Available in PDF, EPUB and Kindle. Book excerpt: Colorectal cancer (CRC) is the second leading cause of cancer related death in the U.S. among men and women combined. In addition to the personal toll it takes, CRC is estimated to cost billions of dollars annually in medical costs. Due to the high rates and costs of CRC, a significant public health effort has been made to promote participation in screening, which has been shown to be effective at reducing CRC incidence and mortality rates; however, despite efforts to promote screening and research showing its effectiveness, screening rates remain low. Approaches to increasing screening participation focus on understanding the decision-making process people go through regarding screening. Health economics and health psychology provide two unique approaches to understanding this decision-making process. Health economics approaches model screening behavior as a function of the screening characteristics themselves while health psychology approaches model screening behavior as a function of individual factors related to perceptions and beliefs. This dissertation set out to explain individual differences in screening preferences using both health economics and health psychology approaches. This dissertation utilized three different methods, including a systematic review and meta-analysis, a latent class analysis of discrete choice data, and a discrete choice experiment (DCE) that included psychological factors as characteristics of the screening process. We found evidence to support the hypothesis that differences in screening preferences exist and that individual factors like socio-demographic and psychological factors may play a role in explaining these differences. We also found evidence to support the validity of including psychological factors as attributes that vary in a DCE. This dissertation discusses the implications of the findings for theory, research, and the development of future screening programs.

But no central surveillance system provides the information needed to analyze how chronic disease impacts the U.S. population, to identify public health priorities, or to track the progress of preventive efforts. A Nationwide Framework for Surveillance of Cardiovascular and Chronic Lung Diseases outlines a conceptual framework for building a national chronic disease surveillance system focused primarily on cardiovascular and chronic lung diseases. This system should be capable of providing data on disparities in incidence and prevalence of the diseases by race, ethnicity, socioeconomic status, and geographic region, along with data on disease risk factors, clinical care delivery, and functional health outcomes. While this type of information on high-resource countries is readily available, Cancer Survival in Africa, Asia, the Caribbean and Central America presents in-depth cancer survival data from 27 population-based cancer registries in 14 low- and middle-resource countries. The striking inequalities in cancer survival between countries and within countries described in this volume are largely related to the differences in general awareness, availability of early detection practices, trained human resources, diagnosis and treatment and the development and accessibility to cancer services, as well as, to a lesser extent, to issues of data quality and reliability. The differences in cancer survival reported in populations observed between and within countries studied in this volume provide valuable insights for future planning and investment by governments in primary prevention activities, early detection initiatives and tertiary care to achieve meaningful cancer control. The calendar period of registration of incident cases for the present study ranges between 1990 and 2001. Data on 564 606 cases of 1-56 cancer sites from different registries are reported. Data from eleven registries were utilized for eliciting survival trend and seventeen registries for reporting survival by clinical extent of disease. Besides chapters on every registry and general chapters on methodology, database and overview, the availability of online comparative statistics on cancer survival data by participating registries or cancer site in the form of tables or graphs is an added feature.

The IOM's National Cancer Policy Forum, which succeeded the Board after it was disbanded in 2005, continued the Board's work to outline ways to increase screening in the U.S. On February 25 and 26, 2008, the Forum convened a workshop to discuss screening for colorectal cancer. Colorectal cancer screening remains low, despite strong evidence that screening prevents deaths. With the aim to make recommended colorectal cancer screening more widespread, the workshop discussed steps to be taken at the clinic, community, and health system levels. Workshop speakers, representing a broad spectrum of leaders in the field, identified major barriers to increased screening and described strategies to overcome these obstacles. This workshop summary highlights the information presented, as well as the subsequent discussion about actions needed to increase colorectal screening and, ultimately, to prevent more colorectal cancer deaths.

Persons who carry genetic mutations linked to hereditary colorectal cancer are the most likely to develop the disease.
