Book excerpt: Introduction : Le but de ce travail était de définir les déterminants du choix des parents concernant le suivi médical de leur enfant et de mettre en parallèle les attentes de ces derniers et le suivi effectif de l’enfant. Matériel et méthodes : Dans le cadre d’une étude de cohorte prospective multicentrique un premier questionnaire (phase 1) a été distribué dans quatre maternités Picardes évaluant le souhait de 330 parturientes pour le suivi médical de leur enfant nouveau-né. Dans les 6 à 12 mois suivant la naissance, nous les avons interrogées (phase2), par entretien téléphonique, sur le suivi effectif dont avait bénéficié leur enfant. Résultats : Pour le suivi médical la population de phase 1 s’orientait à 38% vers le pédiatre, à 35% vers le médecin généraliste (MG) et à 13% vers la PMI. La confiance était le premier critère de choix cité par les parents choisissant le MG alors que c’est le choix d’un spécialiste qui motivait le suivi par un pédiatre ou par la PMI. On notait que 14% des patientes n’avaient pas encore fait de choix. Elles s’étaient finalement tournées à 59% vers le MG. En phase 2, les MG avaient suivi 51% des enfants, les pédiatres 32%, et la PMI 15,5%. 63% des parents avaient respecté leur choix initial et le fait de prendre sa décision seule était pourvoyeur de changement dans le suivi médical de l’enfant. Au cours de leur première année 48% des enfants avaient eu un suivi mixte au cours duquel la répartition des rôles entre les différents praticiens semblait assez évidente pour les parents. L’étude a montré l’influence significative du type de foyer, avec un désintérêt des familles monoparentales pour les pédiatres et de l’âge maternel avec une préférence de la PMI pour les plus jeunes et du pédiatre pour les plus âgées. Le niveau d’étude était significativement plus élevé dans la population suivie par le pédiatre, dont les patients étaient plus fréquemment issus de communes urbaines. L’étude du délai d’accès estimé a montré que le délai le plus court était déterminant pour le MG et la PMI, mais pas pour le pédiatre. Ni les parcours de soins de la mère et de l’enfant, ni la place dans la fratrie ne semblaient influencer le type de suivi médical. Conclusion : Si initialement c’est le choix du pédiatre qui prime de peu sur celui du MG, c’est ce dernier qui est largement favorisé pour le suivi effectif. Le choix du pédiatre est celui d’un spécialiste, celui des parents résidant en communes urbaines, les plus diplômés, les plus âgés. Le recours au MG est majoritaire, c’est celui de la confiance et de la compétence, il est aussi motivé par des raisons pratiques telles que le délai d’accès. Près de la moitié des enfants ont un suivi mixte, ce qui souligne la complémentarité des acteurs et pose la question de la définition du socle de compétence et de la formation de chacun, ainsi que d’un usage des soignants de type libéral (basé sur l’offre et la demande) ou orienté par la Santé Publique.

Book excerpt: Introduction : la participation du père au suivi médical de son enfant a été encouragée par des évolutions dans les domaines juridique, sociologique et médical. L'implication paternelle semble apporter un bénéfice à la santé de l'enfant et au vécu de la mère pendant la période périnatale. Peu de pères assistent néanmoins aux consultations pédiatriques ambulatoires. Objectif : décrire et analyser les facteurs qui déterminent les pères à assister aux consultations de suivi de leur enfant dans sa première année de vie. Méthode : étude qualitative par entretiens semi-dirigés auprès de dix pères qui ont participé aux consultations de suivi de leur enfant, pendant sa première année, au cabinet de médecine générale. Analyse inspirée de la méthode par théorisation ancrée. Résultats : les pères interrogés ont relié leur présence en consultation à leur appartenance à une nouvelle génération prônant une répartition plus paritaire des rôles parentaux. Des éléments biographiques divers, parfois en lien avec la santé, ont eu une influence sur leur participation. Le médecin généraliste est apparu comme un interlocuteur de choix pour répondre à leur besoin d'acquérir des compétences dans le soin à l'enfant, d'être rassuré sur sa santé et d'aborder certaines difficultés plus personnelles. La présence en consultation marquait aussi leur investissement dans l'entité familiale, auprès de la mère et de l'enfant. Des éléments contextuels, individuels et reliés au médecin semblaient favoriser ou au contraire freiner leur participation. Conclusion : ce travail a permis de mieux appréhender les raisons qui amènent les pères à assister aux consultations ainsi que leur expérience du suivi médical. La prise en compte de ces données serait susceptible de consolider la relation de soin avec les pères présents en consultation. Les déterminants de leur participation et les facteurs qui les modulent pourront contribuer à l'élaboration de futurs travaux de recherche.

Book excerpt: Recent years have seen important developments in the computer and game industry, including the emergence of the concept of serious games. It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? What are the possible links between such tools and social networks? Is there a gap between evidence-based results and market development? Are there any quality challenges? What future developments and studies are needed in the field?

Under the rubric of violence against women the two overlapping issues have received increasing attention. Much of the response to these forms of violence has focussed either on advocacy, providing adequate health and legal services and ensuring safety of victims once violence is disclosed, or on screening for violence in the context of health services. When the word "prevention" is used, it is usually in reference to secondary and tertiary prevention - helping women get out of violent situations and preventing further violence. Little attention has been given to primary prevention - addressing the root causes with the goal of reducing the number of new cases. Recent hallmark publications have called for increased investment in primary prevention. As part of the Global Campaign for Violence Prevention, WHO is giving increased attention to the primary prevention of intimate partner violence and sexual violence. Preventing intimate partner and sexual violence against women: taking action and generating evidence is an important new tool for violence prevention researchers, practitioners and advocates provides a planning framework for developing policies and programmes for the prevention of intimate partner and sexual violence. This guide outlines the nature, magnitude, risks and consequences of intimate partner and sexual violence. It outlines strategies to prevent these forms of violence against women and describes how these can be tailored to the needs, capacities and resources of particular settings. It also emphasizes the importance of integrating scientific evaluation into all prevention activities in order to expand current knowledge of what works. The manual describes interventions of known effectiveness, those supported by emerging evidence, and those that could potentially be effective, but have yet to be sufficiently evaluated for their impact. For instance, school-based programmes to prevent violence occurring in "dating relationships" have been shown to be effective in randomized trials in the USA and Canada. Evidence is emerging for the effectiveness of empowerment approaches which use microfinance combined with gender-equality training or are based on communications and relationship skills training such as the Stepping Stones training package, which has been widely used in low- and middle-income countries. Strategies that aim to prevent intimate partner and sexual violence through reducing the harmful use of alcohol also show promise. A six-step framework for planning, implementing and evaluating such prevention programmes and policies is presented. This document is primarily aimed at policy-makers, programme planners and donors from public health and related sectors who seek to advance the prevention of intimate partner and sexual violence against women. In addition to the principal audience, other interested parties will include those working in other government sectors such as education, child welfare, social care, criminal justice and departments of women or gender equality; advocates from civil society organizations; local authorities; environmental and urban planners; and researchers."
