Part 5 discusses bariatric surgery, its history, procedure and effects in details, and other surgical techniques including electric stimulation of the vagus nerve, gastric balloons, intestinal liners, and liposuction.

The Lynchburg Healthy Living program was created to teach at-risk adolescents about exercise and nutrition, as well as to provide them with a role model and coach in how to live a healthy life. The study examined effects of the program on participants' body mass index (BMI), self-reported nutrition and physical activity habits, and perceived health knowledge.

Book excerpt: Multi-component childhood obesity interventions in community-based settings show promise as being effective strategies to prevent childhood obesity, especially in schools, since no other institution has as much contact time with children. The present study aims to evaluate how effectively a three-pronged school-based childhood obesity intervention, related to nutrition, physical activity and preventive health, may positively affect measures of weight status and health behaviors. Thirteen public, parochial and charter schools in Greater Philadelphia area (Bucks, Chester, Delaware, Montgomery and Philadelphia counties) were included in this school-based childhood obesity intervention. Recruited schools were categorized into Intervention schools (Core and Level 1 schools) and Control schools, in which the categorizations varied based on the dosage of the intervention received. Anthropometric measures and self-reported dietary intake and physical activity behaviors were taken on all 5th grade students at baseline, at the beginning of the 2014 to 2015 school year, and follow-up, at the end of the 2014 to 2015 school year. Non-parametic Kruskall-Wallis and Mann-Whitney analyses were conducted. In total, 269 5th grade students participating in this intervention were identified as overweight or obese at the beginning of the school year. Though body mass index (BMI) z-score slightly decreased in this population over the school year, no significant changes were found in difference of BMI z-score and waist circumference over the school year. Cross-sectional analyses indicated that at, follow-up, children in Level 1 schools consumed vegetables other than green salad significantly more frequently than Core and Control schools (P=0.002 and P=0.001, respectively), and consumed soda significantly less frequently than Core and Control schools (P

Book excerpt: During the past three decades the prevalence of child and adolescent obesity has tripled and currently 31.7% of children and adolescents are either overweight or obese. This is of concern due to the reported metabolic, psychological and social consequences associated with excess weight gain. While obesity occurs as the result of a sustained energy imbalance, there are many reported factors associated with its etiology. Interventions that can favorably impact such factors such as a healthy diet and physical activity could help prevent its onset early in life and spare children from reported metabolic and psychological consequences. Schools are one place intervention strategies are needed, however many obesity prevention interventions that have been implemented in this setting have produced mixed or modest outcomes. The after-school time frame is another excellent opportunity for such strategies, however less work has been done in this area compared with school-based interventions and more studies are needed. The purpose of this study was to evaluate the efficacy of a social cognitive theory based childhood obesity intervention with children in the after-school environment. This study employed a group randomized controlled design, whereby a convenience sample of twelve after-school programs were randomized into either an experimental (social cognitive theory based) or comparison (knowledge-based) intervention. A pretest, post-test and three month follow up test was conducted to evaluate the programs effects on BMI-percentile, key obesity prevention behaviors (fruit & vegetable consumption, sugar-sweetened beverage consumption, sugar-free drink & water consumption, the engagement of physical activities, and the engagement of sedentary activities), and three constructs of social cognitive theory (self-efficacy, expectations (comprising of outcome expectations and outcome expectancies), and self-control) related to each behavior. Both interventions consisted of four-30 minute sessions that were implemented over a four-week period. Process evaluations were used during each session to evaluate program fidelity and dose. A convenience sample of 71 children (37 in the experimental and 34 in the comparison) completed the interventions and were used for the final data analyses in this study. Results indicated that study variables and demographic variables were not different for children in the assigned groups at baseline. Process evaluations suggested that both programs were implemented as planned. It was found that BMI-percentile, all obesity related behaviors, and social cognitive theory constructs did not change between groups over the course of the intervention. There was however a significant main effect, indicating an improvement in both groups for fruit and vegetable consumption, the engagement in physical activity, the engagement in screen time, water and sugar free beverage consumption and self efficacy for fruit and vegetable consumption and physical activity. It can be concluded that the two interventions in this study may not have been very different from each other to discern changes. It is also likely that the experimental intervention may not have been of adequate length to truly produce the desired changes targeted in this study. More work is needed in this area to find appropriate, theory-based, health education programs that can complement larger health promotion efforts.

Motivational interviewing (MI) is an emerging intervention for adolescent health risk behavior that has promise. Despite its potential fit in primary health care contexts, primary care providers tend to express reticence in learning or adopting MI practices. This program evaluation project assessed the usefulness of Motivational Interviewing (MI) by primary care providers with adolescents in school based health centers that was implemented by a large randomized clinical trial (RCT) in southwest United States high schools. Methods: As part of the RCT, the primary care providers were required to do MI sessions with 66, 45, 25, and 91 youth participants respectively at their School-based site. It was anticipated that providers would be increasingly comfortable with MI between their first and final participant and that youth would be increasingly satisfied as provider comfort increased. To evaluate this, as part of the RCT, a set of primary care providers working with youth in school based health centers were trained through didactic sessions, homework, and role-play. School-based providers were provided an introductory training in MI, MI reading materials, a study manual, and were required to participate in bimonthly tele-coaching to ensure treatment integrity and fidelity. Primary care provider and adolescent participant survey data from session 1 of the RCT was analyzed to determine provider comfort with MI and adolescent satisfaction with MI. Summary of findings: Despite the standard training practices, providers showed a range of interest in comfort with the intervention. Intervention is crucial, and schools are often suggested as an important venue through which to intervene, although the results of such interventions are inconsistent and mixed. The present study sought to expand on the literature by evaluating the effect of a targeted and universal school-based obesity intervention, StayingFit, in three middle schools in a low-socioeconomic status, rural community. The intervention was tested in both a cluster-randomized (Study 1) and pre-post (Study 2) design. StayingFit was largely ineffective in creating changes in relative weight, behavior, or psychosocial outcomes. Possible reasons for the ineffectiveness of the intervention include the nature of the population and community, insufficient use of behavioral strategies and technology, limited program duration, low completion rates, and lack of parental engagement. Further research is needed to design multi-level and multi-sector interventions that can create meaningful change in high-risk communities.

Book excerpt: The Nutrition-Friendly Schools Initiative (NFSI) was developed in 2006 to provide a framework for ensuring integrated school-based programmes which address the double burden of nutrition-related ill health and to become the nutrition module of the Health Promoting Schools. The NFSI has since been used around the world, including as a self-appraisal tool for schools in 18 countries, in national NFSI programmes and as part of academic research and evaluation projects. The NFSI Framework outlines 26 essential criteria within five broad components: 1. school nutrition policies, 2. awareness and capacity building of the school community, 3. nutrition and health promoting curricula, 4. supportive school environment for good nutrition, and 5. supportive school nutrition and health services. This review summarizes the synthesized evidence from 117 reviews identified as relating to the five components and the 26 essential criteria of the NFSI. The findings may be used — in conjunction with existing UN and WHO guidance and tools — to inform the work of governments, policy-makers and researchers concerned with school-based health and nutrition promotion programmes and initiatives.
