Available in PDF, EPUB and Kindle. Book excerpt: Contexte : Les modalités de prescriptions de psychotropes chez les patients atteints de cancer restent assez peu connues en milieu hospitalier. Notre objectif était donc de décrire la prévalence et les indications de prescription de psychotropes chez les patients atteints de cancer hospitalisés dans un centre de lutte contre le cancer. Méthode : La proportion de séjours avec prescription de psychotropes a été évaluée sur une série de séjours consécutifs chez les patients hospitalisés au Centre Oscar Lambret (COL) à Lille du 8 janvier 2018 au 16 février 2018. Tous les patients âgés de 18 ans ou plus, présentant un diagnostic de cancer, quel que soit le stade, et hospitalisés au moins une nuit, ont été inclus, quel que soit le motif d'hospitalisation. Résultats : Parmi les 179 patients inclus dans l'étude, 89 d'entre eux (soit 50 % des patients inclus ; intervalle de confiance à 95% : 42% à 57%) ont reçu au moins un psychotrope durant leur hospitalisation. 75 patients (soit 84% des patients sous psychotropes) n'en recevaient qu'un ou deux. La moyenne de psychotropes par patient était de 1.8 (SD = 1.1). Les anxiolytiques, principalement les benzodiazépines, étaient les psychotropes les plus couramment prescrits avec 32% des patients concernés, suivis des antidépresseurs (16%), des antipsychotiques (14%), des hypnotiques (11%) et enfin des thymorégulateurs (1%). Les principales indications étaient la détresse psychologique et les troubles du sommeil. Au total, 42 patients (23% des patients inclus) ont présenté au moins un trouble psychiatrique et 12 patients (7%) ont bénéficié d'un suivi psychiatrique durant leur hospitalisation. La pertinence de prescription de psychotropes a été jugée discutable chez seulement 10 patients (11%) ; il s'agissait principalement de prescriptions pour des troubles du sommeil. Le psychiatre du COL n'était à l'origine que d'une seule prescription de psychotropes. Conclusion : La majorité des patients de l'étude bénéficiait d'une prescription de psychotropes pour des indications variées. Les troubles psychiques étaient fréquents dans cette population. Ces résultats soulignent la nécessité d'améliorer l'accompagnement psychique du patient atteint de cancer notamment en renforçant la sensibilisation et la formation de l'ensemble des professionnels à détecter voire traiter la détresse psychique quand cela leur est possible.

En 2013, le montant des remboursements concernant les médicaments psychotropes s'élevait à plus d'un milliard d'euros ce qui représente en termes de volume plus de 154.6 millions de boîtes vendues. J'ai réalisé une étude sur la consommation des médicaments psychotropes en milieu rural et j'ai comparé les résultats obtenus aux données nationales et aux recommandations officielles. L'analyse de 450 ordonnances recueillies en milieu rural et les données nationales se rejoignent : la consommation de médicaments psychotropes augmente avec l'âge et les femmes sont plus consommatrices que les hommes. L'ensemble des résultats souligne également un manque de suivi spécialisé et un mésusage concernant les benzodiazépines anxiolytiques et hypnotiques. Des actions sont donc nécessaires afin de promouvoir une consommation raisonnée des médicaments psychotropes et de maîtriser les risques liés à leur mésusage.

Chez les patients traités par un médicament de substitution aux opiacés, il est difficile de savoir si la consommation associée de benzodiazépines concerne leur substitution, le traitement d'une comorbidité psychiatrique, ou si elle relève d'un abus de psychotropes. Dans le cadre d'une étude menée en officine sur la coprescription de psychotropes chez les patients présentant une ordonnance de buprénorphine, nous avons observé que les patients maintenus sous générique de la buprénorphine ont un nombre de psychotropes codélivrés et des posologies de buprénorphine plus faibles que les patients maintenus sous Subutex®. Ce travail tend à montrer que le maintien du traitement par un générique de la buprénorphine est principalement retrouvé chez les patients les mieux stabilisés sur le plan de leur substitution, et qu'ils ont moins de comorbidité psychiatrique et/ou moins de consommation abusive de psychotropes.
