Book excerpt: La dysoralité, ou trouble de l’oralité alimentaire, peut se manifester par des difficultés alimentaires ainsi que par des troubles touchant l’ensemble du développement de l’enfant, au niveau psychomoteur, langagier et affectif (Thibault, 2007). Dans ce contexte, on peut par ailleurs identifier chez certains enfants des défenses sensorielles orales, tonico-posturales, tactiles et olfactives (Bullinger, 2005 : Senez & Schall, 2010 ; Leblanc, 2012). L’évaluation de la dysoralité nécessite donc l’intervention de différents professionnels de santé comme les orthophonistes, les psychomotriciens, les psychologues, les kinésithérapeutes et les ergothérapeutes (Hermans, 2003 ; Abadie, 2004). Or, il n’existerait à ce jour aucune démarche d’évaluation interdisciplinaire concernant ce type de plainte. Nous avons dans un premier temps cherché à savoir si, dans le cadre d’une plainte relative à l’oralité alimentaire chez des enfants de 12 à 36 mois, les orthophonistes pratiquent une évaluation spécifique par rapport aux évaluations proposées par les autres professionnels. Ensuite, nous nous sommes questionnées sur la façon de créer des liens entre les différentes disciplines afin d’obtenir une évaluation complète, prenant en compte à la fois les difficultés alimentaires et les autres signes pouvant être associés. Pour répondre à ces interrogations, nous avons interrogé, au moyen de questionnaires, 170 professionnels de santé sur leurs pratiques d’évaluation. Les résultats révèlent des spécificités mais également des points communs parmi les évaluations des différentes professions interrogées. En outre, il apparaît que les orthophonistes échangent avec des professionnels d’autres disciplines dans leur évaluation de ce trouble. Enfin, nous avons identifié une demande marquée pour un outil interdisciplinaire d’évaluation. Un outil utilisé comme support aux échanges entre les professionnels pourrait ainsi être élaboré, dans l’objectif d’offrir une évaluation globalisante de l’enfant, qui prenne en compte les multiples manifestations de la dysoralité grâce aux compétences spécialisées et conjuguées de plusieurs professionnels de santé

Book excerpt: Les difficultés alimentaires chez l'enfant peuvent entraver son développement global dans la mesure où elles sont susceptibles d'affecter le développement langagier, moteur, sensoriel et affectif (Cauhépé et al., 1968 ; Leblanc et al., 2012 ; Senez, 2015). Toutes ces composantessont imbriquées et un trouble de l'oralité alimentaire peut avoir de nombreuses conséquences. Les étiologies du trouble de l'oralité alimentaire ou dysoralité sont multiples, de même que ses manifestations souvent identifiées par des défenses sensorielles chez les jeunes patients (Leblanc et Ruffier-Bourdet, 2009). Dans ce contexte, l'interdisciplinarité a une placeprépondérante dans l'évaluation et la prise en soins de ces troubles (Hermans, 2003), or il n'existe actuellement aucun outil d'évaluation permettant une démarche interdisciplinaire dansce domaine. Par ailleurs, une demande d'harmonisation de l'évaluation de l'oralité des enfants de 12 à 36 mois a été mise en avant par Cohou et Gouineau (2013). Dans leur mémoire, lesergothérapeutes, kinésithérapeutes, orthophonistes, psychologues et psychomotriciens ont encouragé la création d'un support interdisciplinaire d'évaluation de l'oralité alimentaire. C'est dans cette perspective que s'inscrit l'élaboration d'un carnet de liaison interdisciplinaire à destination de ces professionnels. Sa création s'est basée sur les données de la littérature, le mémoire de Cohou et Gouineau (2013) et les outils standardisés indiqués comme utilisés par les praticiens concernés. Nous avons supposé que cet outil créé était pertinent au niveau de sa forme, de son contenu, de son utilisation et qu'il serait un apport pour l'évaluation de l'oralité alimentaire. Au moyen de questionnaires, nous avons donc interrogé une population de 51 praticiens afin d'en valider la pertinence. Les résultats ont révélé que le carnet de liaison proposé est un apport intéressant pour évaluer l'oralité de manière interdisciplinaire. Il permet en effet une meilleure lisibilité des observations de chacun tout en respectant les spécificités de chaque profession. Il permet également une meilleure définition des axes thérapeutiques et une synthèse des observations. Si ses modalités d'utilisation s'avèrent également pertinentes, il devra cependant être perfectionné sur le fond par l'ajout d'items le rendant plus exhaustif. Une nouvelle forme protégeant la confidentialité des données pourra également être envisagée en vue d'une utilisation par des équipes interdisciplinaires.
