Ainsi, nous avons présenté un protocole spécifique à 12 sujets témoins âgés de 70 à 85 ans ainsi qu'à 36 patients MA du même âge, répartis en 3 groupes selon la sévérité de leur affection. Nous leur avons proposé des épreuves de fluence verbale, une batterie évaluant les gnosies visuelles, et enfin trois tests de dénomination orale de substantifs et de verbes. L'analyse des résultats a tout d'abord révélé une diminution significative des performances, proportionnelle au degré de sévérité de la démence, aussi bien en termes de pourcentage de réussite qu'en temps de réponse. De plus, cette étude a permis de mettre en évidence l'existence d'une dégradation lexico-sémantique spécifique au cours de la maladie, l'influence du niveau socio-culturel sur les stratégies de réponse employées, la présence d'un effet de couleur de l'image présentée ainsi qu'un relatif effet de classe grammaticale en dénomination.

Un protocole constitué d' un test de dénomination d' images (BIMM) et d' un test non validé d' évocation de synonymes, d' antonymes et d' évocation sur définition (Télexab) a été proposé à 14 sujets témoins âgés de 71 à 88 ans ainsi qu' à 14 patients du même âge. Nous avons comparé leurs résultats. L' analyse des résultats a tout d' abord révélé que Télexab met en évidence le manque du mot au même titre que la BIMM, mais nous avons retrouvé des différences dans la nature des erreurs et des réponses produites en fonction de la modalité auditive (Télexab) ou visuelle (BIMM). De plus, cette étude a permis de montrer que Télexab met en évidence les difficultés des sujets en situation quotidienne ainsi que leurs stratégies pour tenter de pallier à leur manque du mot. Nous soulignons également un effet de classe grammaticale dans les deux épreuves ainsi que la mise en évidence d' une meilleure préservation des aspects sémantiques en début d' évolution.

Ainsi, un protocole constitué d’épreuves de fluence verbale et de tests de dénomination d’images (DO 80, BIMM, planche d’évocation créée) a été proposé à 38 patients MA âgés de 70 à 90 ans. Trois groupes de MA se distinguent par leur suivi : stimulations en groupe du langage, autres stimulations, sans stimulation. Nous avons comparé leurs résultats à un groupe à un stade plus sévère de la démence ainsi qu’à 10 sujets témoins du même âge. L’analyse des résultats montre une diminution significative des performances au niveau des scores et des temps de réponse et met en évidence les bénéfices de la stimulation orthophonique sur l’accès lexical. Nous soulignons au niveau des variables observées, un effet de la fréquence du mot et de la couleur de l’image sur la dénomination au détriment de l’absence d’effet de la longueur du mot et de la classe grammaticale.

Initiée par TRAN afin d'enrichir les outils existants, elle est issue du courant théorique de la neuropsychologie cognitive et a été élaborée dans le cadre de plusieurs mémoires d'orthophonie de Lille, depuis 2005. Elle comporte trois épreuves lexicales (dénomination, désignation et appariement sémantique d'images) composées de 54 mêmes items. Elle a été normalisée auprès de 369 sujets témoins (TRAN et GODEFROY, 2011). La validation a débuté en 2011 auprès de 15 sujets pathologiques (MCI et MA stade léger). Nous avons poursuivi celle-ci auprès de 16 patients MA de stade débutant en comparant quantitativement et qualitativement les résultats obtenus à la BETL à ceux du LEXIS (outil référent de structure comparable). En accord avec la littérature, les troubles lexicaux observés à la phase initiale de la MA sont hétérogènes ; ils peuvent affecter, de manière isolée ou conjointe, la production lexicale, la compréhension lexicale et le traitement sémantique.

L'évaluation de ce trouble s'opère au moyen d'épreuves de dénomination et de fluence. Toutefois, les premières font référence à des modèles de dénomination nécessitant des capacités d'analyse visuelle. Quant aux secondes, elles sollicitent des compétences exécutives. Les modèles d'accès au lexique en production de mots paraissent plus proches de la réalité des patients atteints de la maladie d'Alzheimer en début d'évolution. TELEXAB est un test informatisé, fondé sur des modèles de production verbale de mots: il explore l'accès au lexique sur entrée verbale. Il comporte des épreuves d'évocation d'antonymes, de mots à partir de définitions et de synonymes. Il semble que ce mode d'évaluation de l'accès lexical place le patient dans des situations d'évocation proches de celles observables dans la vie de tous les jours. Cette spécificité peut permettre de révéler des difficultés analogues à celles rencontrées au quotidien et, donc, à utiliser des stratégies de réponses comparables. Enfin, un tel outil permet d'évaluer facilement l'accès aux mots abstraits. Le protocole regroupe : un questionnaire subjectif d'auto-évaluation du manque dumot, un test d'évaluation cognitive globale (MMS), un test de dénomination orale (BIMM : Batterie Informatisée du Manque du Mot) l'outil TELEXAB. Dix-neuf patients malades d'Alzheimer, âgés de 73 à 87 ans, appariés à des sujets sains y ont participé. Les corrélations entre la BIMM et TELEXAB plaident en faveur de la validité de cette nouvelle épreuve lexicale. Par ailleurs, un effet du niveau socioculturel et une corrélation avec le MMS ont été mis en évidence. Dans cette étude, l'effet grammatical en faveur des noms par rapport aux verbes n'est pas mis en lumière par TELEXAB. Enfin, l'analyse qualitative des stratégies identifiées par cette évaluation donne des perspectives de rééducation.

Book excerpt: Dès les premices de la Maladie d'Alzheimer (MA) un manque du mot apparaît fréquemment, associé aux troubles mnésiques typiques de cette neurodégénérescence. Selon les diverses hypothèses non consensuelles avancées dans la littérature, cette anomie de nature lexico-sémantique, signerait une atteinte de l'accès lexical voire une altération du système sémantique. Plusieurs études ont démontré que les patients en début de MA mettent en place spontanément des stratégies compensatoires pour pallier leurs difficultés. Au regard de ces éléments théoriques, nous nous sommes interrogées quant à l'impact d'une prise en soin orthophonique, personnalisée et adaptée aux mécanismes palliatifs mis en place par le patient, sur l'accès lexical. Pour cela, deux participants au stade initial de la MA ont été évalués au travers d'un test d'évocation : Telexab (Medina, 2015). Un profil de réponse a été dégagé pour chacun d'entre eux. Par la suite, un protocole de stimulation de l'accès lexical a été élaboré en fonction du profil et des centres d'intérêt du sujet. L'hypothèse générale avancée était la suivante : une remédiation personnalisée permettrait de développer ou de renforcer les éléments palliatifs déjà mis en oeuvre par le patient. L'objectif étant le maintien de leurs compétences lexicales aussi longtemps que possible. Les résultats ont montré un effet bénéfique de la stimulation proposée sur les capacités langagières des personnes testées et ont permis de valider les hypothèses de départ. Pour l'un des individus, le protocole lui a permis d'instaurer des stratégies palliatives - telles que des circonlocutions - qui n'étaient pas observées initialement. Pour le second, l'impact du protocole n'est pas significatif, mais un gain dans la précision des réponses a pu être relevé. Il serait intéressant de poursuivre cette recherche sur un plus grand nombre de patients, afin de valider son efficacité et rendre compte de la diversité des profils langagier au stade initial de la MA
