Répondre à la douleur de l'enfant repose sur une connaissance précise de cette expérience sensorielle et émotionnelle désagréable. En effet, ceci permet, par l'étude de sa physiopathologie spécifique en pédiatrie, de différencier les douleurs aiguës des douleurs chroniques, ainsi que les douleurs par excès de nociception des douleurs neuropathiques. Secondairement, sa reconnaissance et son évaluation sont aujourd'hui possibles par la mise à disposition d'échelles d'auto ou d'hétéro-évaluation adaptées à l'âge et à la maturité de l'enfant. Finalement, son traitement dispose d'un éventail large de molécules et de techniques adaptées, sûres et efficaces. Le pharmacien, de part sa place d'acteur de santé publique, est un maillon important dans la bonne utilisation des thérapeutiques utilisées pour le traitement de la douleur pédiatrique, ainsi que dans la bonne utilisation de l'automédication antalgique. Il joue un rôle essentiel dans l'éducation des parents et des enfants au niveau de la connaissance et de la bonne utilisation de ces médicaments. Il permet ainsi, par son conseil, une automédication maîtrisée.

Sa prise en charge et son évaluation en ambulatoire ont été peu étudiées. Entre mai 2007 et août 2007, douze médecins généralistes de Midi-Pyrénées ont été interrogés sur leur pratique concernant l'évaluation et la prise en charge de la douleur aiguë de l'enfant en consultation. Une enquête chez cent dix huit de leurs patients, couplée à une enquête chez les praticiens et effectuée en double aveugle, a permis de mettre en évidence une mauvaise connaissance et une mauvaise utilisation des recommandations de la Haute Autorité de Santé, et un manque de prescription des antalgiques de niveau 2 ou 3 de l'Organisation Mondiale de la Santé. Une amélioration est souhaitable pour optimiser l'évaluation et la prise en charge de la douleur aiguë de l'enfant. Nous pensons pouvoir y contribuer en élaborant un outil basé sur nos conclusions et sur les recommandations. Celui-ci comporte un rappel d'échelles d'évaluation de référence et un rappel des différentes pathologies médicales (otites, angines, gingivostomatites) et des douleurs liées aux soins. Une deuxième enquête permettant d'évaluer la pertinence de cet outil reste à réaliser.

Book excerpt: In recent years, critical interest in francophone literature has become increasingly pronounced. In the case of the French Caribbean, the work of several writers (Aime Cesaire, Frantz Fanon, Edouard Glissant and Patrick Chamoiseau, for example) has gained international recognition, and has formed a vital part of more general debates on history, culture, language and identity in the post colonial world. The majority of such writers, however, have been male and, perhaps recalling the preference that France has always shown for the island, have come in large part from Martinique. Mapping a Tradition: Francophone Women's Writing from Guadeloupe aims to explore a different side of francophone Caribbean writing through the examination of selected novels by Jacqueline Manicom, Michele Lacrosil, Maryse Conde, Simone Schwarz-Bart and Dany Bebel-Gisler. Placing the work of these writers in the context of that of their better-known, male counterparts, this study argues that it has provided an important mode of intervention in, and disruption of, a literary tradition which has failed to address questions of sexual difference and has often excluded issues relating to French Caribbean women. At the same time, this study suggests that Guadeloupean women's writing of the last thirty years may he seen to constitute a 'tradition' in itself, replete with its own influences and inheritances. At once within, and outside the 'dominant' tradition, women's writing from Guadeloupe - and Martinique - has come to occupy a position at the forefront of contemporary efforts to expand and redefine a still-burgeoning corpus of literary and theoretical work.
