Book Decentralizing Health Services

Download or read book Decentralizing Health Services written by Krishna Regmi and published by Springer Science & Business Media. This book was released on 2013-11-19 with total page 222 pages. Available in PDF, EPUB and Kindle. Book excerpt: Decentralizing Health Services A Global Perspective Krishna Regmi, editor Current economic, demographic, and environmental shifts are presenting major challenges to health care systems around the world. In response, decentralization--the transfer of control from central to local authorities--is emerging as a successful means of meeting these challenges and reducing inequities of care. But as with health care itself, one size does not fit all, and care systems must be responsive to global reality as well as local demand. Decentralizing Health Services explores a variety of applications of decentralization to health care delivery in both the developing and developed worlds. Outfitted with principles, blueprints, and examples, this ambitious text clearly sets out the potential role of decentralized care as a major player in public health. Its models of service delivery illustrate care that is effective, inclusive, flexible, and in tune with the current era of preventive and evidence-based healthcare . Contributors point out opportunities, caveats, and controversies as they: Clarify the relationships among decentralization, politics, and policy Differentiate between political, fiscal, and administrative decentralization in health care systems Consider public/private partnerships in health systems Explain how the effects of decentralization can be evaluated. Present the newest data on the health outcomes of decentralization Explore some challenges and global issues of health systems in the 21st century And each chapter features learning goals, discussion questions, activities, and recommendations for further reading Heralding changes poised to revolutionize care, Decentralizing Health Services will broaden the horizons of researchers and administrators in health services, health economics, and health policy

Available in PDF, EPUB and Kindle. Book excerpt: In 2000 the Department of Health (DH) announced the establishment of NHS Local Improvement Finance Trusts (LIFT) to develop primary and social care services buildings and facilities in England. LIFT is based on long term joint ventures at national and local level: nationally, Partnerships for Health is a joint venture between DH and Partnerships UK (itself a joint venture between the Treasury, Scottish ministers and the private sector); locally the local joint venture company (the LIFTCo) is owned by representatives of the local health economy, Partnerships for Health and a private sector partner. Unlike PFI deals, LIFT deals are based on the local LIFTCo owning the premises which it builds and refurbishes. This report examines whether LIFT will meet local needs and provide value for money. 42 local schemes had been approved by August 2002, mostly in deprived inner city areas, with a total capital value of £711 million. Most LIFTCos are operational, though few buildings are open; most of the developments have been well received by local stakeholders. NAO conclude that LIFT will work. Nationally it is an attractive way of securing improvements, and local schemes appear to be effective and offer value for money. Local management frameworks need to be strengthened.

The total capital value of the first tranche of buildings was £711 million and the average cost of a building was £5 million. 90% of the capital is provided by debt and the properties are owned by the LIFTCo, with income being generated by rent payments from tenants such as GPs, Primary Care Trust, pharmacies and the local authority. Book High Quality Care for All

Download or read book High Quality Care for All written by Secretary of State for Health and published by The Stationery Office. This book was released on 2008 with total page 92 pages. Available in PDF, EPUB and Kindle. Book excerpt: This review incorporates the views and visions of 2,000 clinicians and other health and social care professionals from every NHS region in England, and has been developed in discussion with patients, carers and the general public. The changes proposed are locally-led, patient-centred and clinically driven. Chapter 2 identifies the challenges facing the NHS in the 21st century: ever higher expectations; demand driven by demographics as people live longer; health in an age of information and connectivity; the changing nature of disease; advances in treatment; a changing health workplace. Chapter 3 outlines the proposals to deliver high quality care for patients and the public, with an emphasis on helping people to stay healthy, empowering patients, providing the most effective treatments, and keeping patients as safe as possible in healthcare environments. The importance of quality in all aspects of the NHS is reinforced in chapter 4, and must be understood from the perspective of the patient's safety, experience in care received and the effectiveness of that care. Best practice will be widely promoted, with a central role for the National Institute for Health and Clinical Excellence (NICE) in expanding national standards. This will bring clarity to the high standards expected and quality performance will be measured and published. The review outlines the need to put frontline staff in control of this drive for quality (chapter 5), with greater freedom to use their expertise and skill and decision-making to find innovative ways to improve care for patients. Clinical and managerial leadership skills at the local level need further development, and all levels of staff will receive support through education and training (chapter 6). The review recommends the introduction of an NHS Constitution (chapter 7). The final chapter sets out the means of implementation.
