Book excerpt: Middle-aged and older African American women report the lowest rates of physical activity compared to men, younger women, and similarly aged White women, as well as higher death rates from conditions associated with physical inactivity. Though inactivity among African American women has received increased attention since the early 1990s, research regarding how older African American women successfully increase and maintain their physical activity levels is lacking. The current study aimed to identify factors associated with successful adoption and maintenance of physical activity among active midlife and older African American women. The study also assessed the applicability of the Physical Activity Evolution model as an explanatory framework for physical activity development among this population. Open-ended individual interviews were conducted with 15 African American women aged 45 years and older who reported engagement in physical activity at nationally recommended levels for 6 months or more. Data from the interviews were analyzed using deductive qualitative analysis methods. Results suggest that using scheduling strategies, receiving support from others, and experiencing noticeable benefits of physical activity are key factors in older African American women's progression toward a physically active lifestyle. These findings were consistent with the Physical Activity Evolution model.

Book excerpt: Given the high risk of relapse during the first year of exercise involvement, it is important to determine the processes of self-motivation which enable novice exercisers to become long-term maintainers. This study was designed to extend previous Stages of Change (Prochaska & DiClemente, 1983) research by comparing the Possible Selves (Markus & Nurius, 1986) of individuals at different points within the Maintenance stage of exercise. Participants consisted of female university employees, spouses, and dependents age 35-59 years who volunteered for the study. Participants completed the Stage of Exercise Scale (SOES; Cardinal, 1995) and a self-administered form of the Possible Selves Inventory (Cross & Markus, 1991) which was adapted to address the exercise domain. Women classified by the SOES as being in the Maintenance stage of exercise V=92) were assigned to one of three groups based on the duration of their exercise maintenance (6 months-5 years, 6-10 years, and 11-20 years). The three maintenance groups were compared with regard to the number and category of open-ended and exercise-related possible selves and the self-efficacy and outcome expectancy associated with three focused selves (most important, exercise-related, and exercise-specific). The results indicated that the three maintenance groups did not differ significantly in their possible selves. These findings provide support for the current conceptualization of the Stages of Change Model (Prochaska & Di Clemente, 1983) and suggest that differences between novice and expert maintainers may be behavioral, rather than cognitive, in nature. Several implications for intervention design and suggestions for future research are discussed.

A lack of activity increases the risk of heart disease, colon and breast cancer, diabetes mellitus, hypertension, osteoporosis, anxiety and depression and others diseases. Emerging literature has suggested that in terms of mortality, the global population health burden of physical inactivity approaches that of cigarette smoking. The prevalence and substantial disease risk associated with physical inactivity has been described as a pandemic. The prevalence, health impact, and evidence of changeability all have resulted in calls for action to increase physical activity across the lifespan. In response to the need to find ways to make physical activity a health priority for youth, the Institute of Medicine's Committee on Physical Activity and Physical Education in the School Environment was formed. Its purpose was to review the current status of physical activity and physical education in the school environment, including before, during, and after school, and examine the influences of physical activity and physical education on the short and long term physical, cognitive and brain, and psychosocial health and development of children and adolescents. Educating the Student Body makes recommendations about approaches for strengthening and improving programs and policies for physical activity and physical education in the school environment. This report lays out a set of guiding principles to guide its work on these tasks. These included: recognizing the benefits of instilling life-long physical activity habits in children; the value of using systems thinking in improving physical activity and physical education in the school environment; the recognition of current disparities in opportunities and the need to achieve equity in physical activity and physical education; the importance of considering all types of school environments; the need to take into consideration the diversity of students as recommendations are developed. This report will be of interest to local and national policymakers, school officials, teachers, and the education community, researchers, professional organizations, and parents interested in physical activity, physical education, and health for school-aged children and adolescents.

Book excerpt: Ethnographic methodology was used to explore the factors that influence middle-aged and older Latin American women's participation in physical activity. Semi-structured interviews and field observations were used to elicit information from the twenty five Latin American women in this study sample. Perceptions of health, the health activities in which the women engage, and the factors that influenced their participation in physical activity comprised the three categories of responses. Facilitators and barriers were identified as the two primary groups and were further sorted into intrinsic or extrinsic factors. A sense of self, decreased feelings of stress, wellbeing, managing chronic disease, the desire to lose weight, and a personal health event comprised the intrinsic subcategory. Extrinsic facilitators identified were social support, music, health information, family history, and accessibility. Barriers (intrinsic) were physical illness or disability, pain, fatigue, lack of self motivation, worry, and embarrassment, whereas the extrinsic barrier group included role demands, such as childcare and household tasks, time limitations, environment, which included weather, and unsafe neighborhoods, inaccessible programs, lack of social support, and limited knowledge of risk factors/disease management. Conclusions of this study were that the Latin American women in spite of their multiple role demands and other barriers did participate in health promotion behaviors, such as physical activity. This study sample of Latin American women identified salient facilitators to participation in physical activity which warrant further qualitative exploration. This study adds to the small but growing body of knowledge of Latin American women's health practices and offers opportunities for the development of culturally sensitive interventions that would promote regular physical activity in this population.

Book excerpt: During the last 25 years, life expectancy at age 50 in the United States has been rising, but at a slower pace than in many other high-income countries, such as Japan and Australia. This difference is particularly notable given that the United States spends more on health care than any other nation. Concerned about this divergence, the National Institute on Aging asked the National Research Council to examine evidence on its possible causes. According to Explaining Divergent Levels of Longevity in High-Income Countries, the nation's history of heavy smoking is a major reason why lifespans in the United States fall short of those in many other high-income nations. Evidence suggests that current obesity levels play a substantial part as well. The book reports that lack of universal access to health care in the U.S. also has increased mortality and reduced life expectancy, though this is a less significant factor for those over age 65 because of Medicare access. For the main causes of death at older ages-cancer and cardiovascular disease-available indicators do not suggest that the U.S. health care system is failing to prevent deaths that would be averted elsewhere. In fact, cancer detection and survival appear to be better in the U.S. than in most other high-income nations, and survival rates following a heart attack also are favorable. Explaining Divergent Levels of Longevity in High-Income Countries identifies many gaps in research. For instance, while lung cancer deaths are a reliable marker of the damage from smoking, no clear-cut marker exists for obesity, physical inactivity, social integration, or other risks considered in this book. Moreover, evaluation of these risk factors is based on observational studies, which-unlike randomized controlled trials-are subject to many biases.

Book excerpt: Because many women miss out on the benefits of exercise, it is important to understand how to encourage their exercise behavior. This study explored provisions of social support as they relate to the transtheoretical model (TTM) stages of change to determine if the importance of perceived general social support provisions and exercise specific social support, both actual and perceived, varied by stage. Further, the association between the stages of change and other variables shown to be related to exercise behavior (i.e., motivation, exercise self-efficacy, and BMI) was explored. The study identified significant differences for the positive social support of reliable alliance. Specifically, women reported higher levels of reliable alliance when in the maintenance stage compared to the preparation and contemplation stages. There were also significant differences by stage for general perceived exercise social support. Women in the contemplation stage reported the lowest level of social support for exercise compared to the other stages. Further, women in the preparation stage reported significantly lower levels of social support for exercise compared to women in the action and maintenance stages. No significant differences were found for actual exercise social support behavior (i.e., working out with another person). There were significant differences between stages for extrinsic and intrinsic motivation and exercise self-efficacy. In addition, BMI levels showed a significant interaction effect related to exercise behavior and age. Specifically, older women in the study reported higher BMI levels. However, this age effect is not present in the maintenance group. Overall, the findings from the current study suggest that it is important to consider multiple variables when attempting to encourage the maintenance of exercise behavior in women.
