Book excerpt: In the early stages of planning the Third International Conference in System Science in Health Care, the steering committee members, most of whom had participated in the first conference in Paris (1976) and the second in Montreal (1980), made some basic decisions about organization of subject matter. The earlier meetings had been very successful in bringing together specialists from the health professions and the traditional sciences. In addition to physicians and nurses, these were representatives of the disciplines of the behavioral sciences, system theory, economics, engineering, and the emergency fields of management science and informatics -all concerned with the development of health resources in a broad system context. The reported research and experience of the many disciplines represented had dealt with one or more of three concerns: 1) a major health problem, such as cardiovascular disease, or an important popUlation at risk, such as the elderly or children or workers; 2) some generic aspect of organization and decision making, including trial and evaluation ofinnovative health strategies; and 3) the methodology of research and analysis in system of health service. The challenge to the conference organizers lay in the eliciting and arranging of experiences in such a way that the health services could be seen as purposeful,living, evolving systems.

Book excerpt: L'application des recommandations en évaluation médico-économique dans le cadre de la recherche hospitalière courante doit s'adapter à l'environnement et aux spécificités contextuelles des structures de soins. Plusieurs travaux ont été conduits afin d'évaluer la qualité des projets de recherche, d'identifier les obstacles rencontrés et de proposer des recommandations pour leur conduite. Dans cet objectif, notre travail a été composé de 4 parties : L'évaluation de la qualité des évaluations médico-économiques. Nous avons mené une analyse des études publiées dans le domaine de la chirurgie cardiaque, discipline particulièrement coûteuse et pourvoyeuse d'un grand nombre d'innovations technologiques. Les résultats de cette étude ont montré que la qualité des évaluations médico-économiques a considérablement augmenté sur la période, laquelle peut être encore améliorée, notamment sur les hypothèses retenues, le choix du modèle, et les sources d'hétérogénéité. L'identification des spécificités contextuelles diminuant la qualité des évaluations médico-économiques. Plusieurs éléments gênant les évaluations ont été identifiés : ces recherches ont dû s'adapter et des solutions méthodologiques ont été apportées, notamment une adaptation du design méthodologique du projet au contexte dans lequel il est mené. L'implémentation d'une nouvelle méthode d'évaluation des technologies de santé en milieu hospitalo-universitaire. La mini évaluation des technologies de santé (mini-HTA) permet d'évaluer l'implémentation d'une innovation dans un établissement de santé, en prenant en compte non seulement son efficacité, sa sécurité et son coût, mais également son impact organisationnel et budgétaire. L'élaboration de recommandations méthodologiques adaptées à l'évaluation médico-économique en milieu hospitalier. Ces travaux ont permis, en se basant sur les constats faits lors de nos précédents travaux, une évolution de la grille de recommandations en la développant et en l'adaptant à la recherche en milieu hospitalier, notamment en élargissant les possibilités de design d'études, en développant les points de vue possibles en EME, et en précisant les sources de données disponibles et adaptées pour la valorisation des coûts. D'après les résultats de nos travaux, la qualité des évaluations médico-économiques menées en milieu hospitalier, pourtant indispensables à l'aide à la décision en France, a progressé mais n'est pas encore suffisante pour l'aide à la décision. Les freins identifiés sont nombreux, aussi bien liés à la méconnaissance de la discipline qu'aux outils disponibles dans les établissements de santé pour conduire des évaluations de qualité internationale. Une nouvelle méthodologie, la mini-HTA, réalisée en amont des études cliniques, est proposée, en apportant une vision plus large de l'impact de l'introduction d'une innovation dans les établissements qui envisagent son adoption, notamment en intégrant l'analyse du contexte de l'implantation. Une version plus exigeante de la grille de recommandations, centrée sur les évaluations médico-économiques en milieu hospitalier, est proposée pour améliorer la qualité des projets d'évaluation menés sur cette thématique.
