Book excerpt: It is estimated that over 30,000 applicants to registered nursing (RN) programs are turned away each year following a rigorous selection process for the limited available program admissions. An alarming number of those students who are accepted and enter upper division programs and depart prior to completion do so because of increased stress and anxiety experienced during clinical activities. Attrition occurs when students repeatedly experience levels of stress and anxiety and feel they have reached a breaking point. Self-efficacy impacts the amount of stress and anxiety an individual can tolerate before encountering negative outcome expectations. The theoretical framework for this study was underpinned by Bandura's (1995) theory of self-efficacy. A multi-case study approach and cross-case analysis was utilized to identify if a link existed between simulation and self-efficacy. Beginning nursing students engaged in a simulated clinical experience prior to the initial acute care clinical experience. Nursing students discoursed about physical and sociostructual influences on agency and environment related to self-efficacy and negative affective behaviors. Based upon students' discourse, this study supports a link does exist between simulation and self-efficacy for beginning nursing students.

The book focuses on describing and explaining salutogenesis as an umbrella concept, not only as the key concept of sense of coherence.

Book excerpt: Psychomotor nursing skills are a fundamental part of curricula for all schools of nursing no matter the degree level of the program. Demands placed on schools to produce a generalist nurse with the ability to safely practice in the clinical setting is high. There is no standard method of teaching or evaluation regarding psychomotor nursing skills, and there is much room for studies to demonstrate best practice. On top of the already mentioned high demands placed on schools of nursing are the demands of content-saturated nursing curricula on the students. Nursing students are placed under stress-producing situations that create high levels of anxiety. Schools expect students to perform at very high levels and to demonstrate competency of skills learned in a practicum setting. There is a need for best practice in teaching and evaluating psychomotor nursing skills. There is also a need for experienced faculty to teach the skills. The purpose of this exploratory study was to evaluate student levels of self-efficacy in the performance of psychomotor nursing skills after implementation of the learning strategy self-directed simulation. Self-directed simulation is a strategy created by the researcher in response to students' level of anxiety and uses theoretical knowledge of ways to decrease student anxiety and increase learning and retention. Self-efficacy was measured in 94 students from a Midwestern school of nursing who were enrolled in a fundamentals of nursing course. Additionally, the researcher conducted 14 qualitative interviews to further investigate the self-directed simulation strategy, self-efficacy, and learning.

Book excerpt: Simulations are frequently used techniques for training, performance assessment, and prediction of future outcomes. In this thesis, the term “human-centered simulation” is used to refer to any simulation in which humans and human cognition are integral to the simulation’s function and purpose (e.g., simulation-based training). A general problem for human-centered simulations is to capture the cognitive processes and activities of the target situation (i.e., the real world task) and recreate them accurately in the simulation. The prevalent view within the simulation research community is that cognition is internal, decontextualized computational processes of individuals. However, contemporary theories of cognition emphasize the importance of the external environment, use of tools, as well as social and cultural factors in cognitive practice. Consequently, there is a need for research on how such contemporary perspectives can be used to describe human-centered simulations, re-interpret theoretical constructs of such simulations, and direct how simulations should be modeled, designed, and evaluated. This thesis adopts distributed cognition as a framework for studying human-centered simulations. Training and assessment of emergency medical management in a Swedish context using the Emergo Train System (ETS) simulator was adopted as a case study. ETS simulations were studied and analyzed using the distributed cognition for teamwork (DiCoT) methodology with the goal of understanding, evaluating, and testing the validity of the ETS simulator. Moreover, to explore distributed cognition as a basis for simulator design, a digital re-design of ETS (DIGEMERGO) was developed based on the DiCoT analysis. The aim of the DIGEMERGO system was to retain core distributed cognitive features of ETS, to increase validity, outcome reliability, and to provide a digital platform for emergency medical studies. DIGEMERGO was evaluated in three separate studies; first, a usefulness, usability, and facevalidation study that involved subject-matter-experts; second, a comparative validation study using an expert-novice group comparison; and finally, a transfer of training study based on self-efficacy and management performance. Overall, the results showed that DIGEMERGO was perceived as a useful, immersive, and promising simulator – with mixed evidence for validity – that demonstrated increased general self-efficacy and management performance following simulation exercises. This thesis demonstrates that distributed cognition, using DiCoT, is a useful framework for understanding, designing and evaluating simulated environments. In addition, the thesis conceptualizes and re-interprets central constructs of human-centered simulation in terms of distributed cognition. In doing so, the thesis shows how distributed cognitive processes relate to validity, fidelity, functionality, and usefulness of human-centered simulations. This thesis thus provides a new understanding of human-centered simulations that is grounded in distributed cognition theory.

Book excerpt: Introduction: Interprofessional clinical simulation has been introduced as the new standard for healthcare education within which peers are able to collaborate about patient care in a real-life scenario. This type of educational tool has been linked to higher confidence in skill and communication practice. However, there has been a lack of regarding the peer interaction as a tool to precept these scenarios. This study describes the effects of peer learning in the context of interprofessional clinical simulation. Methods: This study used a pre/post-test survey design to assess self-efficacy changes as well as post-test likert-type scaling and open-ended reflection responses. Additionally, it utilized a strengths survey assessment for half of the participants. Results: A total of 29 students from the Respiratory Therapy program participated. Two-tailed Paired t-tests were utilized to assess changes in self-efficacy post-simulation demonstrating that students felt a statistically significant increase in most self-efficacy items for both the peer preceptor and peer learner including increased confidence in motivation, coping with conflict and communication skills. Additionally, independent sample t-tests were used to determine if there was any difference in perception of the peer learning experience based on role. There was no statistically significant difference based on role in any of the items assessed. Qualitative data suggested themes of confidence and perspective as being key components to a successful peer learning experience. Conclusions: Peer learning in the interprofessional clinical simulation environment leads to benefit for both the peer preceptor and peer learner. Participation in interprofessional clinical simulation results in increased self-efficacy and enhancement of leadership and communication skills which may impact future practice.

Book excerpt: Abstract : A dietitian must earn the credentials of Registered Dietitian Nutritionist to be employed as a clinical dietitian in the hospital and public health settings (Academy, 2013). According to the Bureau of Labor Statistics, the job outlook for registered dietitians is expected to grow 16% by 2024 (U.S. Bureau of Labor Statistics, 2016). In recent years, it has been increasingly difficult to find Registered Dietitians who are willing to serve as preceptors for dietetic students, leading to a shortage of internship sites (Thompson & Gutschall, 2015). Therefore, dietetic programs are forced to find other means of training, such as simulation for students to achieve dietetics competencies required for their credentialing. Simulation, specifically high-fidelity simulation, offers a real-world setting in which students could learn and achieve competencies. This study explored the use of simulation in dietetics education as it has been used in nursing education and other fields. It also investigated the role of technology acceptance in the successful implementation of simulation as it relates to self-efficacy and student-centered learning. Self-efficacy and student-centered learning were explored for their connection with achievement of clinical competencies in dietetics; particularly, the nutrition focused physical exam. The achievement of dietetics competencies with the use of simulation could implicate the effectiveness of simulation in dietetics education.
