Cependant, il existe également un secteur public en majeure partie représenté par les Centres Hospitalo-Universitaires (CHU). L'objectif de ce travail était de mieux connaître l'orientation des patients à l'issue d'une première consultation dans le Service d'odontologie du CHU de Lille en 2018, de mieux cerner leur profil et d'évaluer leurs besoins en soins bucco-dentaires, en vue à moyen terme, d'améliorer l'organisation du service. Cette étude a permis de connaître le profil et les besoins en soins de 81 sujets dont la majorité était de sexe féminin, la tranche d'âge la plus représentée étant celle des 36-60 ans, appartenant à une catégorie socioprofessionnelle relativement modeste. On a constaté que de nombreux patients nécessitent une prise en charge globale complexe faisant intervenir plusieurs unités fonctionnelles, notamment l'UF de dentisterie restauratrice et endodontie et l'UF de prothèse. Grâce à une meilleure connaissance des utilisateurs du service, ce travail pourrait donc participer à l'amélioration de la qualité de la prise en charge des patients au sein de ce service. Ainsi, ce travail s'inscrit dans une démarche d'évaluation des pratiques professionnelles.

L'éducation à la santé a pour objectif d'aider chacun à choisir librement et consciemment des comportements qui sont plus favorables à sa santé. Les comportements dépendent de plusieurs facteurs et notamment des croyances et représentations sociales. Les « théories profanes » des patients sont bien souvent en décalage avec les « conceptions savantes » des professionnels. Certaines théories s'attachent à dire qu'il faut corriger ces conceptions erronées, d'autres qu'il faut les prendre en compte dans la démarche éducative. Il existe peu de données bibliographiques relatives aux connaissances et croyances des patients en santé bucco-dentaire en France. Ce travail a consisté à conduire une étude épidémiologique transversale par questionnaire rempli en entretien en face à face ayant pour objectif d'évaluer les connaissances et croyances en santé bucco-dentaire des patients consultant l'Unité Fonctionnelle de Première Consultation du Service d'Odontologie-Caumartin du CHRU de Lille. Cette étude constitue une étape préalable à la création d'affiches pédagogiques, objet du tome 2 de cette thèse.

En Odontologie, les indicateurs de qualité (IDQ) sont encore très peu répandus et rarement validés. A l'interface entre santé bucco-dentaire et santé générale, la prise en charge (PEC) parodontale, fondée sur la preuve, est un modèle de choix pour mettre en place une démarche qualité. Les objectifs sont de proposer un ensemble d'IDQ de base pour évaluer et à terme améliorer la PEC parodontale ; d'évaluer la pertinence de ces IDQ à l'aide d'une étude pilote menée au CHU de Lille. A partir d'une revue de littérature approfondie, des IDQ correspondant aux 6 dimensions de la qualité (sécurité, efficacité, efficience, accessibilité, approche centrée sur le patient, équité) ont été proposés et évalués. Les valeurs de ces IDQ ont ensuite été calculées à partir de l'audit des dossiers de 100 patients consécutifs de l'UF parodontologie du CHU de Lille. Une enquête par auto-questionnaire a également été menée sur le même échantillon pour développer des IDQ liés à la satisfaction du patient. Douze IDQ quantitatifs ont été retenus parmi lesquels 6 principaux. Les résultats ont été comparés à la littérature pour identifier des axes d'amélioration de la qualité de PEC. Le questionnaire montre quant à lui une bonne validité pour identifier et étudier les dimensions associées à la satisfaction des patients en parodontologie. Ce travail montre à petite échelle la faisabilité et la pertinence d'IDQ en parodontologie. Un processus structuré de consensus au niveau national permettrait de définir et tester des IDQ communs pour mieux intégrer la démarche qualité dans la PEC parodontale.

Book excerpt: En France, la population handicapée s'élève à 9,6 millions de personnes répartis en 6 catégories de handicaps : mental, sensoriel, moteur, psychique, trouble du spectre autistique et troubles envahissants du développement et troubles dys. Les soins au cabinet dentaire sont complexes et l'anesthésie générale (AG) persiste comme dernière alternative thérapeutique. L'objectif de ce travail est de décrire l'écart entre le nombre et la nature des soins prévus et réalisés lors de la première consultation soit au fauteuil, soit sous AG. Une étude rétrospective des patients atteints de handicap au CHU Saint André de Bordeaux entre 2019 et 2021 a été réalisée. Les patients pour l'AG venaient principalement suite à des douleurs tandis qu'en ambulatoire, ils venaient pour effectuer un suivi. Plus d'un quart des patients avaient attendu plus d'un an leur opération sous AG. Le nombre d'actes ainsi effectués représentait presque le double de ceux prévus initialement et était majoritairement des soins carieux et des extractions. En ambulatoire, les actes étaient surtout de prophylaxie. Après l'étude, près des deux tiers des patients ont été perdus de vu. Au total, 64 patients ont dû être réorientés afin d'assurer une meilleure prise en charge. Il est ainsi important de mieux sensibiliser la population à l'importance d'un suivi odontologique mais également de faciliter les accès aux blocs opératoires à l'odontologie et de mieux former les étudiants aux différentes approches comportementales.

Dans le cadre de notre étude, cette grille d'évaluation commune a permis de conduire une réflexion sur les différentes causes aux écarts constatés qui peuvent être de nature institutionnelle, et/ou organisationnelle.
