Book excerpt: L'aphasie primaire progressive, décrite par Mesulam en 1982, est une affection dégénérative, caractérisée par l'altération sélective du langage, d'apparition insidieuse et d'aggravation progressive, en l'absence de contexte démentiel pendant au moins 2 ans ; elle est associée à une atrophie corticale focale dans les régions pérysylviennes gauches. Bien qu'il existe une grande hétérogénéité clinique, on décrit classiquement deux tableaux sémiologiques d'aphasie primaire progressive distincts (fluent et non fluent) qui comportent pour symptôme essentiel commun, un manque du mot. Certains auteurs incluent dans ce cadre sémiologique l'anarthrie progressive qui débute par un trouble du langage au sens large, puisqu'il concerne la parole, à savoir une anarthrie associée à une praxie bucco-faciale. Dans tous les cas, des troubles des autres fonctions cognitives apparaissent secondairement aux troubles du langage et réalisent plus ou moins tardivement un tableau démentiel. Nous avons étudié le profil évolutif des troubles du langage et des autres fonctions cognitives de 11 patients (7 avec aphasie progressive primaire non fluente, 3 avec aphasie primaire progressive fluente et 1 avec anarthrie) afin de dégager, au delà de l'hétérogénéité clinique, des caractéristiques évolutives spécifiques concernant les troubles du langage et les déficits associés. Cette étude a permis de déterminer certaines caractéristiques sémiologiques concernant l'installation des troubles, en fonction de la classification fluente/non fluente. Ces profils sémiologiques, confrontés aux données de l'imagerie cérébrale morphologique et fonctionnelle, ont permis de déterminer certaines corrélations anatomo-cliniques. En revanche, l'évolution des déficits linguistiques et des troubles associés a montré, qu'au delà de l'hétérogénéité clinique, il existe aussi une grande variabilité évolutive dans l'aphasie primaire progressive.

De nombreux auteurs ont réalisé des études détaillées sur le langage oral de patients atteints d'une APP, en s'attachant plus particulièrement au symptôme prédominant, à savoir, l'anomie. En revanche, peu se sont intéressés au langage écrit, ne réalisant alors que quelques tests, parfois non spécifiques, pour décrire la lecture et l'écriture. C'est pourquoi, nous avons étudié le profil évolutif de 3 patients porteurs d'une APP en nous attachant à décrire plus précisément leur langage écrit. Nous avons pour cela créé un protocole de lecture et d'écriture dictée de mots isolés évaluant l'influence de différentes variables psycholinguistiques : régularité orthographique, fréquence, longueur, concrétude, lexicalité et classe grammaticale. Au-delà de la variabilité évolutive de chaque patient, ce travail a permis de mettre en évidence une altération du langage écrit concernant essentiellement l'écriture, qui, en référence au modèle cognitif à deux voies d'interprétation du langage écrit, situe les difficultés soit au niveau central, soit au niveau périphérique.
