Book excerpt: Most newly licensed registered nurses go to work in acute care hospitals, which means they enter an increasingly complex healthcare environment where they experience staffing shortages, high nurse-patient ratios, and workplace violence. The purpose of this study is to attempt to understand the experiences of newly licensed registered nurses who have endured the early years of bedside hospital nursing and continue to work in their first nursing job. The existential phenomenological philosophy of Merleau-Ponty serves as the guiding framework for this qualitative research study. Following IRB approval, criterion and snowball sampling were used to recruit newly licensed registered nurses who graduated between May 2012 and May 2013. An open-ended unstructured interview format was used to collect data from nine nurses willing to be interviewed about their clinical experiences. Participants ranged in age from 24 to 43 years, represented day and night shifts, and included acute and critical care settings from five different health systems across the state of Tennessee. Data were analyzed by the researcher and the Interpretive Research Group at the University of Tennessee using the Thomas-Pollio existential phenomenological approach. All transcripts were read and analyzed for meaning units and global themes which were used to develop a thematic structure. The researcher, research group, and willing study participants agreed upon the final thematic structure. Five figural themes emerged from the data: 1) I found the perfect fit; 2) We're a pretty cohesive group; 3) It's about caring for the patients; 4) I've learned a lot; and 5) Knowing I make a difference. Study rigor was maintained by bracketing, data saturation, interdisciplinary review, member checking, and the use of direct quotes to support findings. This research has implications for nursing education and clinical practice. Findings are applicable to student nurses, newly licensed registered nurses, nurse educators, clinical preceptors, nurse managers, and hospital administrators.

Book excerpt: A qualitative study of newly licensed nurses in their first five years of practice, this study looked at twelve participants' receptions into the nursing workforce, and their intent to stay or leave their first nursing positions were examined. Factors influencing their perceived satisfaction in their first nursing jobs were found to be reception by other nurses and doctors, perceived support from management, and their perceptions of preceptors/mentors. From this study a grounded theory emerged. From that theory, A Model for Promoting Newly Licensed Nurse Success was developed that includes: zero-tolerance in the organization for incivility, a formal preceptor/mentoring program that incorporates Benner's skill acquisition model (2001), and formal leadership training for all levels that also incorporates Benner's model (2001).

Book excerpt: Abstract In response to the job shortage for newly licensed registered nurses (RNs), transition to practice and residency programs have been established in many areas of the country including the San Francisco Bay Area. The programs were designed as demonstration projects to assist new graduates by increasing their competence, confidence, and skills, and their employability. Evidence from new graduate programs suggests that the programs provide an important bridge that is necessary for the critical transition from classroom to clinical practice and from newly licensed RN to qualified nursing professional. The University of San Francisco (USF) Transition-To-Practice (nurse residency) program in ambulatory care described in this manuscript was developed and implemented as an innovative and alternative approach to traditional residency programs that has proven successful in increasing the employability of new RN graduates. The program provides a strategy that will enhance newly licensed nurses transition from the academic environment to the professional environment. Transition-to-practice (nurse residency programs) reflect an organizational commitment to support new nurses as they mature into competent professionals. The recent Institute of Medicine report, The Future of Nursing: Leading Change, Advancing Health (2010) predicts that in the near future nursing care will be provided less in traditional hospital settings and will instead occur in the community. Therefore it is important to expand transition programs outside of the acute care setting. Community based programs will prepare new nurses for successful practice in ambulatory care settings who will then be ready for the expected transformation of nuring care into the community. The benefits of transition-to-practice (nurse residency) programs include newly licensed RNs who will gain valuable experience in community settings and improved patient care that is an important investment in the future of healthcare.

This study will use a descriptive qualitative approach with a sample of first year nurses. Kolb's Experiential Learning Model serves as this study's conceptual framework. For the current study, the sample consisted of 10 newly graduated, female nurses with less than one year of experience working in the hospital setting were interviewed. Data analysis included interviews and transcription by the researcher. Finally, participants were asked about themes to increase rigor. Four themes emerged from this research: 1) how simulation is being used, 2) the perceived value of simulation, 3) simulation versus "real life," and 4) simulation and preparation for practice.

Two variables which have the potential to contribute to nurse attrition, but have received less inquiry attention are nurse mood and nurse comfort. Nursing job can be very stressful, especially when the work setting is in the hospital in-patient units. Little is known about how new nurses' mood and comfort impact their decision to stay or leave their hospital inpatient jobs. The objective of this study was to identify the relationships that exist between newly licensed nurses' (a) comfort and intention to stay and (b) mood and intention to stay at their hospital, in-patient-unit jobs. A non-experimental study that involved the use of a survey was completed using non-probability convenient sampling (N = 100) and two instruments called Nurse Comfort Questionnaire (Appendix A) and the Profile of Mood States 2 (Appendix A). Intention to stay was measured using: 1. I intend to stay here; and 2. Do you intend to stay at current job for longer than two years?" Statistics included descriptive, chi square, t test, correlation, and regression. Results showed that 63.5% (N = 61) stated "yes" to the question, "Do you intend to stay at the current job for longer than two years," while 36.5% (N = 35) stated "no." For the statement of "I intend to stay here," from a sample of (N =96), 64.6% (N = 62) selected "somewhat agree," "agree," or "strongly agree." The subjects who had no opinion were 7.3% (N = 7), and 28% (N = 27) selected somewhat disagree, disagree, or strongly disagree. Overall, the nurses' comfort level mean score was low at 70.7% (N = 96), while the mood level mean score was slightly better at 75% (N = 91). Both comfort and mood were significantly associated with intention to stay. Significant variables from the regression analysis included "lively", "deceived," "I am content," "Most of my work is true nursing work," "My body aches from my work," "I feel like I belong here," and "My schedule fits my lifestyle.
