Book excerpt: This cross-sectional study characterizes a group of high-risk youth's HIV knowledge (Brief HIV-KQ) and cognitions (maladaptive, coping appraisal, and adaptive cognitions), as well as aspects of their sexual behavior and condom use. Descriptive statistics and regressions were used to analyze archival data collected from 794 youth (46.3% male and 53.7% female) ages 16 – 24 (M = 18.66; SD = 1.95) who attended an educational and vocational training program in a large urban area of the Mid-South. Results indicate that these youth were twice as sexually active as the national average yet used condoms more frequently. There were gender and age differences in sexual behaviors, condom use, and HIV knowledge. Future interventions with these youth should be directed toward correcting knowledge deficits and increasing risk perception and behavioral skills acquisition. .

Book excerpt: Young females are at three times higher risk of contracting HIV than their male counterparts in Jamaica. Using Jamaica's 2004 Knowledge, Attitudes, Behaviour and Practices survey, this research investigates factors contributing to HIV/AIDS-related risky behaviors of young females. Risk perception as a function of knowledge and as an influence on behavior is also examined. The findings reveal that only older females, 25 to 49 years, practice safer behaviors in response to increased knowledge. This highlights the disjunction between knowledge and behavior, emphasizing the need for a holistic approach to addressing the social context within which adolescents are put at risk.

Studies indicate that the incidence of HIV infection and the prevalence of high-risk behaviors among young populations continue to rise in developing countries. AIDS has severe economic impacts, especially in high-risk countries such as Nigeria. This project used secondary data from the 2003 Nigeria Demographic and Health Survey. The 2003 NDHS is a cross-sectional, multistage, stratified survey with systematic sampling that was designed to obtain information on key population and health indicators such as sexual activity, knowledge about AIDS and other STDs, and behavior associated with AIDS and other STDs for the entire Nigerian population, including people living in rural areas, urban areas, and six major geographic regions. Analyses were restricted to sexually active adolescents and young adults 15 to 24 years of age who are Nigerians living in Nigeria in 2003. The sample size was (n=2,418), of which 2,009 are females and 409 are males. Non-Nigerians and Nigerians in Diaspora were excluded in the original design of the survey. Also, those younger than 15 years or older than 24 years were excluded in this study. This research examined the association between independent variables and other covariates with risky sexual behaviors such as lack of condom use and multiple sex partners, as well as possible difference by gender. Odds ratios (ORs) were calculated using multivariate logistic regression models, adjusting for age, education and religion. All tests were done at 95% significant leve. l-- Abstract.

Book excerpt: High-risk sexual behaviors in the adolescent population greatly increase their risk of obtaining a sexually transmitted disease (STD) and/or human immunodeficiency virus (HIV). The Centers for Disease Control (2002) estimate that there are currently 20 million people in the world with HIV and 15 million people become infected with a STD every year. HIV is considered to be the seventh leading cause of death for children between 5 and 14 years of age and the sixth leading cause between 15 and 24 years of age (Rogers, 2000). A pretest-posttest research utilization project was completed to determine whether and educational experience would be effective in changing high-risk behaviors in the adolescent population. Fifty-three students from a high school health class and five parents participated in the intervention focusing on increased knowledge and improved communication skills between parents and adolescents regarding sexual decisions and prevention of STD's and HIV. The program was successful based upon a change in the decision to remain abstinent until marriage from 59% to 83% on the posttest. Communication was another area that demonstrated successful intervention; the parent as the main source of information about sex increased from 48% to 62%. Ninety-eight percent of the students recommended the program for future students. Success of the parent class was determined from qualitative remarks obtained, the change in preference from an abstinence only message for their adolescent to an abstinence and condom use message, and the participants feeling more prepared to talk to their adolescent about sex, STDs and HIV.

STIs are often asymptomatic (especially in women) and are therefore often undiagnosed and unreported. Untreated STIs can have severe health consequences, including chronic pelvic pain, infertility, miscarriage or newborn death, and increased risk of HIV infection, genital and oral cancers, neurological and rheumatological effects. In light of this, the Centers for Disease Control and Prevention, through the National Association of County and City Health Officials, commissioned the National Academies of Sciences, Engineering, and Medicine to convene a committee to examine the prevention and control of sexually transmitted infections in the United States and provide recommendations for action. In 1997, the Institute of Medicine released a report, The Hidden Epidemic: Confronting Sexually Transmitted Diseases. Although significant scientific advances have been made since that time, many of the problems and barriers described in that report persist today; STIs remain an underfunded and comparatively neglected field of public health practice and research. The committee reviewed the current state of STIs in the United States, and the resulting report, Sexually Transmitted Infections: Advancing a Sexual Health Paradigm, provides advice on future public health programs, policy, and research.
