However, there is limited research exploring the risk factors associated with hookah use among this population. This study examined two risk factors, harm perceptions and beliefs about government evaluation of hookah, associated with current use of hookah among 18-24 year old college students, and looked at differences between current hookah users and non-users on cigarette smoking status, various demographic characteristics, and the two risk factors (harm perceptions and beliefs). Participants were 5,028 university students aged 18-24 (M age = 20.5 years; 59.6% female) from seven public universities within a larger university system. Students completed an online survey, which assessed their knowledge, beliefs, and behaviors related to hookah use. In this sample, 10.8% of participants reported current or past 30-day hookah use. There were differences between current hookah users and non-users on cigarette smoking status, gender, race/ethnicity, harm perceptions, as well as beliefs about government evaluation of hookah. Cigarette smokers, males, non-Hispanic White students, students reporting lower harm perceptions regarding hookah use, and those who believed the government evaluates hookah for safety were significantly more likely than their peers to be current hookah users. Results from a logistic regression analysis indicated that after controlling for cigarette smoking status, gender, and race/ethnicity, harm perceptions of hookah use, but not beliefs about government safety evaluation of hookah, was associated with an increased likelihood of current hookah use. Cigarette smoking was the strongest correlate of current hookah use; current cigarette smokers were seven times more likely than non-smokers to have used hookah in the past 30 days. Findings point to the necessity of educating college students, particularly cigarette smokers, about the dangers of hookah use. Additionally, tobacco prevention and cessation programs should be implemented in order to reduce initiation and continued use, and it is important to educate college student smokers about the dangers of dual use of this product with cigarettes.

Two large US universities; www.Facebook.com. Participants: 307 Facebook profiles were coded and 216 of these profile owners completed an online survey. On average, participants were 18.8 years old (SD=0.7), female (54%), Caucasian (70.4%), and approximately half were from each university. Outcome measures: Lifetime and frequency of hookah use, substance smoked in hookah, cigarette and marijuana use, hookah references displayed on Facebook. 27.8% of participants endorsed hookah use; there were no significant differences between age, gender, race, or university for hookah use. Hookah users reported smoking tobacco (78%), hashish (12%) and both tobacco and marijuana/hashish (10%) in their hookah. Compared to non-hookah smokers, hookah smokers were more likely to report using cigarettes (OR=3.41,95%CI=1.2-9.64) and marijuana (OR=15.01,95%CI= 6.5-34.65). Hookah references were present on 5% of Facebook profiles. More than one quarter of college students smoke hookah. Most smoke tobacco in their hookah, and hookah smoking is associated with polysubstance use. Some hookah users reference this behavior on Facebook. Hookah may present new risks for nicotine addiction in this population.

Book excerpt: This study examined factors that may protect African-American young adults in college from early onset of cigarette smoking, and factors that may place them at risk for cigarette smoking, using the framework of the Social Cognitive Theory. It was hypothesized that existing regulation of smoking, positive attitude toward academics and school connectedness would be related to low cigarette-smoking rates, while exposure to pro-tobacco promotions would be related to high cigarette smoking rates. A convenience sample of 576 undergraduates at Dillard University of New Orleans completed a Harvard College Alcohol Study survey. Forty six percent (n=269) completed the survey. Ninetyone percent were African American, and 13% of them were ever-smokers. Chi Square analysis was used to test for a statistically significant difference in response rates between ever-smokers and non-smokers. Results indicated no significant differences between respondents for any outcome measures. The Social Cognitive Theory constructs were not supported by the findings. The results of this study do not support any of the hypotheses. The Social Cognitive Theory constructs used to examine the hypotheses are not supported by the findings. Future studies should address the limitations of this research by using a random sampling technique, sampling across all academic classification levels, and assessing the validity and reliability of the Harvard College Alcohol Study survey measures for cigarette smoking.

Book excerpt: Use of tobacco products among college students continue to rise across the United States, leading many post-secondary institutions to adopt 100% tobacco-free policies. Disparities in tobacco use and policy support are perpetuated by racism and sexism and can be further exacerbated by one’s racial or gender identity. This cross-sectional study utilizes an intersectionality framework to analyze responses of students enrolled at the University of Washington in the Fall of 2020 or the Fall of 2021 to determine how gender and race intersect to influence tobacco use, policy awareness, and policy support. Using responses from an online survey, 16 logistic regression analyses were conducted. The study showed that there was a significant association between gender and policy support (p=0.001), as well as gender and past 30-day tobacco use (p=0.015); females were more likely to support tobacco policies and less likely to report recent tobacco use. The only significant gender x race interaction was for policy awareness regarding hookah use (p=0.03); females, who identified as a race other than white, were more likely to properly identify policy regulations related to hookahs. We recommend that UW develops targeted messaging strategies to address unique risk factors among student groups to gain support of policy changes, as well as improve the clarity, placement, and promotion of tobacco policies on campus to address low overall awareness of current policies. Future research efforts should include additional sociodemographic factors that have been shown to contribute to disparities in tobacco use and policy support, such as sexual orientation and socioeconomic status, to identify the need for strategies that provide appropriate support for these groups on campus.

Book excerpt: In 2014, electronic nicotine delivery systems (ENDS, also referred to as vaping or e-cigarette use) surpassed combustible cigarettes as the most commonly used tobacco product among youth. This is a critical public health concern as early onset of ENDS use has been linked to subsequent initiation of cigarettes and other tobacco products. Among adolescents and young adults who have used tobacco, the use of two or more products is also quite common. However, detailed information regarding specific patterns and trends in use within the context of increasingly popular ENDS is lacking. The current dissertation aimed to address this critical gap in three ways. First, chapter one reviews the distinguishing features of a variety of vaping devices, as well as the correlates and consequences associated with their use among adolescents and young adults. Next, chapters two and three empirically examine national patterns and age-trends in the use of ENDS and other nicotine delivery systems. Finally, sociodemographic differences in these patterns and trends, as well as the association between nicotine and marijuana use, are examined. The first empirical study examined nuanced age-trends in recent combustible cigarette, ENDS, and hookah use continuously from ages 14 to 24. Age-varying associations between marijuana use and each tobacco product were also examined. Data were from Wave 3 of the Population Assessment of Tobacco and Health (PATH) Study. The prevalence of past 30-day cigarette, ENDS, hookah, and marijuana use followed distinct age-trends. Cigarette and ENDS use were also significantly related to recent marijuana use across most ages. The association between ENDS and marijuana use was strongest at ages 14 and 15, whereas the association between cigarette and marijuana use was strongest at age 17. Age-varying prevalence estimates, as well as the age-varying association between ENDS and marijuana, differed by sex and race/ethnicity. The second empirical study examined patterns and transitions in lifetime nicotine use among young adults ages 18 to 24 using latent class and latent transition analysis (LCA, LTA). Data were from Wave 1 and Wave 2 of the PATH Study. LCA demonstrated that young adults were best represented by five distinct patterns of use: ENDS Only (2%), Hookah Only (10%), Cigarettes Only (19%), Combustibles Only (23%), and Poly, No Smokeless (45%). Differences in latent class membership emerged based on sex and race/ethnicity; the proportion of males and NH white young adults were highest in the ENDS only, Combustibles Only, and Poly, No Smokeless classes. Young adults who had ever used marijuana were also more prevalent within the Combustibles Only and Poly, No Smokeless classes. Young adults in the Combustibles Only class at Wave 1 had a 31% chance of starting to use ENDS by Wave 2. Young adults in the Poly, No Hookah class at Wave 1 had an 11% chance of starting to use hookah by Wave 2. Overall, this dissertation highlights the importance of research examining ENDS, polytobacco, and marijuana use among young people. Innovative statistical techniques provided added insight into who is at risk for using specific nicotine delivery systems at what age, as well as the factors contributing to polytobacco use and co-use with other drugs. Detailed information of this kind is necessary in order to prevent nicotine addiction and the associated health consequences, and to reduce overall burden of nicotine-related illnesses on the United States.

Book excerpt: Purpose: We intended to determine the prevalence, frequency, beliefs, and physician discussion rate of hookah among students. We concurrently aimed to survey physicians to determine their hookah knowledge and self-reported hookah discussion rate. Ultimately, we hoped to originate useful educational materials for both groups. Methods: We surveyed a random sample of undergraduate students at an urban university. We employed logistic regression modeling to ascertain variables associated with hookah users versus non-users and frequent hookah smokers versus rare smokers. Results: Out of 375 subjects, 13 (3.5%) were hookah naïve. 78 (20.8%) had never tried hookah while 284 (75.7%) smoked hookah. 64 students (22.6%) reported smoking hookah at least several times a month. 17 students (4.7%; 95% CI = 2.52%, 6.88%) reported a physician asking about hookah. In adjusted logistic regression models, individuals who believed hookah was healthier than cigarettes (OR = 2.193, 95% CI = 1.220, 3.942) and smoked marijuana (OR = 3.530, 95% CI = 1.837, 6.782) were significantly associated with being more likely to smoke hookah than non-users. Self-reported cigarette smoking (OR = 2.762, 95% CI = 1.430, 5.337) was the only variable associated with frequent hookah users. Out of 19 physicians surveyed, 1 (5.3%) reported asking patients about hookah, 6 (31.6%) never heard about hookah, and 18 (94.7%) expressed the desire to learn more. Conclusions: Smoking hookah is highly prevalent among college students partly because many do not know the health consequences. Simultaneously, many physicians do not know or ask about hookah. Useful educational resources will be valuable.

Companies are promoting their products through television and radio advertisements that use celebrities, sexual content, and claims of independence to glamorize these addictive products and make them appealing to young people.
