les moyens de poursuite de l'AM à la reprise de l'activité professionnelle.

In clinical settings, these are two important blood electrolytes, are frequently measured and influence care decisions. Yet, blood electrolyte concentrations are usually not influenced by dietary intake, as kidney and hormone systems carefully regulate blood values. Over the years, increasing evidence suggests that sodium and potassium intake patterns of children and adults influence long-term population health mostly through complex relationships among dietary intake, blood pressure and cardiovascular health. The public health importance of understanding these relationships, based upon the best available evidence and establishing recommendations to support the development of population clinical practice guidelines and medical care of patients is clear. This report reviews evidence on the relationship between sodium and potassium intakes and indicators of adequacy, toxicity, and chronic disease. It updates the Dietary Reference Intakes (DRIs) using an expanded DRI model that includes consideration of chronic disease endpoints, and outlines research gaps to address the uncertainties identified in the process of deriving the reference values and evaluating public health implications.
