Lutter contre la douleur est un enjeu éthique. L'évaluation de la douleur est difficile dans le contexte de l'urgence, d'autant plus difficile que le patient est âgé, et/ou présente des troubles cognitifs. Cette étude prospective a eu pour objectif de comparer l'évaluation de la douleur par 2 échelles différentes, l'échelle numérique (EN) et une échelle d'hétéro-évaluation comportementale (ALGOPLUS®) potentiellement plus adaptée aux troubles cognitifs. Elle a été réalisée au SAU du Centre hospitalier de Lyon Sud comportant entre le 1er janvier 2014 et 31 décembre 2014 et a inclus tous les patients de plus de 70 ans consultant au SAU pour une fracture de l'extrémité supérieure du fémur ou un accident vasculaire cérébral dont l'autoévaluation de la douleur est jugée impossible. Les patients de l'étude avaient des caractéristiques similaire Le temps moyen d'évaluation de la douleur à partir de l'heure d'arrivée était de 11 ± 9 min. et le délai moyen entre l'évaluation et l'administration d'antalgiques était de 158. ± 132 min. dans toute l'étude, sans différence significative entre les deux phases. L'antalgique le plus utilisé était le paracétamol que ce soit dans la phase pré-évaluative (EN) ou la phase évaluative (ALGOPLUS®). Nous avons comparé 2 pathologies l'AVC et la FESF. Les patients avec une FESF sont plus douloureux dans la phase Algoplus : 64% avaient un ALGOPLUS® [4-5] contre 43% pour une EN [6-10] (p=0.001). Pour l'AVC, 25% des patients avaient un Algoplus [4-5] et 20% [2-3] contre 0% une EN [6-10] et 91% EN [0-3] (P
