It is the second leading cause of cancer related deaths amongst women, and it is estimated that roughly 39,840 women will die of this disease this year. Breast cancer occurs across all populations and ethnicities. When African-Americans (AA) present with breast cancer, they usually have poorly differentiated tumors, and are more likely to be diagnosed with an advanced stage tumor. When compared to Caucasian (Cau) women, African-American women also have higher breast cancer mortality. The causes of these differences are not yet definitively known, but it has been suggested that the disparities that are present between African-American and Caucasian women are due to a number of factors. A few which have been mentioned are differences in Body Mass Index (BMI), socioeconomic status, health care coverage, and the level of obtained education that exists between the two ethnic groups. Although these factors may account for a small percentage of the difference seen between the two ethnic groups, the underlying cause that may explain why African-American women are at a greater risk of developing aggressive breast cancer may be due to differences in gene expression. A focus of my research project is the comparison of genome-wide gene expression differences between African-American and Caucasian women. Preliminary results from the comparison of normal breast tissue (obtained from reduction mammoplasty) from Caucasian and African-American women suggest there are marked differences in gene expression patterns. Pathway analysis of differentially expressed genes shows that they are involved in protein folding and the immune response. I am currently expanding this study to include a comparison of 10 AA to 10 Cau normal breast samples. These samples are being LCM dissected to separately compare gene expression in epithelial and stromal tissue. Cross comparisons between ethnic groups and tissue types will provide an understanding of normally occurring differences between AA and Cau women, which may help to explain the observed cancer disparities. Data from the study of normal tissue will be compared to gene expression data from triple negative breast cancer (TNBC) patients from both ethnicities.

Health care, screening behaviors (e.g., mammography) and late-stage disease detection account for significant differences in breast cancer morbidity and mortality among ethnically diverse women (ACS, 2007). The current study sought to identify variables that best predict women's breast cancer screening behaviors and to develop screening typologies for women in a large multiethnic sample (N = 15,130) from the California Health Interview Survey 2005 (CHIS, 2006). The study examined whether ethnicity (Hispanic, African American, Asian, and non-Hispanic white) moderated the relationship between emergent typologies and screening outcomes. Variables of interest addressed the following themes: health behaviors, women's health, cancer history, health insurance, health care utilization, mental health, as well as acculturation, racial discrimination, education, and socioeconomic status. Latent profile analysis (LPA : Lanza, Flaherty, & Collins, 2003) was employed to empirically derive and subsequently predict screening using each variable simultaneously to develop typologies of women. Post-hoc analyses using logistic regression were utilized to explain significant ethnicity by class interactions. Results revealed three substantive risk domains, Health, Stress, and Demographic, were significantly related to breast cancer screening. LPA revealed two classes, deemed Healthy and Health Risk, emerged significant for the Health domain. The aims of this study are: a) to survey a population-based, ethnically diverse sample to examine relationships between family history of breast cancer, knowledge of and attitudes toward genetic testing for inherited susceptibility, and sociodemographic factors, such as education and ethnicity; b) to conduct focus groups with Asian, Hispanic, African American, and White women to learn about possible differences in their attitudes concerning genetic testing. A new aim was planned and developed during the last year. The third aim is to follow-up the survey respondents from Aim I to determine the influence of the respondents' family history of breast cancer, ethnicity, and knowledge and attitudes towards breast cancer and genetic testing on their use of genetic counseling and genetic testing, and use of services such as mammography and pap tests in the 4-5 years elapsed since the original survey. This annual report describes our progress in developing and conducting the follow-up study planned as Aim 3.
