Cette prise de conscience amorcée dans le passé devrait se traduire par une réorientation de la recherche vers cette population et une application des recommandations internationales, relancées par la Commission Européenne et l'Agence Européenne des Médicaments et récemment confirmées par le Rapport "Politique du médicament en EHPAD, 2013". Tout le mérite de l'auteur est d'avoir instruit son dossier sur les différences à considérer chez le sujet âgé (en pharmacocinétique, en pharmacodynamie ou en profil de tolérance) pour définir les particularités méthodologiques des essais thérapeutiques à mener, particularités qui ouvrent la voie à une nouvelle réglementation tout à fait propice à des initiatives et à de nouvelles discussions permettant de rapprocher prescription de médicaments et situation clinique réelle.

On estime que l’iatrogénie est responsable de près de 20% des hospitalisations chez les plus de 80 ans. Chez le sujet âgé, le vieillissement physiologique et la fréquence des comorbidités modifient le rapport bénéfice-risque des médicaments. Le sujet âgé est soumis d’une part, à des modifications pharmacocinétiques et pharmacodynamiques liées au vieillissement. D’autre part, il présente plusieurs pathologies qui justifient la prescription de traitements multiples, modifiant les objectifs thérapeutiques et le risque iatrogène. Pour améliorer la qualité de la prescription médicamenteuse il est nécessaire de connaître ces facteurs et les évaluer systématiquement. Certains problèmes liés à thérapeutique médicamenteuse chez la personne âgée peuvent être évités grâce à une analyse pharmaceutique des prescriptions rigoureuse et systématique. Ces problèmes liés à la thérapeutique médicamenteuse (PLTM) sont facilement détectables à l’aide d’outils informatiques, notamment dans le cadre d’un processus informatisé de sécurisation du circuit du médicament. L’objectif principal de notre étude est de calculer une criticité théorique, selon une méthode inspirée de l’AMDEC, pour chaque PLTM identifié dans trois unités de l’EHPAD du CHU de Dijon, afin de les hiérarchiser du point de vue du pharmacien, et du médecin. Parmi 117 ordonnances (soit 981 lignes de prescription) analysées un jour donné, le 19 janvier 2012, 159 PLTM différents ont été identifiés. Les 10 PLTM les plus critiques ont été détaillés, puis une synthèse commune, pharmaceutique et médicale a été établie. Ce travail insiste sur l’importance d’une prise en charge multidisciplinaire dans le cadre de la sécurisation du circuit du médicament.
