Elle génère une morbidité et une mortalité importantes qui demeurent élevées malgré les avancées thérapeutiques importantes de ces derniers siècles. Cependant, ces études sont peu généralisables du fait d'une population bien différente de la réalité clinique car plus jeune et moins comorbide. Nous avons voulu étudier la population hospitalisée pour insuffisance cardiaque au centre hospitalier universitaire de Grenoble-Alpes sans critères d'exclusion afin d'étudier les facteurs pronostiques. METHODE : Les patients inclus pouvaient consulter les urgences ou être hospitalisés directement dans un service de médecine conventionnel cardiologique ou non, ou un service de réanimation. Les données démographiques, cliniques, biologiques, thérapeutiques, le parcours de soin, le type de cardiomyopathie et le facteur déclenchant étaient recencés. Le pronostic à 1 an était recherché. RESULTATS : 239 patients ont été inclus du 21 novembre 2016 au 21 février 2017 inclus. La mortalité à un an était de 30% (n=26). L'âge moyen était de 80.3 ans. Après ajustements, un poids supérieur à 70 kg et un taux bas de NT proBNP sont associés à un meilleur pronostic. CONCLUSION : Le poids étant un outil simple utilisable quotidiennement au cabinet, pourrait permettre de dépister simplement les sujets âgés insuffisants cardiaques au pronostic défavorable. Des études supplémentaires sont nécessaires pour préciser si le paramètre poids chez les sujets âgés de plus de 80 ans est supérieur à l'indice de masse corporelle dans l'évaluation du pronostic de l'insuffisance cardiaque dans le cadre du paradoxe du syndrome de l'obésité.

Book excerpt: INTRODUCTION : l'insuffisance cardiaque est une pathologie fréquente chez le sujet âgé institutionnalisé. Sa prévalence est estimée à 20 % en EHPAD. Son diagnostic est compliqué par la présence de signes cliniques atypiques et de nombreuses comorbidités. Les examens complémentaires, comme l"échocardigraphie-doppler, sont sous-utilisés et il existe une sous-prescription des traitements recommandés. Notre étude a pour objectif principal d'évaluer le suivi des recommandations de la HAS dans la prise en charge des patients avec insuffisance cardiaque en EHPAD. L'objectif secondaire est d'étudier la survenue d'événements intercurrents (décompensation cardiaque aiguë, décès sur une période de six mois). MATÉRIEL ET MÉTHODES : il s'agit d'une étude descriptive observationnelle, longitudinale, réalisée dans deux EHPAD, avec étude de survenue d'événements intercurents à trois et six mois, entre le 1er Août 2018 et le 1er février 2019. Les résidents de 70 ans et plus présentant une insuffisance cardiaque ont été identifiés par analyse exhaustive de l'ensemble des dossiers médicaux. RÉSULTATS : 57 patients ont été inclus dans notre étude. La prévalence de l'insuffisance cardiaque était de 25.6%. La moyenne d'âge était de 89.4 ans, plus des trois-quarts étaient des femmes. 70% des sujets inclus ont bénéficié d'une échocardiographie, avec une FEVG conservée dans 67.5% des cas. La prescription de bétabloquants, IEC (ou ARA II) et diurétiques était respectivement de 45.6%, 73.7% et 84.2% pou񖉖񖜖 l'ensemblele de la population. 33% des patients bénéficiaient de l'association bétabloquants-IEC (ou ARA II). 26.3% des patients ont présenté une décompensation cardiaque aiguë durant l'étude. Les hommes étaient significativement plus touchés et l'insuffisance rénale était statistiquement plus fréquente dans le groupe ayant décompensé. 17.5% des patients sont décédés, les décompensations cardiaques représentaient 20% des causes de décès. CONCLUSION : L'échocardiographie est largement utilisée en EHPAD, mais la prise en charge thérapeutique n'est pas optimale, notamment en ce qui concerne la prescription de bétabloquants et IEC (ou ARA II). En témoignent les taux élevés de décompensations cardiaques aiguës et de décès.

Dans moins de 20% des cas, il s'agissait d'un séjour programmé. La fraction d'éjection ventriculaire gauche (FEVG) moyenne est de 35.9 +/- 12.9%. Un tiers des patients ont une FEVG conservée. Les patients insuffisants cardiaques (IC) à FEVG préservée sont plus souvent de sexe féminin, âgés, et hypertendus. Une cardiopathie ischémique est plus souvent retrouvée. La durée de séjour moyenne est de 10 jours. 80% des patients ont été suivis sur une période de 16 mois. 97 patients sont décédés dans le suivi. Près de 60% des patients ont bénéficié d'une éducation concernant leur pathologie, 40% ont bénéficié de l'introduction d'un béta-bloquant, 9% de l'implantation d'un multisite. 2/3 des patients ont été réhospitalisés dans les 3 premiers mois suivant l'hospitalisation initiale dans notre service. Notre prise en charge semble conforme aux données admises mais le pronostic de l'IC reste sombre.

Notre étude rétrospective se propose de préciser les caractéristiques des patients hospitalisés pour insuffisance cardiaque dans le service de médecine interne gériatrique, d'analyser leur prise en charge et leur suivi, et de rapporter les stratégies thérapeutiques mises en oeuvre chez ces patients en fonction de différents facteurs. 30 dossiers patients ont ainsi été analysés de façon très approfondie, ces patients ayant été suivis dans le service entre le lei janvier 2004 et le 30 septembre 2007. Le délai moyen de suivi était de 11.4 +/- 8.6 mois. Le nombre moyen de consultations de suivi était de 4.7 +/- 2.5 et on a dénombré 0.7 +/- 0.9 réhospitalisations pour poussée d'insuffisance cardiaque. L'âge moyen des patients est de 87.1 +/- 6.1 ans. 6.67 % des patients provenaient d'une institution, bien que 23.3 % d'entre eux aient été diagnostiqués comme déments. Les principales cardiopathies responsables étaient les cardiopathies ischémiques (43.3 %), seules ou associées, et hypertensives (33.3%), là aussi seules ou associées. La clairance moyenne de la créatinine était de 41.4 +/- 13.9 ml/min, 26.7 % des patients ayant une clairance inférieure à 30m1/min. 90% des patients ont bénéficié d'une échographie cardiaque, et 40 % des patients avaient une fraction d'éjection ventriculaire gauche supérieure ou égale à 45%. 13.33 % des patients sont traités par bêta-bloquant seul, 30 % par inhibiteur de l'enzyme de conversion (IEC) seul et 53.34 % par l'association IEC + bêtabloquant. 3.33 % des patients ne sont traités ni par IEC ni par bêta-bloquant. La mortalité globale est de 43.33 % et la mortalité cardio-vasculaire de 26.67 %. Le taux de mortalité post-hospitalier varie en fonction du type de traitement suivi : 66.67 % dans le groupe « IEC seul », 50 % dans le groupe « bêtabloquant seul » et seulement 29.41 % dans le groupe « IEC + bêta-bloquant ». Notre étude a mis en évidence des progrès considérables en matière de prise en charge des patients insuffisants cardiaques : le recours à l'échographie est désormais quasiment systématique, et la prescription des traitements de référence (IEC + bêta-bloquants) a été généralisée. Ceci a pu être rendu possible par la présence d'un cardiologue récemment rattaché au service de médecine interne gériatrique et par le suivi par une équipe pluridisciplinaire lors de consultations en hôpitaux de jour dans ce service.

The Environmental Protection Agency's decision in the summer of 2001 to delay implementing a new, more stringent standard for the maximum allowable level for arsenic in drinking water generated a great deal of criticism and controversy. Ultimately at issue were newer data on arsenic beyond those that had been examined in a 1999 National Research Council report. EPA asked the National Research Council for an evaluation of the new data available. The committee's analyses and conclusions are presented in Arsenic in Drinking Water: 2001 Update. New epidemiological studies are critically evaluated, as are new experimental data that provide information on how and at what level arsenic in drinking water can lead to cancer. The report's findings are consistent with those of the 1999 report that found high risks of cancer at the previous federal standard of 50 parts per billion. In fact, the new report concludes that men and women who consume water containing 3 parts per billion of arsenic daily have about a 1 in 1,000 increased risk of developing bladder or lung cancer during their lifetime.
