Il nous appartient donc de concilier sans jamais les opposer, les impératifs occluso-fonctionnels et esthétiques, tout en garantissant la stabilité des résultats. Pour ce faire, nous disposons de repères squelettiques et cutanés censés nous éclairer dans notre tâche. Or, ces valeurs de référence, érigées en véritables normes, ne correspondent pas ou plus aux critères esthétiques de notre société. La pratique d'une orthodontie moderne implique la mise à jour de références esthétiques en adéquation avec notre temps pour reconnaître et satisfaire les exigences de notre patientèle. Il est pour cela nécessaire de sonder la population via la réalisation de tests d'évaluation esthétique afin de mettre en exergue ces nouveaux critères de beauté. Le protocole comporte cependant de nombreux paramètres à maîtriser pour s'assurer l'obtention de résultats statistiquement significatifs. Leur réussite implique des choix réfléchis quant aux différents volets du protocole, choix dictés par le but de l'étude. La présente thèse dont l'objectif principal est d'analyser chacun des points du protocole des tests d'évaluation esthétique facilitera, nous l'espérerons, la démarche des chercheurs en atténuant les obstacles et difficultés.

Book excerpt: Objectifs : Cette étude pilote avait pour objectif principal de déterminer si un critère subjectif choisi tel que la coupe de cheveux (couleur et longueur) peut influencer la perception et donc l’évaluation esthétique d’un profil. L’objectif secondaire était de déterminer si cette influence dépendait de la formation des personnes interrogées (orthodontistes, dentistes ou profanes), ou de leur sexe. Il s’agissait ensuite de déterminer si cette subjectivité variait dans le temps. Matériels et Méthodes : La population étudiée était scindée en 3 jurys distincts : 30 dentistes représentés pas les étudiants en 6ème année de la faculté de Nice, 30 profanes choisis aléatoirement et 30 orthodontistes représentés par les 4ème années de CECSMO de différentes facultés de France. Toutes les personnes interrogées avaient entre 20 et 30 ans. Nous avons choisi d’étudier 4 profils de jeunes filles entre 12 et 14. Nous avons selectionné un profil normodivergent de classe I, un profil de classe II, un profil de classe III et un profil hyperdivergent. Nous avons truqué les photographies de telle façon à obtenir pour chaque profil : une photographie originale, un profil en ombre chinoise, et 2 photographies truquées sur lesquelles nous avons modifié la coupe de cheveux (couleur et longueur). Nous avons également, et uniquement pour le profil de classe I, modifié la photographie originale en lui ajoutant des « piercings ». Un diaporama contenant 17 photographies mélangées a donc été présenté aux 3 jurys, qui devaient remplir un questionnaire en cochant pour chaque profil leur évaluation esthétique correspondante (très harmonieux, plutôt harmonieux, plutôt disgracieux ou très disgracieux) et répondre à la question : pensez vous que cette patiente ait besoin d’un traitement orthodontique ? Ce même diaporama a à nouveau été présenté à chacun des jurys une semaine plus tard afin d’évaluer l’évolution des résultats dans le temps.Le test du chi deux, le test exact de Fisher, une ANOVA, et les kappas ont été utilisés pour analyser les données enregistrées. Résultats : Une forte influence des critères subjectifs choisis a été démontrée pour les profils de classe II et hyperdivergent, avec même certaines fois une amélioration esthétique de la dysmorphose, tandis que peu ou pas d’influence de ces mêmes critères n’était observée pour les profils de classe I et III. Il n’y avait pas de différence d’évaluation en fonction du sexe de l’examinateur. Les dentistes et profanes semblent toutefois moins sévères concernant l’évaluation du profil de classe II, comparés aux orthodontistes. Nous concluons que pour les profils très harmonieux (classe I normodivergent) ou très disgracieux (dysmorphose très marquée, classe III) les critères subjectifs ne semblent pas influencer la perception esthétique du profil, tandis que bien au contraire lorsqu’il s’agit de dysmorphoses peu marquées, les critères subjectifs semble réellement jouer un rôle dans la perception du profil. Nous pouvons ajouter qu’une collaboration entre l’orthodontiste et un professionnel tel qu’un visagiste semble intéressante à envisager, au même titre que celle avec un chirurgien esthétique, pour une rhinoplastie par exemple. Ainsi, l’orientation de son patient vers ce type de consultation pourrait faire partie du panel de solutions proposées par l’orthodontiste.

Le praticien doit être capable d’analyser le visage du patient dans sa globalité et de proposer au- delà des objectifs purement occlusaux, un plan de traitement qui réponde à sa demande. Dans une première partie, nous rappelons les critères d’évaluation de l’esthétique à travers les différentes étapes de l’élaboration du dossier orthodontique et à différentes échelles (macro-, mini- et micro-esthétique). Puis, nous analysons les répercussions sur les tissus mous des traitements orthodontiques avec extractions de prémolaires. Dans une seconde partie, nous présentons une étude rétrospective dont l’objectif principal est d’évaluer l’impact direct du recul des dents sur le recul des lèvres en se basant sur un échantillon de 30 patients adolescents traités par extractions de prémolaires en cabinet privé. Pour cela, nous avons analysé les téléradiographies de profil prises avant le traitement (T0) et un an après débaguage (T,). Nos résultats tendent à prouver que les traitements orthodontiques avec extractions de prémolaires n’entrainent que de faibles modifications du profil cutané. Une corrélation a été mise en évidence entre le recul des incisives maxillaires et celui de la lèvre supérieure avec un ratio de un tiers. L’épaisseur et la tonicité labiales joueraient un rôle crucial dans la réponse des tissus mous.

En effet, tout patient peut bénéficier d’une prise en charge dès lors que le traitement est initié avant le 16e anniversaire. Face au déficit croissant de la Sécurité Sociale, qui s’élevait à 30 milliards d’euros fin 2009, il paraîtrait judicieux que la prise en charge se fasse en fonction du besoin de soins plutôt que de diluer les ressources. Ainsi la prise en charge concernerait des patient ayant un besoin de soin avéré car dans le système actuel , la plus petite rotation dentaire bénéficie de la même prise en charge que le plus grand syndrome. L’utilisation d’indices permettant l’évaluation du besoin semble intéressante car il s’agit d’un moyen d’évaluation objectif. Ils sont d’ailleurs déjà utilisés dans les pays Scandinaves . Au sein de cet ouvrage, nous avons détaillé (étude de la littérature) et utilisé 2 indices : l’IOTN et l’ICON afin d’évaluer la simplicité de mise en oeuvre et les difficultés rencontrées dans leur établissement. L’IOTN (Index of Orthodontic Treatment Need ou indice d’évaluation du besoin de traitement) comprend deux composantes : une composante esthétique, une composante santé bucco‐dentaire (seul le trait occlusal le plus sévère est enregistré). L’évaluation esthétique se fait sur une planche de dix photographies allant de la denture la plus attirante à la moins attirante d’un point de vue alignement dentaire. L’ICON (Index of Complexity Outcome and Need ou indice d’évaluation de la complexité de résultat et de besoin) est plus récent et issu d’un consensus international. Il enregistre l’ensemble des anomalies et pas seulement la plus sévère. Il comporte la même composante esthétique. Il permet de plus d’évaluer la complexité et les résultats du traitement en fonction de la malocclusion initiale, ce qui paraît indispensable face aux études réalisées par l’URCAM mettant en avant que 31.7% des traitements terminés présentent au moins une dysmorphie résiduelle. L’utilisation d’indices d’évaluation du besoin de traitement permettrait de déterminer si une malocclusion nécessite réellement un traitement orthodontique et donc d’éviter un traitement chez des patients ayant un faible besoin de traitement, où le risque d’abandon est grand et où le résultat n’apporterait pas entière satisfaction. Ceci permettrait aussi de limiter les dépenses de la Sécurité Sociale.
