The goal of this evaluation was to replicate the findings of Allen et al. (1997), which demonstrated a significant reduction in problem behaviors, including teen pregnancy, following the TOP® intervention relative to a no-treatment control group within a randomized control trial design. Previous evaluations conducted between 1990 and 2001 demonstrated the effectiveness of TOP® (Allen et al., 1990; Allen et al., 1997; Allen and Philliber, 2001). Further, the Brookings Institution recognized TOP® in its 2007 report to the U.S. Congress, Cost Effective Investments in Children, as one of the best programs in the country, stating that every $1 invested in TOP® produced a $1.29 return on investment (Isaacs, 2007).

The implementation had a large sample size that allowed for a robust evaluation strategy. This was a large-scale replication of an evidence-based program, TOP®. This program consists of a long-term approach (nine months), high dosage for participants (25 sessions with 20 hours of service learning), a comprehensive curriculum, youth-generated discussion and dialogue, and the development of ongoing relationships with trusted adults. This report describes the implementation and impact of this program including the cluster randomized controlled trial, the implementation process and findings, and the impact study and findings.

Youth at greatest risk for initiating early sexual debut and for engaging in risky sexual behaviors such as having unprotected sexual intercourse and intercourse with multiple partners are the primary targets for these interventions. Abstinence-only programs are one of several approaches that aim to reduce the incidence of birth to teens. These programs teach abstinence as the only 100% effective means of preventing HIV or pregnancy. An efficacy study of Promoting Health Among Teens! Abstinence Only Intervention (PHAT-AO), conducted in Philadelphia, found that compared to the youth in 3 the control groups, youth in the abstinence-only intervention group were more likely to delay sexual initiation and recent sexual intercourse. In 2010, PHAT-AO was listed by the Office of Adolescent Health as one of several evidence-based interventions for which funding was available to conduct replication studies. Program Reach, a non-profit agency located in the Bronx, New York, applied for and was awarded a five year grant to conduct an effectiveness study of PHAT-AO.

Book excerpt: This report presents final impact findings from a large-scale demonstration project and evaluation of the Teen Prevention Education Program (Teen PEP), an in-school, peer-to-peer sexual health promotion program that aims to reduce sexual risk behaviors and associated outcomes among high school students. Although teen pregnancy rates in the United States have decreased over the past 25 years (Martin et al. 2015), teenage pregnancy remains a serious concern. Teen parenting is linked to negative outcomes for both teen parents and their children. Teen mothers are less likely to graduate high school, have lower earnings, receive public assistance for longer periods, and are more likely to be single parents (Hoffman 2008; Perper et al. 2010). Children of teen mothers have worse educational, criminal, and health outcomes as well (Hoffman 2008). Furthermore, teen pregnancy is associated with high costs to the general public; in 2010, teen pregnancy and childbirth in the United States cost taxpayers more than $9.4 billion (National Campaign to Prevent Teen and Unplanned Pregnancy 2013). Teen PEP, developed and implemented by the Center for Supportive Schools (CSS) and HiTops, Inc., seeks to reduce rates of teen pregnancy and associated sexual risk behaviors through a combination of school-based, peer-led interactive workshops and school-wide initiatives.
