Book excerpt: Les urgences pédiatriques sont un lieu d'étude privilégié de la prise en charge de la douleur aiguë chez les enfants. L'introduction d'un nouvel outil, l'EVENDOL, pour évaluer la douleur chez les enfants de moins de 6 ans, a permis une étude prospective de la prise en charge de la douleur aiguë chez l'enfant. Cette étude prospective, réalisée du 15 mai au 15 septembre 2006 a porté sur 1399 enfants parmi les 6730 enfants de passage aux urgences médico-chirurgicales pédiatriques de Clermont-Ferrand sur cette période. Les enfants de plus de 6 ans ont été évalués avec EVA et les enfants de moins de 6 ans avec l'EVENDOL. Pour chaque enfant, le recueil de données comporte le motif de la consultation, la personne qui amène l'enfant, les gestes douloureux réalisés et leur prévention, ainsi que les traitements administrés avant, pendant et après leur sortie des urgences. Résultats : 1399 enfants ont consulté pour un motif supposé douloureux et 31,8% des enfants avaient moins de 6 ans ; 50,18% consultaient pour un motif chirurgical et 49,61% pour un motif médical ; 69,48% des enfants ont été évalués par EVA et 31,02% des enfants par EVENDOL ; 65,22% des enfants ayant subi un geste douloureux aux urgences ont reçu une prémédication antalgique ; 35,95% des enfants ont reçu un traitement antalgique lors de leur passage aux urgences ; un traitement a été prescrit à 64,26% des enfants à leur sortie des urgences. Une feuille d'évaluation de la douleur fait désormais partie intégrante du dossier d'admission des enfants aux urgences pédiatriques. Cette étude constitue une action ponctuelle d'évaluation, mode d'entrée dans l'évaluation des pratiques professionnelles. Même si elle reste encore à améliorer, la prise en charge de la douleur chez l'enfant progresse, principalement grâce à la formation et à la sensibilisation des équipes soignantes.
