Book excerpt: Evaluation of clinical skills is a demanding and complex process and is dependent on many complex factors, such as teaching and learning approaches, simulated learning, and psychometrically validated assessment tools. Therefore, it is imperative that adequate strategies and methods are employed to evaluate the success of a nursing care activity. One such strategy in the field of nursing care is the application of objective structured clinical examination (OSCE) of a nursing activity. The purpose of this article is to highlight the importance of evaluating nursing activities in a simulated clinical environment with OSCE to determine synchronicity of the teacher and student assessment. A cross-sectional study was carried out, in which we compared the evaluation of nursing activity by the teacher and the 51 students. Summative content analysis was used to analyze open-ended questions about possible improvement of performed nursing activity. The data showed a large discrepancy (81.9%) in evaluating nursing activity between the teacher and the student. The synchronicity between the teacher and student assessment modality occurred only in 18%. Students were mostly less successful in their assessment of competence with knowledge about carrying out interventions (36.5%), preparing for interventions (24.3%), and infection control (14.4%). Clinical skills acquisition remains an essential element of a student nurse's development, as competence in nursing skills is essential to patient safety. Simulation is viewed as an increasingly popular approach to the teaching and assessing of clinical skills. The process of evaluating nursing activity demands the usage of objective instruments that require objectivity, fairness, impartiality, and comprehension. The use of OSCE is one such method of promoting reliable and valid assessments in nursing skills.

Book excerpt: Purpose of the Student Transitional Experience into Practice (STEP) study was to explore differences in perceptions of practice readiness between associate degree student nurses and their preceptors after a clinical capstone experience. Significance: Commonalities are reported by student nurses and newly licensed registered nurses facing the daunting task of becoming competent clinical practitioners. The novice nurse has reported that exposure to the workplace setting, professional roles, and acquisition of psychomotor skills aided in their perception of practice readiness and perceived competency level. Nursing educators seek innovative teaching modalities that assist in producing sound, competent generalist graduate nurses. Graduate nurses need to be able to demonstrate theoretical competency on the NCLEX-RN and be able to function independently as a registered nurse once in practice. Method: This STEP study was a pilot quantitative, retrospective design employing secondary data analysis of associate degree nursing students at a multi-campus Midwest community college. Instruments: The Casey-Fink Tool measured perceptions of student nurse practice readiness and their preceptors’ perceptions of student readiness following the final clinical capstone experience. The survey instrument is a three-factor set of correlated subscales: demographics/clinical experience, competency skill performance, and professional identity of the student nurse. Procedure: This STEP study utilized collected data from the Casey-Fink Tool survey data obtained during the spring 2018 clinical capstone course from 100 students and preceptors. Analysis: A descriptive analysis of the Casey-Fink Tool survey data, including exploratory factor analysis, was used to identify subscales and study findings. Nursing Implications: This STEP study advances the future of nursing education by exploring curricular methodologies to aid in the preparedness and practice readiness of the student nurse upon graduation.

Book excerpt: Qualitative naturalistic descriptive methodology was used to describe how pre-licensure nursing students and clinical nurse educators perceive students learn to clinically reason in the clinical education setting; and, to learn how clinical time is utilized to develop clinical reasoning. Semi-structured interviews were conducted among ten students who were enrolled in junior level courses in traditional and accelerated baccalaureate nursing programs and eight clinical educators, who taught junior level clinical courses, at the same Midwestern university school of nursing. The findings from this study revealed rich data that warranted analyzing student and educator perceptions separately, though some similar ideas emerged. Using thematic analysis, 93 thematic categories from student nurse perceptions and 71 categories from clinical educator perceptions were discovered with six (6) overarching themes emerging, three from each participant group. The themes were from student perceptions were: 0́−Tying It All Together,0́+ It0́9s All About Learning, and The Environment Influences Learning. The themes from clinical educator perceptions were: Instructor at the Core, Clinical Educator Perceptions Differ, and Approaches to Teaching.This study found that students perceive the development of clinical reasoning (1) is profoundly influenced by the clinical educator0́9s level of commitment to their success; (2) is shaped by teaching strategies that are grounded in sociocultural, constructivist, and adult educational theory that promote higher order thinking; and (3) is encouraged by supportive learning communities of discourse that encourage peer collaboration and focus on the tenets of the nursing process while utilizing reliable resources. Clinical educator perceptions reveal they (1) do not recognize the influence they may have in developing clinical reasoning and professionalism in nursing students; and (2) are committed to student learning and value humanistic behavior but are inconsistent in implementing the described effective educational strategies. Both students and clinical educators perceived clinical site inclusiveness to highly influence the learning environment. Educator to student ratios and medication administration policy were identified as potential barriers to educator availability for students.This information can guide nurse educators in developing and implementing strategies, within the clinical education setting, that cultivate clinical reasoning among their nursing students; it can also help nursing students foster clinical reasoning among themselves. This information can also aid nurse educators and facility administrators in developing collaborative relationships that promote environments conducive to the development of clinical reasoning within a variety of clinical sites in which pre-licensure nursing students may one day be employed.
