L'évaluation des compétences de communication permet de rendre compte des stratégies de compensation de l'aphasique mais aucun outil n'est adapté à la phase aiguë. L'intérêt d'une prise en charge précoce semble communément admis mais aucun traitement spécifique n'est privilégié actuellement. De nombreux auteurs soutiennent l'intérêt d'une approche gestuelle de la rééducation de l'aphasie globale. Le protocole Visual Action Therapy (VAT) développé par Helm a notamment montré son efficacité en phase chronique. Notre objectif a été de développer un outil d'évaluation de la communication adapté à la phase aiguë. Vingt-trois sujets contrôle et 22 sujets aphasiques ont été recrutés pour la normalisation de cette échelle. Celle-ci a ensuite été proposée à deux patients pendant la phase aiguë de leur AVe. Chacun a bénéficié d'une thérapïe linguistique classique et d'une thérapie gestuelle adaptée de la VAT dans un ordre aléatoire. L'échelle d'évaluation de la communication a montré une bonne fiabilité interexaminateurs. Elle a permis de différencier une population témoin et une population aphasique concernant différents aspects de la communication. Les sujets témoins se sont montrés plus compétents sur le plan verbal, et les sujets aphasiques ont davantage exploité la communication non verbale, ce qui a finalement induit des performances totales très comparables entre les deux populations. Cette échelle a également permis de différencier les aphasiques en phase aiguë et ceux en phase chronique, les stratégies de compensation tendant à s'améliorer avec la distance à l'AVe. Pour autant, ni le profil aphasique, ni les progrès linguistiques n'ont pu expliquer une telle amélioration. Les sujets aphasiques ont répondu favorablement au protocole VAT puisqu'ils ont montré une progression régulière dans les étapes. Aucun n'a pu franchir la première étape de difficulté de la thérapie linguistique. Une progression des capacités de communication a été observée à l'aide de notre échelle au cours de la rééducation malgré l'absence de récupération du langage.
