Mais dans quelles situations cliniques courantes un médecin généraliste peut-il être amené à pratiquer l'hypnose médicale ?. Notre objectif est de réaliser une photographie objective des Résultats de Consultation (RC) de l'hypnose en médecine générale. Nous avons réalisé une étude observationnelle transversale auprès de 90 patients consultant 14 médecins généralistes aquitains formés à l'hypnose. L'enquête à été menée sous forme d'un entretien individuel structuré, par un unique observateur externe. Près de 89% des RC de l'hypnose sont liés à des problèmes psychiques ou psychosomatiques. Les deux plus fréquents sont "l'anxiété-angoisse" et le "tabagisme", indépendamment du sexe. Les douleurs ne représentent que 6% des motifs de consultations. Les patients ont en moyenne 43,8 ans et sont majoritairement des femmes (64%). Ils utilisent l'hypnose sur les conseils d'une tierce personne et essentiellement en seconde intention ou en traitement adjuvant. Ils sont motivés par les résultats positifs de l'hypnose et son innocuité. Les médecins sont âgés en moyenne de 54,6 ans. Ils pratiquent plusieurs médecines alternatives. Ils décrivent un épanouissement personnel et professionnel grâce à l'hypnose, et une amélioration de leur exercice et de la relation patient/médecin. Des enquêtes à plus grande échelle sont nécessaires pour préciser l'utilité de l'hypnose en médecine générale. La création de registres d'hypnothérapeutes faciliterait la coordination des soins pour les patients et les médecins correspondants. Un cotation des actes d'hypnose dans certaines indications et un remboursement par les caisses d'assurance maladie permettraient un accès plus égalitaire à ces thérapies. Une information sur les médecines alternatives pourrait être intégrée dans le cursus universitaire des futurs médecins.

Book excerpt: Sigmund Freud infamously referred to women's sexuality as a “dark continent” for psychoanalysis, drawing on colonial explorer Henry Morton Stanley’s use of the same phrase to refer to Africa. While the problematic universalism of psychoanalysis led theorists to reject its relevance for postcolonial critique, Ranjana Khanna boldly shows how bringing psychoanalysis, colonialism, and women together can become the starting point of a postcolonial feminist theory. Psychoanalysis brings to light, Khanna argues, how nation-statehood for the former colonies of Europe institutes the violence of European imperialist history. Far from rejecting psychoanalysis, Dark Continents reveals its importance as a reading practice that makes visible the psychical strife of colonial and postcolonial modernity. Assessing the merits of various models of nationalism, psychoanalysis, and colonialism, it refashions colonial melancholy as a transnational feminist ethics. Khanna traces the colonial backgrounds of psychoanalysis from its beginnings in the late nineteenth century up to the present. Illuminating Freud’s debt to the languages of archaeology and anthropology throughout his career, Khanna describes how Freud altered his theories of the ego as his own political status shifted from Habsburg loyalist to Nazi victim. Dark Continents explores how psychoanalytic theory was taken up in Europe and its colonies in the period of decolonization following World War II, focusing on its use by a range of writers including Jean-Paul Sartre, Simone de Beauvoir, Octave Mannoni, Aimé and Suzanne Césaire, René Ménil, Frantz Fanon, Albert Memmi, Wulf Sachs, and Ellen Hellman. Given the multiple gendered and colonial contexts of many of these writings, Khanna argues for the necessity of a postcolonial, feminist critique of decolonization and postcoloniality.
