Book excerpt: INTRODUCTION : La démarche palliative a pour but «d'asseoir et développer les Soins Palliatifs dans tous les services et établissements en facilitant la prise en charge des patients en fin de vie et de leurs proches, par la mise en place d'une dynamique participative prenant en compte les difficultés des soignants ». L'objectif de ce travail est d'évaluer la démarche palliative dans un service de médecine interne. MATÉRIEL ET MÉTHODE: Ce travail est divisé en deux parties complémentaires. La première est une étude de dossiers, via le Dossier Patient Personnalisé, étudiant l'application des grands principes des soins palliatifs pour les patients décédés en soins palliatifs (identifiés par le codage principal ou secondaire soins palliatifs) entre le 31/12/2012 et le 31/12/2014 dans le service de médecine interne du CHRU Bretonneau de Tours. La deuxième partie est une enquête menée à l'aide d'un questionnaire en août 2015 auprês des personnels du même service, s'intéressant à leur formation, à leur ressenti et aux points de la prise en charge palliative qu'ils estiment devoir être améliorés. RÉSULTATS : Dans la première partie, 45 dossiers de patient décédés ont été retenus. Une partie des critères sélectionnés étaient présents, certains bien tracés (évaluation de la douleur jusqu'au décès dans 88,9 % cas), d'autres peu retrouvés (absence d'identification de la personne de confiance dans 88,9 % des observations). Dans la seconde partie, la participation du personnel a été forte (54 réponses sur 66 questionnaires distribués (81,8 %)). On constate que 27,8 % des personnels sont formés en soins palliatifs, 98,1 % des répondants ont déjà rencontré des difficultés dans la prise en charge palliative des patients, 83,3 % ont ressenti de la souffrance ou un mal-être dans les suites. Les points à améliorer en priorité sont la prise en charge symptomatique du patient à la phase agonique et la formation des personnels. CONCLUSION : La démarche palliative est présente dans le service de médecine interne du CHU. mais peut encore être développée. Le manque de formation et de communication est responsable de difficultés, voire de souffrances pour les personnels. Dans l'avenir on pourrait imaginer la mise en place d'un projet de service en soins palliatifs permettant d'apporter des améliorations simples mais efficientes (formation, communication. traçabilité).
