La sédentarisation croissante contribue à une augmentation démontrée de la morbidité et de la mortalité. C'est dans ce contexte que la promotion des APS est devenue un véritable enjeu de santé publique s'inscrivant dans la plupart des recommandations médicales actuelles. La prescription d'APS sur ordonnance en est un outil majeur. Objectif principal : Déterminer les facteurs de résistance à la pratique des APS des patients et les freins à la prescription de ces APS. Méthode : Pour réaliser cette étude observationnelle, descriptive, nous avons diffusé un questionnaire par courrier électronique entre le 01 août 2016 et le 30 septembre 2016 à l'ensemble des médecins généralistes du département d'Indre-et-Loire dont nous disposions une adresse électronique valide, soit 361 médecins généralistes. Les analyses ont été effectuées via le logiciel R. Lorsque les effectifs l'exigeaient, les tests de Chi2 et Fisher ont été employés, afin de tester l'indépendance de deux variables qualitatives. Résultats : Nous avons obtenu 154 réponses au questionnaire sur les 361 médecins inclus dans l'étude, soit un taux de réponse de 41%. Si certains médecins faisaient déjà la promotion (conseil minimal oral pour 93% des médecins interrogés) voire la prescription de l'activité physique (84% en prévention primaire et 72% en prévention tertiaire), ils utilisaient rarement des outils lors de leurs consultations (85% des médecins n'en utilisent pas) et collaboraient peu avec les professionnels du sport adapté (19%). La mise en place de la prescription individualisée sur ordonnance dans la pratique quotidienne était de leur ressort en tant que médecin généraliste pour 93% d'entre eux mais prenait trop de temps (66%) et était limité par leur connaissance sur ce sujet (68%). Enfin, les facteurs de résistance à la pratique des APS pour les patients étaient le faible niveau socio-économique (61%), de faibles revenus (51,5%), l'éloignement des structures sportives (50%) et le manque d'éducation (42%). Conclusion : La prescription d'APS adaptée est une thérapeutique non médicamenteuse validée. Ce travail confirme l'intérêt et l'adhésion des médecins généralistes d'Indre-et-Loire à la prescription de l'APS. Cependant certains freins à la prescription sont limitants d'où la nécessité de développer des formations continues et des outils pédagogiques simplifiés.
