Research has been conducted into the impact of the diagnosis but little research has looked at the effect of the initial surgical repair. This study aimed to discover more about the experience of surgery and its impact on mothers. The study also focused upon whether factors such as attachment, loss of control and expectations of appearance were relevant. Design: A qualitative methodology was chosen in order to allow participants to express their experiences in their own terms, rather than being bound by the researcher's ideas or perceptions. Method: Seven semi-structured interviews were conducted with mothers of babies with CLP who had had routine lip and/or palate surgery in the last three years. Mothers were recruited from the Cleft Lip and Palate Service based at the Morriston Hospital in Swansea. Results: Interviews were analysed using Interpretative Phenomenological Analysis (IPA). The super-ordinate themes which emerged were concerned with the Context of CLP, Emotions, Coping, the Impact of Others, Information and Expectations and Considerations of Surgery. Conclusions: Implications for clinical practice included provision of reference information, more opportunities to share experiences with other parents, as well as the need to provide time for parents to spend time together. Emphasis was also placed on professionals maintaining a friendly and approachable attitude, which was highly valued. The need to spend time with families to better understand their concerns for surgery, their needs for information and their strategies for coping were also emphasised.

Book excerpt: This study focused on the psychosocial aspects of cleft lip and/or palate on maternal emotional reactions and the family, with emphasis on the effect on the maternal-infant bond. Interviews were conducted with 40 mothers of 1-year-old infants with non-syndromic cleft lip and/or palate. The interviews were analyzed using the phenomenological method, which is explained. The focus was the unique interpretation of the mothers' experiences. The main result was that a defect that interfered with feeding disturbed the early maternal-infant bonding process more than a defect that only interfered with the child's appearance. Additional findings include: the birth of a child with a cleft is considered a shock by most mothers; the most severe problem has been the feeding problem; surgery, even if desired, is considered as a shock by most of the mothers; feelings of depression, fear, or bitterness are common among mothers of infants with a cleft; and the information that the mothers received at the hospital when the baby was born was inadequate. Descriptions are included for three cases involving different types of cleft: cleft palate, cleft lip, and cleft lip and palate. Implications for parent education are discussed, such as the importance of providing parental support to facilitate successful parenting, parents' need for information, and the importance of contact and a sense of community. Book excerpt: The purpose of this study was to assess parental satisfaction of children with nonsyndromic cleft lip and/or cleft palate concerning feeding and surgery education. This retrospective cross-sectional study used an original questionnaire. Four multidisciplinary teams in Ohio and Kentucky distributed the questionnaire to sixty-one parents of children between the ages of 3 months and 4 years with nonsyndromic clefts. Fifty percent of parents were satisfied with their experiences at the birth hospital. Parents who received feeding instructions within 12 hours after their childʼs birth felt more adequately prepared to feed their child than those who did not (p=0.0042). Over 90% of parents were satisfied with the care provided by their multidisciplinary team. Since half of the parents in this study were dissatisfied with their experiences at the birth hospital, birth hospitals need to ensure parents of children with orofacial clefts are educated about feeding techniques within 12 hours after birth.
