Book Extracorporeal Photopheresis

Download or read book Extracorporeal Photopheresis written by Hildegard T. Greinix and published by Walter de Gruyter. This book was released on 2012-10-01 with total page 228 pages. Available in PDF, EPUB and Kindle. Book excerpt: Extracorporeal photopheresis (ECP) is the first FDA approved cellular photoimmunotherapy for cancer, namely cutaneous T-cell lymphoma, and has demonstrated efficacy in various diseases that have a suspected involvement of pathogenic T-cells including prevention and treatment of acute and chronic graft-versus-host disease, organ transplant rejection, selected autoimmune diseases and Sezary syndrome. In ECP, patient’s blood mononuclear cells are collected, exposed to ultraviolet light in the presence of extracorporeally administered liquid 8-methoxypsoralen and reinfused. Besides its considerable efficacy ECP has an exceptional safety profile, does not cause general immunosuppression and thus, does not increase risk of infectious complications and relapse of malignant disease. Exciting preclinical data and clinical observations provide insight into the mechanisms of action of ECP and support its immunomodulatory role. This book will provide an overview on the historical development of ECP, technical aspects for its use in various patient populations including children and adults, novel research findings both in preclinical models and in various patient cohorts and will stimulate further research in organ, tissue and cell transplantation and autoimmune disorders.

Lu00f3pez Corral.Complejo Asistencial Universitario de Salamanca-IBSAL, Hematology, Salamanca, SpainKEY WORDS: GVHD, Extracorporeal Photopheresis, alloSCT.CONTEXT: GVHD is the most common complication after alloSCT. Steroids are the first line but more than 40% of these patients will not response.OBJECTIVE: Evaluate efficacy and safety of ECP for refractory GVHD.DESIGN: retrospective study of 84 patients treated with ECP between january-2010-april-2018 in our centre.PATIENTS: 84 patients with steroid-refractory acute/chronic GVHD after alloSCT. Patients who didnu00b4t complete 8 procedures were not evaluable.INTERVENTIONS: an integrated system of continue flow, processing a standard volumen. Patients with aGVHD received 2 consecutive sessions/week during 12 weeks, then 2 sessions/fortnight until stop steroids. Protocol for cGHVD was 2 sessions/fortnight during 12 weeks and 2 sessions/month during 3 months until stop steroids.MAIN OUTCOMES MEASURES: ORR, OS and security.RESULTS: -aGVHD(n=35): median age of 48 (11-71) and 65.7% men. Organ involved was skin (91.4%), gut (71.4%) and liver (40%). 15 patients present grade II, 3 grade III and 17 grade IV. ORR was 78.1% (CR:53.1%). All of patients with grade II-III responded, but only 10 of 17 patients with grade IV GVHD. Best organ response was skin (ORR:87.1%). Grade IV had and adverse outcome (median OS not reached VS 3 months, p=0.016) and CR after ECP was associated with favourable outcome (median OS not reached VS 6 months, p=0.061). With a median follow-up after ECP of 5 months (0-51), 51,4% live.-cGVHD(n=49):, median age of 55 (9-69) and 54% men. 24 had severe cGVHD. Principal organs involved were skin and mucosas. ORR was 69%(CR:24.4%). Responses of moderate was superior (80%). Best organ responses were oral (ORR:84%) and skin (ORR:80%). After a median follow-up of 29 months (3-96), OS after ECP at 2 years was 78.3%.- Security: 8 CMV reactivation and 2 fungical infections ocurring during ECP. Steroids could stop in majority of the patients. 6 GVHD reactivacion were observed during ECP, the majority respond with intensification.CONCLUSIONS: ECP is an effective and safe salvage therapy for GVHD, especially in grade II-III aGVHD and moderate cGVHD. Best organ responses are skin in aGVHD and skin and mucosa in cGVHD.

Book Chronic Graft Versus Host Disease

Download or read book Chronic Graft Versus Host Disease written by Georgia B. Vogelsang and published by Cambridge University Press. This book was released on 2009-04-20 with total page 427 pages. Available in PDF, EPUB and Kindle. Book excerpt: Chronic graft versus host disease (GVHD) is the most common complication of allogenic bone marrow transplantation. Because of the protracted clinical course of chronic GVHD, transplant centers and hematology/oncology offices are inadequately equipped to manage these immuno-incompetent patients with a multi-system disorder. Practitioners need to be able to recognize and effectively manage chronic GVHD as a late effect of more than half of allogenic transplantations. The text is oriented for the clinician, with chapters covering staging, organ site and system-specific manifestations, treatment options, and supportive care. Drs Georgia B. Vogelsang and Steven Z. Pavletic have been pioneers in the recognition of the multi-organ complexity of this disease and have gathered the input of a variety of subspecialist physicians for this book. This book fills the gap in practical literature on chronic GVHD, providing a comprehensive, up-to-date, and clinically relevant resource for anyone who deals with cancer patients post-transplant.

Book New Approaches to Improve Extracorporeal Photopheresis for the Treatment of Graft versus host Disease

Download or read book New Approaches to Improve Extracorporeal Photopheresis for the Treatment of Graft versus host Disease written by and published by . This book was released on 2016 with total page 218 pages. Available in PDF, EPUB and Kindle. Book excerpt: Graft-versus-host disease (GvHD) is a severe complication occurring after hematopoietic stem cell transplantation (HSCT). It is caused by alloreactive T cells contained in the graft, and associated with severe morbidity and mortality. Glucocorticoids (GC) are the gold standard for first-line treatment of GvHD but once patients become steroid-refractory, they need to be treated with second-line therapies such as extracorporeal photopheresis (ECP). ECP is an immunomodulatory therapy currently used in the clinic for treating patients suffering acute GvHD (aGvHD) or chronic GvHD (cGvHD). During...

Book Extracorporeal Photopheresis in Acute and Chronic Steroid refractory Graft versus host Disease an Evolving Treatment Landscape

Download or read book Extracorporeal Photopheresis in Acute and Chronic Steroid refractory Graft versus host Disease an Evolving Treatment Landscape written by Hildegard Greinix and published by . This book was released on 2022 with total page 0 pages. Available in PDF, EPUB and Kindle. Book excerpt: Abstract: Patients with steroid-refractory graft-versus-host disease (GvHD) are known to have a poor prognosis and for decades no approved drug has been available to treat this serious condition. Although ruxolitinib, a selective Janus kinase (JAK)1/2 inhibitor demonstrated significantly higher response rates in randomized trials compared to the best available therapy, and thus, is of benefit in both acute as well as chronic GvHD, there is an urgent medical need to improve results, such as durability of responses, response in eye, liver and lung manifestations and reduction of infectious complications. In this "Review" article we would like to offer strategies for improving treatment results in patients with steroid-refractory GvHD by combining ruxolitinib with extracorporeal photopheresis (ECP), a leukapheresis-based immunomodulatory treatment frequently applied in T-cell mediated immune disease including GvHD. Our article explores key published evidence supporting the clinical efficacy of both ruxolitinib and ECP in the treatment of GvHD and highlights their potentially complementary mechanisms of action
