Book excerpt: Contexte: Une diversité de prise en charge des patients avec un syndrome coronaire aigu sus ST , liée à une multiplicité des circuits, sans évaluation médico économique de l'efficience des parcours en préhospitalier. Objectif : étude médico économique des circuits basée sur les différentes alternatives qui se présentent au patient. Méthode : Une étude coût-efficacité effectuée à partir du registre Fast-MI. Le critère d'efficacité retenu est la procédure de reperfusion quelque soit les délais. Résultats : Sur 1714 patients, 26% des patients ont une prise en charge efficiente. Les cas les plus critiques suivent des circuits compromettant l'éligibilité de la thérapeutique. Les résultats régionaux sont homogènes malgré des disparités liées aux circuits. Conclusions : Une meilleure information des patients et des professionnels de santé permettrait d'éviter la dispersion des circuits empruntés par les patients dans une approche régionale.
