Book excerpt: Objectifs : Evaluer l’efficacité d’un programme d’éducation thérapeutique destiné aux personnes migrantes vivant avec le VIH, s’exprimant un minimum en français, avec comme critère principal de permettre aux personnes de mieux connaître et comprendre le fonctionnement des principaux organes de leur corps, ainsi que de mieux appréhender leur maladie et le traitement. Méthodologie : Etude pilote rétrospective de type avant-après. Chaque session du programme comprenait deux parties : une sur le corps (organes, système immunitaire, nutrition), l’autre sur la maladie, le suivi médical, le traitement, les effets secondaires. Plusieurs outils pédagogiques étaient utilisés (mannequin, films, schémas, .....) selon une méthode participative. Trois évaluations ont été faites : avant , en fin de programme et 3 mois après. Résultats : De 2005 à 2009, 8 sessions ont été organisées, regroupant 77 migrants vivant avec le VIH. Après le programme, on a observé une augmentation des connaissances de 33% avec une déperdition de moins de 4% au cours du suivi à 3 mois. 97% des participants déclarent être satisfaits du programme et estiment avoir les connaissances nécessaires pour améliorer leur observance du suivi médical et des traitements (95%), leur relation avec leur médecin (75%), et leur qualité de vie (74%). Conclusion : Le programme a atteint ses objectifs puisque les connaissances de base sur les thèmes développés étaient toutes acquises en fin de programme et maintenues lors du suivi. Au vu de l’impact positif de l’Ecole de Sante au près de la population cible, cela conforte la place des associations dans la réalisation de ce type de programme d’éducation thérapeutique du patient.

Book excerpt: L'infection par le VIH étant désormais une maladie « chronique », l'éducation thérapeutique (ETP) est devenue très importante. Au sein de ce service de Maladies infectieuses prenant en charge environ 300 personnes vivant avec le VIH (PvVlH), un programme d'ETP a été validé par l'ARS dès 2011. Un espace dédié permet de réaliser des séances individuelles et une équipe pluridisciplinaire propose également des séances collectives. Les PvVlH suivies ici sont majoritairement précaires et ont souvent une langue maternelle autre que le français.Objectifs : Evaluer et améliorer le programme d'ETP en proposant une consultation pharmaceutique (CP) individuelle. Matériels et Méthodes : Elaboration d'un questionnaire d'évaluation des acquis à 3 dimensions (qualité de vie, connaissances, évaluation du programme) avec 35 questions au total, discuté et validé avec l'ensemble des membres de l'équipe. Ont été évalués tous patients ayant bénéficié d'au moins une séance d'ETP dans l'année. Les évaluations ont duré 7 mois (mars à septembre 2019). Dans un second temps, la CP a été conçue afin de renforcer les compétences des patients en matière de sécurité relatives à leur traitement antirétroviral (ARV) et d'adaptation. Un dossier pharmaceutique a été élaboré comprenant le bilan médicamenteux, un jeu éducatif et une évaluation de la séance (degré de satisfaction et de connaissances).Résultats : Le questionnaire d'évaluation a été testé auprès de 30 patients : 26,7 % des personnes ne savaient pas citer le nom de leur(s) médicament(s), 90 % prenaient correctement leur ARV, 40 % oubliaient au moins une fois par mois la prise des ARV, 50 % ne savaient pas comment gérer les oublis, 29,6 % des patients concernés ne savaient pas citer les effets indésirables liés à leur ARV, 83,3 % avertissaient les professionnels de santé en cas de médicament prescrit par ailleurs, 60 % ne connaissaient pas les deux principaux indicateurs de suivi du VIH (charge virale, CD4+), 80 % savaient gérer leur traitement ARV lors d'un déplacement, 73,3 % savaient préparer un séjour prolongé à l'étranger. La CP a été testée auprès de 22 patients et semble bénéfique. Après la consultation, 9,1 % des patients ne savaient pas citer les médicaments de leur traitement, 95,5 % utilisaient un moyen de stockage approprié pour les prises médicamenteuses, 90,9 % planifiaient bien la prise de leur traitement sur 24h, 18,2 % savaient expliquer partiellement pourquoi le traitement devait être suivi sans interruption et 81,8 % savaient bien l'expliquer, 50 % savaient adaptaient partiellement les horaires de prise lors de circonstances exceptionnelles et 50 % savaient bien l'adapter, 85,7 % des patients concernés géraient bien les effets secondaires les plus fréquents, 4,5 % ne comprenaient pas les dangers de l'automédication et 22,7 % le comprenaient partiellement, 4,5 % géraient partiellement leur stock de médicaments lors d'un séjour prolongé Discussion - Conclusion : La mise en place d'une CP individuelle intégrée au programme d'ETP doit permettre d'améliorer la prise en charge des PvVlH. La CP vient s'associer aux séances collectives dédiées aux médicaments. Elle doit être consolidée, puis élargie au plus.grand nombre. A terme, ce type de consultation pourrait être étendu vers la ville.

Y a-t-il eu des répercussions sur les consultations médicales et les changements de traitements ? Les bons résultats obtenus sur la charge virale se sont-ils maintenus à 24 mois de suivi ? Matériel et méthodes : Une étude randomisée menée fin 1999 a inclus 244 patients sous HAART, 123 dans le groupe intervention (GI) bénéficiant d'entretiens de counseling et 121 dans le groupe témoin (GT) avec un suivi médical traditionnel. A 6 mois, les patients des 2 groupes ont pu bénéficier des entretiens d'observance. Les données recueillies sur une période de 24 mois portent sur la fréquentation des entretiens d'observance et des consultations médicales, sur les changements de traitements et les résultats immunovirologiques. Résultats : Au cours des 24 mois d'étude, les patients du GI ont poursuivi les entretiens infirmiers tandis que les patients du GT en on peu, ou pas, bénéficié, le nombre de consultations médicales est significativement plus élevé dans le GT (p
