One of these factors is the availability of numerous tests and test versions that are use clinically, in the absence of a standardized test battery. Thus, the purpose of this study was to determine if three different versions of the DD test (Auditec, Audiology Illustrated, and VA) and three different versions of the FP test (Auditec, Audiology Illustrated, and VA), resulted in the same or different performance in older adults, in order to help lessen the ambiguity of APD assessment. The methods of this study included administering the DD and FP test versions, according to published guidelines, to older adults and comparing their scores to published normative data. The data was analyzed using a repeated measures analysis of variance (ANOVA) for each DD test version and each FP test version, in order to compare the mean test scores for each version. The results of this study indicated significant differences across the FP and DD test versions. The VA FP test version and Audiology Illustrated or Auditec DD test versions were recommended for clinical use.

Book excerpt: The American Speech Language Hearing Association has suggested guidelines for evaluating auditory processing disorders, but currently there is no standard testing tool that meets all of the guidelines. In 1997, Domitz and Schow proposed a test battery named the Multiple Auditory Processing Assessment (MAPA) and provided data from the Beta I version of it which attempted to incorporate most of these guidelines. Shiffman gathered more data from the Beta II version in 1999, but more refinement was necessary. The Beta III version is now more difficult and includes information for a form A and B. These forms were tested on school children ages 8-11 for equivalence and to see if the ceiling effect had been eliminated while still maintaining reasonable test results for children 8-11. All tests on both forms demonstrate at least some measures of equivalency. The ceiling effect has been eliminated for all tests, and test results appear reasonable in most respects as compared to results on Beta I and Beta II MAPA.

Book excerpt: When listening difficulties emerge despite a normal functioning peripheral auditory system and cognitive abilities, an auditory processing disorder (APD) can be diagnosed. This disorder can be seen in the older and younger populations, but it is more detrimental in children when language is being acquired. In addition, APD can present with other disorders affecting language, learning, and attention. In other developmental disorders, the prevalence of the disorder in males is higher than it is in females. The present study aimed to analyze gender differences on tests of auditory processing (AP). After a peripheral hearing assessment, children were tested in several areas to ensure they were typically developing. These areas included intelligence (Test of Nonverbal Intelligence - version 3, TONI-3), language (Clinical Evaluation of Language Fundamentals Screening Test - version 4, CELF-4), reading (Comprehensive Test of Phonological Processing, CTOPP), and attention (Integrated Visual and Auditory Continuous Performance Test, IVA-CPT). An AP test battery including dichotic digits test (DDT), frequency pattern test (FPT), duration pattern test (DPT), random gap detection test (RGDT), compressed and reverberated words (CRW), auditory figure ground +8 signal-to-noise ratio (SNR) (AFG+8), AFG 0 SNR, and listening in spatialized noise sentences test (LiSN-S) were utilized. No gender differences were observed in any of the AP tests. However, age differences were observed in almost every AP test. Further research is needed on a larger sample to help specify local normative data, by age and gender as warranted.

Book excerpt: The issues surrounding the comparability of various tests used to assess performance in schools received broad public attention during congressional debate over the Voluntary National Tests proposed by President Clinton in his 1997 State of the Union Address. Proponents of Voluntary National Tests argue that there is no widely understood, challenging benchmark of individual student performance in 4th-grade reading and 8th-grade mathematics, thus the need for a new test. Opponents argue that a statistical linkage among tests already used by states and districts might provide the sort of comparability called for by the president's proposal. Public Law 105-78 requested that the National Research Council study whether an equivalency scale could be developed that would allow test scores from existing commercial tests and state assessments to be compared with each other and with the National Assessment of Education Progress. In this book, the committee reviewed research literature on the statistical and technical aspects of creating valid links between tests and how the content, use, and purposes of education testing in the United States influences the quality and meaning of those links. The book summarizes relevant prior linkage studies and presents a picture of the diversity of state testing programs. It also looks at the unique characteristics of the National Assessment of Educational Progress. Uncommon Measures provides an answer to the question posed by Congress in Public Law 105-78, suggests criteria for evaluating the quality of linkages, and calls for further research to determine the level of precision needed to make inferences about linked tests. In arriving at its conclusions, the committee acknowledged that ultimately policymakers and educators must take responsibility for determining the degree of imprecision they are willing to tolerate in testing and linking. This book provides science-based information with which to make those decisions.

Book excerpt: The auditory processing abilities of 33 older adults (10 male, 23 female) ranging in age from 50 to 65 years were evaluated using the Frequency Pattern Test (FPT), Competing Sentences Test (CST), and Low-Pass Filtered Speech Test (LPFST). All participants were given each test: (1) in accordance with published guidelines relative to number of test items, use of repetitions of incorrectly answered test items, and scoring methods used to standardize the test; and (2) with variances from standardized methodology specific to number of test items, use of repetitions, and scoring methods. Results revealed significant effects of varying use of repetitions, as well as an interaction between number of test items and use of repetitions, on test outcome. Based on these findings, it is imperative that audiologists administer tests of auditory processing disorder (APD) in accordance with published guidelines in order to improve test reliability. If audiologists choose to vary test administration procedures from those used to standardize the test, they must obtain their own normative data values by which to compare their results. In doing so, over-diagnosis of APD will be reduced and the diagnostic value of tests of APD will be enhanced. It is this author's hope that by highlighting the importance of maintaining uniform test administration and scoring procedures, at least a portion of the current controversy surrounding the assessment and diagnosis of APD will be reduced.

Abstract: Among complications associated with non-syndromic cleft lip and/or palate (NSCL/P) in school age children, conductive hearing loss has been thoroughly investigated because the symptoms are noticeable and the treatment is often easy to access. Research on central auditory processing disorder [(C)APD] has been rarely explored in this clinical population. However, children with NSCL/P have been reported to have delayed speech and language development, as well as poor academic performance in general compared to craniofacially normal peers, despite their peripheral hearing problems typically resolving with age. In order to investigate suspected (C)APD in children with NSCL/P, the present research program was initiated. The study aimed to utilize a comprehensive test battery to assess auditory status in children with NSCL/P, and to investigate whether they have a greater frequency of (C)APD compared to craniofacially normal children. 147 children with NSCL/P and 60 normal children were recruited. They were Mandarin speakers and attending regular schools. The children with NSCL/P did not have peripheral hearing problems at time of assessment. There were three types of assessment tools used in the present research program. Firstly, hearing health tests were conducted to evaluate peripheral hearing function. Case history and auditory questionnaire reports were completed by caregivers to obtain basic medical and developmental information (Chapter 3). Secondly, auditory behavioral tests of (C)APD functioning were administered, including a gap detection test and a speech in noise recognition test (Chapter 4). Thirdly, an auditory evoked potential (AEP) assessment was conducted as an objective evaluation of the central auditory nervous system (Chapter 5). Based on the results of this research program, it was concluded that, firstly, children with NSCL/P showed behaviors typically found in children with (C)APD when questionnaire results were considered. Children with cleft palate (CP) showed the most negative outcomes, and cleft lip (CL) group children showed results equivalent to craniofacially normal children. Secondly, behavioral assessment results showed that compared to control group children, maturation for temporal resolution abilities was delayed in children with NSCL/P. Also, the ability to use interaural timing and intensity cues for speech recognition in a noisy environment was poorer in children with CP and CLP. Finally, abnormal AEP findings in children with cleft suggested longer neural transmission times and delayed development of the auditory nervous system may occur in this population. In summary, the research program found that children with NSCL/P are at higher risk of auditory processing difficulties compared to craniofacially normal children. In addition, a comprehensive test battery is more appropriate for making an accurate diagnosis of (C)APD in this population than a single assessment protocol. The present research program has contributed to an enhanced awareness of potential (C)APD in children with NSCL/P, which had not been investigated using a comprehensive test battery for a large sample of children with cleft disorders in any previous studies. Book excerpt: Objective: The purpose of this paper is to determine the relationship between the SCAN-C Test for Auditory Processing Disorders In Children-Revised (SCAN-C) and the Clinical Evaluation of Language Fundamentals, Third Edition (CELF-3) language assessment tool in a group of children suspected of auditory processing problems. Study Design: A retrospective chart review was conducted to compare the standard scores of the SCAN-C and the CELF-3 of 30 children referred for a multidisciplinary evaluation. Methods: All children were administered the SCAN-C and the CELF-3. Results: Significant differences were identified between the SCAN-C composite standard score and the total receptive language standard score as well as the total expressive language standard score of the CELF-3. A more in depth analysis revealed significant correlations between a number of the SCAN-C subtests and the CELF-3 subtests. Discussion: The findings imply that these two test approaches quantify different aspects of processing abilities in children.
