Book excerpt: The Picture Exchange Communication System® (PECS®) is often used by children diagnosed with autism spectrum disorder (ASD) as a means of functional communication. Although there is extensive research indicating that PECS is an evidence-based intervention for children with ASD (e.g., Wong et al., 2015), little is known about how best to train parents to support their child's PECS use. Of those studies that do explore parent training approaches, few measure the caregiver's fidelity implementing PECS or explore whether parents generalize or maintain skills post-training. Similarly, little is known about how to train parents to implement PECS via telehealth. The purpose of the current study was to bridge the gap between PECS and telehealth research and to explore strategies to help parents support their child's PECS use at home. One father-mother dyad was recruited. The father was the primary training recipient (i.e., parent trainee). The mother participated in training sessions as the role play partner (i.e., surrogate parent). Researchers used behavioural skills training (BST) to teach target PECS skills and applied strategies of general case training (GCT) to actively program for generalized behaviour change. A multiple baseline design across skills was used to monitor the father's fidelity during mediator training sessions and a multiple probe design was embedded to monitor both the father's and mother's fidelity in the natural environment with their child. Results demonstrated that the parent trainee acquired PECS skills within the training setting. However, parents did not reliably demonstrate all of the PECS skills in the generalization setting during follow-up.

Book excerpt: The picture exchange communication system (PECS) is a functional communication system frequently used with individuals diagnosed with autism (Frost & Bondy, 2002). Few empirical investigations have evaluated staff training procedures to train staff how to teach consumers to use PECS, and none have been published to date on training staff to promote the use of PECS outside of the training environment. Using a multiple probe design the current investigation evaluated staff training procedures for teaching Phases 1-3 of PECS to adults with autism spectrum disorders, with direct care staff. Typical staff training procedures resulted in little improvement from baseline performances for each participant. Performance following behavioral skills training improved significantly, approaching mastery criterion levels.

Book excerpt: The Picture Exchange Communication System (PECS) is an alternative augmentative communication system that uses a variety of behavioural techniques to teach nonspeaking individuals functional communication skills. Ten empirical evaluations of PECS have been reported in the literature since it was developed, many of which have considerable weaknesses such as lack of a baseline assessment of using PECS, lack of relevant discrimination skills of the participants, and a lack of treatment integrity measures. The purpose of this study was to provide further empirical data regarding the efficacy of PECS with adults with severe intellectual disabilities. During initial baseline assessments of three participants, all three demonstrated some of the skills required for PECS (e.g., removing a picture from a binder and giving it to the experimenter). However, none of the participants were able to reliably select the food item that corresponded to the picture they had selected. Therefore, training for this skill was provided in a modified multiple-baseline design across participants. Participants 1 and 2 met mastery criterion after considerable training using individualized fading programs. Participant 3 showed an overall improvement across baseline assessments and therefore training was not implemented. Correspondence between picture and subsequent item selection is an important component of PECS and results of this study suggest that this skill may require extensive training for some persons with severe intellectual disabilities. Baseline discrimination skills that could significantly impact learning efficiency and future research are discussed.

Book excerpt: PUBLIC ABSTRACT: As more individuals with intellectual disabilities live in integrated, community group home settings, it is increasingly important that direct support staff are well trained to implement behavioral interventions. However, there are often barriers to providing staff members with sufficient training. For example, there is typically a low supervisor-staff ratio. These studies evaluated the effects of watching a video model and completing a brief quiz on staff's ability to implement Phases I (study 1) and II (study 2) of the Picture Exchange Communication System (PECS) in group homes for adults with intellectual disabilities. Moreover, the effects of this staff training on client's correct use of PECS was also evaluated. In study 1, all four staff member's performance improved after watching the video and taking the quiz. Likewise, in study 2, all three staff members' performance improved after watching the video and taking the quiz.

Book excerpt: Abstract: Rubinstein-Taybi Syndrome (RTS) is a rare congenital neurodevelopmental disorder that often presents with corresponding speech and language delays. However, the available literature on communicative development in RTS is currently very limited. The purpose of this retrospective single-case research study was to evaluate the effectiveness of the Picture Exchange Communication System (PECS), an aided augmentative and alternative communication (AAC) system developed to teach functional communication to children with limited communication, on communicative abilities in RTS by providing a detailed profile of the intervention procedures utilized for a 6-year-old child with RTS. The aim of this investigation was both to contribute to the existing literature on the syndrome, as well as to document the success of the PECS system in children with RTS. Specifically, the current study explored the participant communicative progression and development in the areas of (a) communication initiation, (b) vocalization, and (c) eye contact after Phases 1-4 of the PECS protocol were implemented. Data records from 26 evaluation and intervention sessions completed during the participant Spring 2014 attendance at the CSULB Department of Speech-Language Pathology Speech and Language Clinic were analyzed in order to evaluate the effects of the PECS protocol on the participant communicative abilities. Results indicated that the PECS treatment significantly improved the participant communicative abilities, namely, by increasing her initiation of communicative exchanges, increasing use of vocalizations and word/phrase approximations, and increasing eye contact with communication partners. The current study supports the use of the PECS protocol in children with RTS as a functional communication system.

It has been established that The Picture Exchange Communication System (PECS) is useful in teaching children a functional communication system. Schwartz and Garfinkle (1998) found PECS to be an effective tool in teaching social skills to children with disabilities. The purpose of the present research was to replicate the findings of Schwartz and Garfinkle and to extend the literature. A multiple baseline across behaviors (i.e., greetings, requests, and responses) was used to assess the effects of PECS on social communication between peers and also examine whether the use of PECS would be effective in increasing the spoken language of these children when communicating with peers. Results of this study indicated that PECS was an effective teaching tool in increasing social communication between peers and increasing spoken language. Social validity questionnaires determined that teachers and parents found the social communication skills to be important and that this intervention was helpful.

Book excerpt: The current literature regarding the Picture Exchange Communication System (PECS) demonstrates that some individuals diagnosed with autism acquire speech or gain increases in speech during PECS training; however, the current research base has several limitations. There is a lack of research regarding possible explanations for those speech gains, including possible pre-requisite skills for speech acquisition or procedures that can account for speech acquisition. Some research suggests a link between imitation (both motor and vocal) and language acquisition. The current study examined whether individuals with a generalized motor imitative repertoire are more likely to develop speech during PECS training than individuals without a generalized imitative repertoire. This study also served as a replication of a preliminary study. Results of both the preliminary study and current study are compared and discussed.
