Dans la causation du sujet, des opérations logiques doivent se produire, opérations qui caractérisent le sujet dans sa dépendance signifiant au champ de l'Autre. Nous pouvons situer l'autisme comme une entrave dans ces opérations. Ainsi, l'autisme est la conséquence de la non-installation de ce lien, due à plusieurs facteurs, parmi lesquels s'incluent les aspects génétiques et psychiques. Ce travail n'est pas axé sur l'autisme, il vise à penser les effets d'un enfant autiste sur sa famille. Des recherches actuelles montrent que le risque pour les frères et soeurs d'enfants autistes d'être également atteints est d'environ 4 à 18%. L'objectif spécifique de cette recherche est de vérifier l'existence de signes de risque psychique dans la fratrie de l'enfant autiste. Pour ce faire, nous utiliserons : les indicateurs cliniques de risques pour le développement — IRDI . Les signes de la recherche PREAUT. L'évaluation Psychanalytique à l'âge de 3 ans - AP3 ; et Le Child Behavior Checklist - CBCL dans la fratrie des enfants atteints d'autisme. Afin de vérifier s'il existe un lien entre la sévérité de la maladie de l'enfant autiste et les difficultés de développement dans la fratrie, les enfants autistes seront évalués par La Childhood Autism Rating Scale - CARS. Les résultats seront articulés de façon à analyser les manifestations singulières exprimées par ces enfants à partir des tendances trouvées dans les résultats quantitatifs. Le but est de réfléchir à la façon dont chaque enfant peut trouver ses propres réponses à ce qui le précède.

Book excerpt: OBJECTIF : étudier l'adéquation entre les prises en charges proposées actuellement aux frères et sœurs d'enfants autistes et atteints de troubles envahissant du développement (TED), et leurs besoins. METHODE : 1) Analyse bibliographique permettant une étude éclectique des liens fraternels, de l'Autisme et des TED et de leurs retentissements sur la fratrie. 2) Etat des lieux des prises en charge proposées actuellement sur Paris pour les fratries dans les réseaux de soins pédopsychiatriques et associatifs, réalisé à l'aide d'un questionnaire et d'entretiens avec les professionnels. RESULTATS : Les liens fraternels sont fondamentaux dans le développement psychique de l'individu de la naissance à la vieillesse. La confrontation à l'enfant autiste constitue pour la fratrie un traumatisme psychologique. Les retentissements de l'autisme sur la fratrie sont avérés et multiples, mais générateurs de symptômes peu bruyants. Il existe aujourd'hui sur Paris un manque de soins et de soutien proposés aux fratries, ainsi qu'un manque d'information des équipes soignantes sur les retentissements fraternels de l'autisme et des TED. Néanmoins les prises en charge actuelles s'organisent autour de deux perspectives : Un axe de repérage des signes précoces d'autisme et de prévention pour les puinés, et un axe de soutien ou de soin psychothérapeutique. CONCLUSION : II existe une inadéquation entre les besoins des fratries et l'offre des prises en charge qui leur sont actuellement proposées. La formation des soignants sur les problématiques fraternelles, combinée au tact du clinicien nous parait essentielle pour une mise en place éventuelle et adaptée, de prises en charges pour les fratries, selon les cas.

Ces donn es justifient pleinement d'avoir une perspective th rapeutique globale de l'enfant autiste et une certaine vigilance l' gard des fr res et soeurs. Ce travail, qui interroge plusieurs niveaux de lecture souvent pr sent s comme antagonistes, est un plaidoyer pour une vision authentiquement pluridisciplinaire de la psychiatrie de l'enfant et de l'adolescent.

Au sein même de l'autisme, des différences langagières existent également, chaque enfant développant un profil qui lui est propre. Ce sont ces divergences que nous avons entrepris d’étudier dans ce mémoire, qui a pour objectif d’analyser de manière qualitative le développement langagier de trois enfants autistes de même âge réel. Une analyse de l’émergence des pré-requis ainsi que des comportements langagiers nous a permis de mettre en évidence une certaine cohérence dans le développement de ces enfants. Des troubles inhérents à l’autisme interfèrent dans cette évolution, néanmoins un langage semble pouvoir se construire de manière relativement logique au vu du handicap que présente chaque enfant. Leur communication verbale et non verbale présente des particularités qui entretiennent un lien avec des comportements observables précocement. Dès lors, un travail orthophonique précoce et soutenu pourrait permettre de révéler une cohérence dans le développement et aider l’enfant autiste dans ses acquisitions langagières. De plus, il ressort de cette étude que les enfants atteints d’autisme présentent des profils langagiers différents qui nécessitent des prises en charges individualisées et adaptées.

L'objectif de cette étude est de rechercher si les frères et sœurs de personnes avec autisme instaurent des adaptations pour communiquer avec elles et si elles jouent avec elles. À travers douze entretiens, menés à l'aide d'un questionnaire, avec des frères et sœurs de personnes avec autisme, il apparaît que les frères et sœurs de cette étude adaptent leur langage oral, utilisent des adaptations visuelles et tactiles et mettent en place une gestion particulière des échanges. Ils développent leur observation et leur compréhension de leur frère ou de leur sœur avec autisme. Les impasses de la communication dans les fratries sont systématiquement liées à la symptomatologie autistique. Les frères et sœurs de personnes avec autisme de cette étude jouent avec elles, surtout à des jeux sensori-moteurs de contact physique. Toutefois, les jeux sont souvent limités par les difficultés des personnes autistes avec le tour de rôle. Ces résultats dessinent des pistes de travail de l'orthophoniste avec les frères et sœurs de personnes avec autisme.
