Book excerpt: L'essor des Technologies de l'Information et de la Communication (TIC) a permis le développement de simulateurs de consultation virtuelle sur ordinateur appelés Patients Virtuels. Ces logiciels permettraient à leurs utilisateurs de développer, entre autres, leur raisonnement clinique. Le simulateur de consultation Alphadiag, crée en partenariat avec l'Université Claude Bernard Lyon 1, s'inscrit dans l'intégration des TIC dans l'enseignement médical. L'objectif principal de ce travail était d'évaluer la perception des étudiants vis à vis de l'utilisation du simulateur de consultationflAlphadiag en formation médicale initiale. Une étude pilote descriptive a été réalisée auprès d'étudiants volontaires inscrits en 5ème année de médecine de la Faculté de Médecine Lyon-Est. Au préalable, 4 cas cliniques inspirés de situations-types du référentiel métier et compétences en médecine générale ont été conçus spécifiquement pour l'évaluation du simulateur Alphadiag. Un questionnaire évaluant l'outil et chacun des 4 cas cliniques a été renseigné par tous les participants volontaires au cours de 2 sessions. Un pré-test et un post-test ont également été renseignés pour chaque cas clinique. Un test des rangs de Wilcoxon pour données appariées a été réalisé pour comparer les moyennes des pré-tests et des post-tests. Vingt étudiants ont accepté de participer à l'évaluation du simulateur Alphadiag. La grande majorité d'entre eux a trouvé le logiciel facile d'utilisation (85%), réaliste (100%) et convivial (100% ). La totalité des étudiants a jugé le logiciel intéressant comme outil pédagogique (100%). La majorité des étudiants a jugé les cas cliniques intéressants, utiles pour leur formation, de bonne qualité et d'un intérêt identique ou supérieur par rapport à un cas clinique classique. Une amélioration non statistiquement significative des connaissances théoriques a été observée pour chacun des 4 cas cliniques. Ces résultats devront être confirmés par une étude ultérieure afin de vérifier les bénéfices du logiciel à long terme sur le développement du raisonnement clinique des étudiants. Avant l'intégration de ce simulateur dans le curriculum médical, son type d'utilisation sera à définir (formative ou sommative). Son utilisation en formation médicale initiale offrirait la possibilité d'un apprentissage mixte (présentiel et à distance) ce qui renforcerait probablement la motivation des étudiants. Une attention particulière sera à porter sur la qualité des cas cliniques qui devront être validés par des experts cliniciens et enseignants, afin de permettre leur labellisation universitaire

La simulation est un excellent moyen de formation et cette étude souligne que pour la majorité des DUMG la simulation intervient dans la formation ou la formation et l'évaluation mais pas l'évaluation seule. La simulation devient un outil indispensable de la formation pour garantir la sécurité des soins aux patients ainsi que le respect des principes éthiques. Reste à définir sa place dans le cursus des études médicales.

Elle fait partie des compétences indispensables à acquérir lors de la formation initiale en médecine générale. Cette étude décrit l'élaboration et l'évaluation de séances d'enseignement de la communication par la simulation de consultations en milieu ambulatoire, à destination des internes de première année de médecine générale. Notre méthode pédagogique reposait sur la participation de patient simulés. Les consultations, avec l'interne de première année dans le rôle du médecin, étaient filmées et retransmises en directe. L'innovation résidait dans le fait que les patients simulés étaient des pairs internes en deuxième année de médecine générale. La communication du médecin était évaluée par l'intermédiaire du guide Calgary Cambridge : auto-évaluation et hétéro-évaluation par les enseignants et pairs observateurs. Les opinions des internes sur l'enseignement ont été recueillies par entretien semi directifs. Une analyse thématique de ces entretiens a ensuite été réalisée. Deux séances de simulation se sont déroulées dans les locaux d'une Maison de Santé Pluriprofessionnelle de Nice, en présence d'internes de médecine générale, d'étudiants en psychologie, d'un enseignant en médecine générale et d'un psychologue. Concernant l'évaluation formative : l'hétéro-évaluation de la communication était concordante entre les enseignants alors que l'hétéro-évaluation réalisée par les pairs était plus hétérogène. Les étudiants, très satisfaits de cet enseignement, souhaitent la généralisation de ces séances. Pour eux le recours aux patients simulés, l'environnement proche du réel, l'implication des pairs et l'inter-professionnalité sont des leviers à l'apprentissage de la communication.

L'analyse de la littérature fait la démonstration de l'efficacité des méthodes de simulation en santé en terme d'amélioration significative des connaissances, des pratiques et des comportements dans la formation initiale ou continue des professionnels de santé, grâce à une pratique réaliste à la fois expérientielle et réflexive. Son utilisation dans le cadre du DPC est désormais encouragée et recommandée par la HAS. Il existe peu de données spécifiquement sur la place de la simulation en santé en médecine générale. Sujets et Méthodes : notre travail visait donc à étudier l'état des connaissances et de la pratique ainsi que l'intérêt porté par les médecins généralistes à la simulation en santé au moyen d'un questionnaire en ligne adressé aux praticiens de médecine générale du Maine et Loire. Résultats : 210 praticiens ont répondu au questionnaire soit un taux de réponse de 40 %. Les résultats montrent qu'il existe un déficit de connaissance (23,44 % n'en avait jamais entendu parler) et de pratique (46,12 % n'avait jamais pratiqué en formation initiale ou continue), en particulier chez les plus âgés et portant surtout sur la formation continue (21,84 % des praticiens seulement). Ils y portent pour autant un grand intérêt puisque l'immense majorité pense que la simulation serait un outil utile pour leur formation continue (92,38 %) et est prête à participer à des programmes dédiés (86,67 %) portant aussi bien sur des compétences techniques (gestes d'urgence, gestes techniques) que non techniques (communication, éducation thérapeutique, travail en équipe). Conclusion : il apparaît donc nécessaire de mieux porter la simulation à la connaissance des praticiens et de développer des programmes utilisant cette méthode et dédiés à la médecine générale qui viendraient enrichir l'offre de formation continue déjà existante.

Although typical aging does not cause low vision, four major causes of vision impairment are directly related to the aging process: (1) age-related macular degeneration (AMD), (2) diabetic retinopathy, (3) glaucoma, and (4) cataracts. Instances of these impairments are expected to double by the year 2050, making low vision a significant public health issue. This Practice Guideline includes occupational therapy assessment and intervention guidelines for older adults who have visual acuity impairments, visual field impairments, or both as a result of a low vision diagnosis. Screening, referral, and evaluation are covered for clients for central visual field impairments, such as AMD and cataract; peripheral field impairments, such as glaucoma, hemianopsia, and quadrantanopsia; and mixed visual field loss, such as diabetic retinopathy. This work can help occupational therapists and occupational therapy assistants, as well as individuals who manage, reimburse, or set policy regarding occupational therapy services, understand the contribution of occupational therapy to treating older adults with low vision. Appendixes include selected diagnostic and billing codes and evidence tables.
