Book excerpt: Face au manque de travaux sur la mémoire prospective dans la dyslexie, nous avons proposé un protocole d’évaluation informatisée de la mémoire prospective à des adolescents dyslexiques et non dyslexiques âgés de 10 à 15 ans. Le protocole était également constitué d’un questionnaire d’auto-évaluation de la mémoire prospective et d’épreuves complémentaires de mémoire de travail, mémoire épisodique et fonctions exécutives car ces fonctions interviennent dans le fonctionnement de la mémoire prospective. Les analyses statistiques ont mis en évidence des difficultés dans les tâches prospectives pour les dyslexiques. L’analyse du questionnaire montre que les oublis prospectifs relèvent plutôt de la mémoire à court terme avec un indice environnemental pour les adolescents dyslexiques. On observe également que quelques scores de mémoire prospective sont corrélés à des scores pour les épreuves complémentaires. Cette étude pourra ouvrir de nouvelles perspectives d’accompagnement du patient en orthophonie en proposant des consignes adaptées aux difficultés de mémoire prospective observées. De plus, la prise de conscience du fonctionnement de la mémoire prospective pourra permettre aux adolescents dyslexiques et leur entourage de mieux comprendre leur fonctionnement pour améliorer la vie quotidienne et augmenter leur autonomie.

Book excerpt: De nombreux auteurs reconnaissent actuellement la dyslexie comme un trouble impliquant diverses perturbations cognitives. Des dysfonctionnements de la mémoire de travail, des fonctions exécutives et de la mémoire épisodique rétrospective ont notamment été évoqués. La mémoire prospective, dont le fonctionnement fait intervenir ces processus, pourrait alors être altérée, suscitant des répercussions au quotidien. Une seule étude s’est intéressée à son efficience dans la dyslexie. Notre objectif est d’évaluer le fonctionnement de cette mémoire auprès de 25 adolescents dyslexiques et témoins appariés à travers un paradigme informatisé, auquel nous avons ajouté un questionnaire afin d’évaluer la plainte quotidienne. Nous avons également évalué les processus sous-tendant son fonctionnement. Les performances des adolescents dyslexiques confirment l’hypothèse d’une défaillance de la mémoire prospective. Les difficultés sont majorées chez les adolescents dyslexiques lorsque plusieurs intentions d’action sont présentes en mémoire, avec des capacités de gestion du temps affaiblies. Toutefois, les difficultés observées occasionnent peu de plainte au questionnaire. Les résultats aux épreuves complémentaires révèlent une atteinte globale de la MDT et un déficit de la mémoire épisodique rétrospective. Les difficultés exécutives sont plus nuancées. Les analyses mettent en avant l’implication de certaines de ces composantes dans la réalisation différée d’intentions d’action. Cette étude permet une compréhension plus globale des difficultés rencontrées par les patients dyslexiques nécessaire à la prise en charge et permet d’envisager une intervention sur la mémoire prospective en orthophonie.

Bien qu'elles apportent des informations essentielles sur la mémoire sémantique, elles présentent l'inconvénient d'être sensibles à d'autres fonctions cognitives que la mémoire sémantique et plus particulièrement les fonctions exécutives. Plusieurs études ont mis en évidence que les tâches de libre association pourraient être des mesures plus pures de la mémoire sémantique, elles pourraient donc être une alternative intéressante. Nous évaluons donc, dans un premier temps, dans quelle mesure la tâche de libre association est une mesure adéquate de la mémoire sémantique. Dans un second temps, la tâche de libre association est éprouvée auprès d'une population d'enfant ayant une dyslexie. Les résultats nous permettent de voir que la tâche de libre association pourrait être une bonne alternative ou complément à la tâche de fluence verbale. Néanmoins, les enfants dyslexiques ne se différencient pas des enfants au développement typique sur cette tâche. Les résultats sont discutés quant à notre protocole de recherche et des pistes pour de futures recherches sont proposées.

Plusieurs études ont mis en évidence que les personnes dyslexiques présentaient des troubles de la mémoire de travail (MT). Le premier objectif de la thèse est de préciser la nature des déficits de MT chez les enfants dyslexiques. Le second objectif est d'étudier la potentielle efficacité d'une remédiation ciblée de la MT de l'ordre sériel et de l'item chez les enfants dyslexiques. Nous avons évalué les capacités de MT de l'ordre sériel et de l'item, les compétences phonologiques et le contrôle attentionnel chez des enfants dyslexiques et au développement typique. Une remédiation ciblant soit la MT de l'ordre sériel soit la MT de l'item a ensuite été proposée à quatre adolescents dyslexiques. Nos résultats ont montré une hétérogénéité des profils de MT dans notre groupe d'enfants dyslexiques, avec une dissociation entre les troubles de la MT de l'item et de l'ordre sériel. De plus, ces déficits de MT n'étaient pas simplement la conséquence de difficultés attentionnelles ou phonologiques des enfants dyslexiques. En revanche, nos résultats suggèrent que leurs troubles phonologiques pourraient être consécutifsà leur déficit de MT de l'item. Par ailleurs, si certaines de nos données suggèrent deseffets potentiels d'une remédiation ciblée de la MT de l'item et de l'ordre sériel, ellesmontrent également l'importance de mener d'autres études avant de pouvoir généraliser nos résultats.

Although typical aging does not cause low vision, four major causes of vision impairment are directly related to the aging process: (1) age-related macular degeneration (AMD), (2) diabetic retinopathy, (3) glaucoma, and (4) cataracts. Instances of these impairments are expected to double by the year 2050, making low vision a significant public health issue. This Practice Guideline includes occupational therapy assessment and intervention guidelines for older adults who have visual acuity impairments, visual field impairments, or both as a result of a low vision diagnosis. Screening, referral, and evaluation are covered for clients for central visual field impairments, such as AMD and cataract; peripheral field impairments, such as glaucoma, hemianopsia, and quadrantanopsia; and mixed visual field loss, such as diabetic retinopathy. This work can help occupational therapists and occupational therapy assistants, as well as individuals who manage, reimburse, or set policy regarding occupational therapy services, understand the contribution of occupational therapy to treating older adults with low vision. Appendixes include selected diagnostic and billing codes and evidence tables.

It has long been clear that these processes change considerably in early childhood, transforming a relatively stimulus-driven toddler into a child capable of flexible, goal-directed problem solving. However, the nature of these processes has remained elusive. In a programmatic series of 9 experiments, the authors examine circumstances that help or hinder executive function in 3- to 4-year-old children. The results provide the basis for a revision of their Cognitive Complexity and Control (CCC-r) theory, according to which there are age-related increases in the complexity of the rules that children can formulate and use when solving problems. The revised theory (a) specifies more clearly the circumstances in which children will have difficulty using rules at various levels of complexity, (b) provides a more detailed account of how to determine the complexity of rules required in a task, (c) takes account of both the activation and inhibition of rules as a function of experience, and (d) highlights the importance of considering intentionality in the study of executive function.
