Although low back pain (LBP) and unilateral knee osteoarthritis (UKOA) present a substantial burden to society, the majority of the literature has focused on single joint problems. This study is the first to attempt to understand both the temporal and biomechanical relationships between UKOA and LBP. The study is the first to investigate biomechanical symmetry measures bilaterally during walking, sit-to-stand (SitTS), and stand-to-sit (StandTS) in asymptomatic subjects, subjects with UKOA only, subjects with LBP only, and subjects with UKOA and LBP in combination. The study found that 74% of patients with UKOA who were candidates for total knee arthroplasty reported a lifetime prevalence of LBP. Furthermore, these UKOA patients with LBP reported that their LBP first occurred, on average, 10 years prior to becoming a candidate for TKA. Asymptomatic individuals without LBP or UKOA generally exhibited high levels of symmetry during walking, SitTS, and StandTS, and statistically significant differences between the biomechanical variable measured from the non-dominant (ND) and dominant (D) limbs were not found for 26 of the 30 biomechanical variables during walking, SitTS, and StandTS. This study was the first of its kind to investigate the hypotheses that patients with UKOA and LBP would exhibit distinct patterns of biomechanical symmetry/asymmetry differing from patients with UKOA or LBP in isolation. Patients with UKOA and LBP displayed less hip rotation range of motion (ROM) in their surgically affected (A) limb during walking while this parameter was greater in the A limb for patients with only UKOA. Conversely, patients with UKOA and LBP exhibited larger values in the A limb for hip abduction ROM, hip flexion ROM, and knee abduction ROM during walking while patients with only UKOA had smaller values in the A limb for all of these variables. All of the biomechanical variables measured during SitTS and StandTS showed distinct variation between patients with UKOA and LBP and patients with only UKOA. Patients with UKOA and LBP had larger peak hip flexion moments in their A limb than their non-affected (NA) limb while patients with only UKOA displayed the opposite pattern. Patients with UKOA both with and without LBP were generally more reliant on their NA limb when performing the SitTS and StandTS tasks. Patients with UKOA and LBP had larger levels of asymmetry than patients with only LBP during walking vertical ground reaction forces (VGRF), hip abduction ROM, and stance time while patients with only LBP had larger levels of knee rotation ROM asymmetry during walking than patients with UKOA and LBP.
